The value of MRI compared to mammography in the assessment of tumour extent in invasive lobular carcinoma of the breast

Aims: Invasive lobular carcinoma of the breast (ILC) is known to be substantially underestimated by mammography, which makes correct planning of treatment difficult. MRI has been proposed as a valuable adjunct to mammography. The purpose of the current study is to evaluate its value, compare it to mammography and assess the possible causes of over- and underestimation of lesion size on MRI. Method: The mammograms and MRI scans of 67 consecutive patients with ILC were retrieved and re-evaluated. Size measurements were correlated to the sizes extracted from the pathology report. Results: MRI measurements correlated better to pathologic size (r = 0.85) than mammographic measurements (r = 0.27). Underestimation of tumour size was more common on mammography (p < 0.001); overestimation occurred with equal frequency (p = 0.69). Overestimation on MRI, caused by non-malignant findings, was attributed to enhancing lobular carcinoma in situ. Conclusion: MRI is a more accurate modality for determining tumour size in patients with ILC than mammography. The typical underestimation of lesion size by mammography can be prevented with the aid of MRI, without increasing the risk of lesion overestimation. (C) 2007 Elsevier Ltd. All rights reserved.

Summary: Accurate assessment of lymph node metastases in prostate cancer patients is crucial. PSMA PET/CT and nano-MRI are imaging modalities with high potential for identifying LN metastases. This study compared the results of these modalities, finding that nano-MRI detected more suspicious LNs, especially smaller ones, across all anatomical regions of the pelvis. It suggests that nano-MRI could complement PSMA PET/CT in imaging PCa patients, but further investigation is needed.

Summary: This study evaluated the response of adjacent DCIS to NST-containing HER2-blockade in HER2-positive breast cancer patients and identified predictive factors for DCIS response, including absence of suspicious calcifications on pre-NST mammography, dual HER2-blockade treatment, near complete response on MRI, absence of calcifications and Ki-67 > 20% in DCIS on pre-NST biopsy. These findings suggest that mastectomy may not always be necessary for patients with extensive DCIS in HER2-positive breast cancer before NST.

Summary: The study evaluated the outcomes of pre-biopsy MRI pathways in biopsy-naive men with suspicion of prostate cancer in routine clinical practice. The results showed that pre-biopsy MRI can be used as a decision tool without compromising cancer detection rates and with a higher biopsy avoidance rate. There were comparable cancer detection and complication rates between TR- and TP-MRDB.

The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.

Neoadjuvant chemotherapy followed by surgery for HPV-associated tonsillar cancer: Does imaging reflect the pathological response?

Summary: This study evaluated the outcome of tonsillar cancer managed with neoadjuvant chemotherapy followed by surgery. The results showed that neoadjuvant chemotherapy reduced tumor volume and pathological adverse features, significantly decreasing the need for adjuvant therapy. A greater reduction in tumor volume predicted a complete pathologic response. There was no significant difference in survival rates between the groups.
