Utilizing BRAF(V600E) mutation as a marker may reduce unnecessary prophylactic central neck dissection (pCND) in clinically nodal negative (cN0) neck for small <= 2 cm) classical papillary thyroid carcinoma (PTC). We aimed to assess whether BRAF is a significant independent predictor of occult central nodal metastasis (CNM) and its contribution to the overall prediction after adjusting for other significant preoperative clinical factors in small PTC. Primary tumor tissue (paraffin-embedded) from 845 patients with small classical cN0 PTC who underwent pCND was tested for BRAF mutation. Clinicopathologic factors were compared between those with and without BRAF. BRAF was evaluated to see if it was an independent factor for CNM. Prediction scores were generated using logistic regression models and their predictability was measured by the area under the ROC curve (AUC). The prevalence of BRAF was 628/845 (74.3%) while the rate of CNM was 285/845 (33.7%). Male sex (odds ratio (OR): 2.68, 95% CI: 1.71-4.20), large tumor size (OR: 2.68, 95% CI: 1.80-4.00), multifocality (OR: 1.49, 95% CI: 1.07-2.09), lymphovascular permeation (OR: 10.40, 95% CI: 5.18-20.88), and BRAF (OR: 1.65, 95% CI: 1.10-2.46) were significant independent predictors of CNM, while coexisting Hashimoto's thyroiditis (OR: 0.56, 95% CI: 0.40-0.80) was an independent protective factor. The AUC for prediction score based on tumor size and male sex was similar to that of prediction score based on tumor size, male sex, and BRAF status (0.68 vs 0.69, P=0.60). Although BRAF was an independent predictor of CNM, knowing its status did not substantially improve the overall prediction. A simpler prediction score based on male sex and tumor size might be sufficient.

Summary: Based on electronic health records in Hong Kong, the risk of Bell's palsy increased following BNT162b2 vaccination, particularly within 14 days after the second dose. However, the absolute risk remains very low, so the benefits of COVID-19 vaccination still outweigh the small risk of Bell's palsy.

Summary: In this study, it was found that a third dose of mRNA vaccine significantly improved antibody levels against the ancestral virus and the Omicron variant in Chinese adults aged >= 30 years who had previously received 2 doses of inactivated COVID-19 vaccine. The third dose also had a well-tolerated safety profile.

Summary: Despite low circulation of influenza, 42 cases were identified among 15,026 travelers in quarantine in Darwin, Australia, in 2020. Testing during quarantine is an important source of viruses for influenza vaccine strain selection and underscores the potential for influenza epidemics as global travel resumes.

Summary: Introduction and Objectives: Operations are a major cause of sodium-glucose co-transporter 2 inhibitor (SGLT2i)-associated diabetic ketoacidosis (DKA). This study aimed to investigate the risks of SGLT2i-associated postoperative DKA. Methods: A population-based cohort of patients with type 2 diabetes who underwent operations during 2015-2020 was analyzed. Results: Preoperative SGLT2i use was associated with increased risks of postoperative DKA in patients with type 2 diabetes. Conclusion: Clinicians could optimize patients' outcomes by appropriate prescription of SGLT2i, while watching out for high-risk features. Implementing automatic electronic health record pop-up alerts may reduce the risk of SGLT2i-associated postoperative DKA.

Summary: We administered a third dose of BNT162b2 to 314 adults aged >= 30 years who had previously received 2 doses of inactivated vaccine. Blood samples were collected before the third dose, after 1 month, and after 6 months. The study found robust antibody responses to the ancestral strain at 6 months after receiving BNT162b2, but weaker and declining antibody responses to Omicron BA.2 after the third dose.

Discharge of postoperative patients with an opioid prescription is associated with increased persistent opioid use, healthcare expenditures and mortality: a retrospective cohort study

Xiaodong Liu, Carlos K. H. Wong, Tingting Wu, Eric H. M. Tang, Ivan C. H. Au, Lanlan Li, Chi W. Cheung, Brian H. -H. Lang

Summary: This is a retrospective cohort study investigating the risk factors for persistent opioid use after surgical discharge and the association between opioid prescription at discharge and postoperative emergency department visits, readmission, and mortality. The study found that prescribing opioids on discharge was associated with an increased risk of persistent opioid use, 30-day emergency department visits, 30-day readmission, and 30-day all-cause mortality. Therefore, minimizing opioid prescriptions at discharge could improve perioperative patient outcomes.

User Experience of Augmented Reality Glasses-based Tele-Exercise in Elderly Women

Inhwa Yoo, Hyoun-Joong Kong, HyunJin Joo, Yeonjin Choi, Suk Wha Kim, Kyu Eun Lee, Jeeyoung Hong

Summary: This study aimed to compare the user experience differences of elderly women participating in remote sports exercise programs using tablet and augmented reality (AR) glasses. The results showed no significant differences in attractiveness, stimulation, and novelty between the two groups, but there were significant differences in efficiency, perspicuity, and dependability. Therefore, when developing AR glasses-based exercise programs for the elderly, the efficiency, clarity, and stability of the program should be considered to meet participants' needs.

Bilateral axillo-breast approach robotic thyroidectomy in pediatric patients with thyroid disease and cancer

Ja Kyung Lee, Jee-Hye Choi, Woochul Kim, JungHak Kwak, Hyeong Won Yu, Su-jin Kim, Young Jun Chai, June Young Choi, Kyu Eun Lee

Summary: This study retrospectively evaluated the clinical characteristics and surgical outcomes of 26 pediatric patients aged 5-18 years who underwent BABA robotic thyroidectomy. The operation time and surgical completeness were similar to robotic thyroidectomy in adults, and most complications were minor and transient. Therefore, robotic thyroidectomy can be considered as a surgical option regardless of patient age or diagnosis.

Summary: During extreme events like COVID-19 crisis, people tend to engage in collective online behaviors to express awareness, take action, and work through concerns. This study in Hong Kong SAR during the first two waves of COVID-19 provides a framework for evaluating the interactions among individuals' emotions, perceptions, and online behaviors. The study results show a strong correlation between online behaviors, such as Google searches, and the real-time reproduction numbers.

Hospitalizations and mortality during the first year of the COVID-19 pandemic in Hong Kong, China: An observational study

Hualei Xin, Peng Wu, Jessica Y. Wong, Justin K. Cheung, Eric H. Y. Lau, Gabriel M. Leung, Benjamin J. Cowling, Joshua Nealon

Summary: Hong Kong implemented a strict COVID-19 elimination strategy in 2020. This study estimated the impact of pandemic responses on hospitalizations and deaths. The findings showed a reduction in hospitalizations and an increase in deaths, potentially due to changes in healthcare seeking behavior and the vulnerability of certain population groups. Better healthcare planning is necessary during public health emergencies.
