Summary: The 5-year incidence of diaphragmatic herniation following esophagectomy for cancer (DHEC) is 10.3% and is associated with a favorable prognosis. Risk factors for DHEC include presence of hiatal hernia, previous hiatus surgery, gastroesophageal junction tumor location, neoadjuvant chemoradiotherapy, and minimally invasive abdominal phase. Surgical repair of DHEC is associated with acceptable morbidity, but recurrence rate is high.

Summary: The study aimed to determine the clinical characteristics of patients who achieved pathologic complete response (pCR) after neoadjuvant chemotherapy (NACT) and to identify predictive or prognostic factors associated with pCR. The results showed that women with pCR after NACT had significantly longer recurrence-free survival (RFS) compared to those with residual viable tumor at the time of interval tumor-reductive surgery, and CA125 may serve as a biomarker for identifying these extreme responders preoperatively.

Summary: In this exploratory analysis of patients with platinum-sensitive recurrent ovarian cancer, we assessed postoperative complications after secondary cytoreductive surgery (SCS) with or without hyperthermic intraperitoneal chemotherapy (HIPEC). The results showed that 40% of the patients had grade >= 3 complications and 60% had grade < 3 complications, with anemia and abdominal infections being the most common. The complications were associated with slight delays in chemotherapy initiation but did not significantly impact oncologic outcomes.

The researchers used post-chemotherapy contrast-enhanced CT scans to delineate the residual masses and obtained tumor textures using LifeX software. They constructed a radiomics score based on eight texture features and evaluated its performance in predicting malignancy. The results suggest that the radiomics score can help predict the malignant character of residual post-chemotherapy masses in NSGCTs before surgery, thus limiting overtreatment.

Patients treated with adjuvant radiotherapy showed improved 5-year overall survival (OS) and a trend towards enhanced 5-year progression-free survival (PFS). However, additional chemotherapy in cases of serous carcinoma with poor histological prognostic factors should be cautiously considered due to similar OS and PFS but increased toxicity.
