Summary: This retrospective study compared postoperative pain and analgesic consumption in obese patients undergoing transvaginal hybrid NOTES cholecystectomy (NC) or traditional laparoscopic cholecystectomy (LC). The results showed that the NC group had less postoperative pain and lower analgesic requirements compared to the LC group.

Summary: A recent study compared conventional laparoscopic cholecystectomy with hybrid transvaginal natural orifice transluminal endoscopic surgery-cholecystectomy (TV NOTES-C). The patients undergoing TV NOTES-C experienced less postoperative pain and required less analgesics, had shorter hospital stays, and returned to normal activities faster. They also reported better cosmetic results and higher satisfaction compared to the conventional group.

Summary: The study compared the postoperative outcomes of patients who had undergone previous hysterectomy (PH) and those who had not (NH) after laparoscopic cholecystectomy. While PH patients were older and required more trocars and longer procedure time, there was no significant difference in intraoperative and postoperative complication rates compared to NH patients.

Summary: Assessing kidney function is vital for diagnosing and managing kidney diseases. The widely used indicator, glomerular filtration rate (GFR), has challenges in direct measurement. Serum creatinine, a commonly used marker, has limitations in accuracy. Cystatin C, unaffected by muscle mass, shows stronger associations with cardiovascular disease and mortality. Equations have been developed combining creatinine and cystatin C to estimate GFR, improving accuracy. Cystatin C-based estimates outperform creatinine-based estimates in predicting outcomes, especially in elderly and non-white ethnic groups, aiding in early detection of kidney disease and guiding decision-making.

Summary: This study compared the ability of an AI-based platform using deep neural networks and a conventional platform to detect rhythm abnormalities. The AI-based analysis strategy was found to be faster and at least as accurate as the conventional analysis, with slight differences in detecting atrial fibrillation and ventricular tachycardia.

Summary: The study aimed to investigate the use of spectral computed tomography (SCT) hybrid images in patients with hypervascular abdominal tumors. The results showed that hybrid images have the potential to efficiently exploit the additional information provided by SCT, but rigid weighting did not significantly improve diagnostic performance in this study.

Summary: Most Spanish specialists in the clinical management of spinal cord compression are familiar with scoring systems and guidelines, but they do not always apply them in practice. There is room for improvement in the management of spinal metastatic disease (SMD). The correct interpretation rate of scoring systems for spine instability and spinal cord compression is relatively low.

Summary: Bilateral adrenal hemorrhage is a rare but important condition that can lead to adrenal insufficiency if not promptly recognized and treated. Diagnosing this condition can be challenging due to its generalized and nonspecific symptoms. We present two cases of patients with acute abdominal pain who were found to have bilateral adrenal hemorrhage through laboratory tests and CT scans, and were subsequently diagnosed with adrenal insufficiency and treated accordingly.

Summary: The study investigated the prevalence of incidental findings on transvaginal ultrasound scans in women referred with pelvic organ prolapse, finding that the incidence of incidental ultrasound findings was low and predominantly benign. However, these findings sometimes resulted in further investigations and changes in treatment plans.

Summary: This article discusses the often neglected issue of olfactory testing in clinical practice and highlights the significant limitations that loss of smell can have on everyday life. The authors summarize relevant studies and experiences, providing an overview of the possible causes of olfactory impairment and the methods used to test and diagnose the condition. The article also explores the link between nasal polyposis, chronic rhinitis, and olfactory impairment.

Summary: This study presents two cases of subclinical Crohn's disease diagnosed after ingestion of fruit pits leading to bowel obstruction, and reviews the relevant literature. It demonstrates the increased risk of bowel obstruction in Crohn's disease patients due to the ingestion of fruit pits, which may result in atypical presentations of the disease.

Summary: This article compared different methods for the treatment of distal malignant biliary obstruction (DMBO) after endoscopic retrograde cholangiopancreatography (ERCP) failure. According to the available studies, none of these methods seems to be superior to another, although percutaneous trans-hepatic biliary drainage (PTBD) seems to have a slightly higher rate of adverse events. Therefore, all these methods appear to be equally effective for drainage of DMBO, although EUS-guided approaches could be less invasive and have fewer adverse events.

Summary: EUS-guided biliary drainage (EUS-BD) is an effective method for palliative treatment of malignant biliary obstructions, especially in patients for whom ERCP has failed. This review compares various techniques and concludes that EUS-BD, in expert hands, is a minimally invasive and effective option for these patients.

Summary: Through a systematic literature search and critical review, it was found that endoscopic ultrasound-guided biliary drainage (EUS-BD) could become the preferred treatment for concurrent malignant biliary obstruction (MBO) and gastric outlet obstruction (MGOO). EUS-GEA was also found to be an effective option for MGOO treatment.

Summary: This systematic review examined available biliopancreatic endoscopic techniques in altered post-surgical anatomy of the upper GI tract. Various successful techniques, with high success rates and low adverse events, have been developed over the years for biliopancreatic diseases. Researchers recommend considering novel techniques when performing biliopancreatic endoscopy in patients with altered anatomy.
