Laparoscopic Paraesophageal Hernia Repair To Mesh or not to Mesh. Systematic Review and Meta-analysis

Summary: This study compared the outcomes of laparoscopic paraesophageal hernia repair with mesh reinforcement or primary suture repair. The results showed that patients undergoing LPEHR had similar early and late recurrence rates regardless of the type of mesh used. However, overall morbidity seemed to be higher in patients repaired with nonabsorbable mesh.

Summary: With an aging population, the number of octogenarians undergoing paraesophageal hernia repair (PEHR) is increasing. Octogenarians are more likely to undergo non-elective operations and have higher postoperative mortality rates. Elective PEHR procedures have better outcomes, but octogenarians still have higher mortality rates, longer lengths of stay, and higher readmission rates within 30 days compared to non-octogenarians.

Summary: This study analyzed outcomes of octogenarians compared to non-octogenarians undergoing paraesophageal hernia repair (PEHR). The study found that octogenarians were more likely to undergo non-elective operations, and had higher mortality, longer length of stay, and higher readmission rates within 30 days compared to non-octogenarian elective patients. However, being an octogenarian for elective PEHR was not an independent predictor of perioperative mortality, while non-elective operation was. The findings suggest that elective PEHR in octogenarians is reasonable but requires a thorough risk-benefit analysis.

Summary: This study reviewed the outcomes and recurrence rates of laparoscopic paraesophageal hernia repair (LPEHR) with mesh reinforced cruroplasty using absorbable mesh. The results showed no significant differences in recurrence rates between different types of absorbable mesh up to 2 years postoperatively, and patient-reported outcomes were equivalent.

Mesh shrinkage after transabdominal preperitoneal inguinal hernia repair

Summary: Synthetic mesh is commonly used for inguinal hernia repair, and it has been found that the mesh contracts after surgery regardless of the material used. This study developed a method for indirectly measuring the postoperative mesh area and found that the mesh shrinks over time without affecting patient outcomes.

Tissue-Adhesive Decellularized Extracellular Matrix Patches Reinforced by a Supramolecular Gelator to Repair Abdominal Wall Defects

Summary: Biodegradable, decellularized extracellular matrix (dECM)-based biological patches reinforced by a supramolecular gelator were developed to treat abdominal wall defects. The reinforced dECM patches had improved mechanical strength and tissue adhesion compared to the original dECM. In vivo experiments demonstrated that the reinforced dECM patches promoted collagen deposition and blood vessel formation, while suppressing the accumulation of CD68-positive macrophages.

Summary: Laparoscopic revision paraesophageal hernia repair (PEHr) is a safe and effective procedure for relieving foregut symptoms associated with paraesophageal hernias. However, some patients may still require revision surgery. This study demonstrates that laparoscopic revision PEHr is associated with a low rate of morbidity and mortality, and may improve quality-of-life outcomes for patients.

Summary: This study describes the incidence and source of contagion of COVID-19 in patients with inflammatory bowel disease (IBD), as well as the risk factors for a severe course and long-term sequelae. The results show that the prognosis of COVID-19 in IBD patients, even in those on immunosuppression, is similar to that in the general population, indicating that there is no need for more strict protection measures in IBD.

Summary: This study uses machine learning methods to explore potential influencing factors of Ustekinumab in the treatment of Crohn's Disease. The study found that less exposure to previous biologics has a positive effect on remission, and age, body mass index, and baseline fecal calprotectin are negative risk factors for both remission and drug survival.

Summary: Comorbidities and occupational exposure are the most relevant factors for COVID-19 in patients with IBD. The risk of COVID-19 does not seem to be increased by immunosuppressants or biologics, but there may be a potential effect of 5-aminosalicylates that requires further investigation.
