Impaired Opening of the Upper Esophageal Sphincter in Patients with Medullary Infarctions

The aim of this study was to report on nine dysphagic patients with medullary infarction and to evaluate swallowing characteristics based on the location of the lesions.We retrospectively reviewed the medical records of these nine patients. The medullary lesions were midlateral (three patients), dorsolateral (one patient), inferodorsolateral (four patients), and paramedian (one patient). The levels of the lesions were upper (four patients), middle (two patients), upper and middle (two patients), and middle and lower medulla (one patient). Dysphagia after medullary infarction was more common in patients with upper or middle medullary level and dorsolateral medullary level lesions. The common findings on videofluoroscopic swallowing studies in patients with lateral medullary infarctions were impaired upper esophageal sphincter opening, aspiration from pyriform sinuses' residue caused by pharyngeal weakness, and multiple swallowing to clear boluses from the pharynx to the esophagus. In patients with medullary infarctions, the lesion levels and loci and their related clinical findings can be useful in predicting dysphagia and aspiration. Because severe dysphagia with serious complication is very common in patients with medullary infarctions, active diagnostic and therapeutic approaches are needed.

DNA Extracted from Cytologic Slides Is a Valuable Source for PCR-Based Molecular Tests

Summary: This study evaluated the feasibility of using DNA extracted from differently prepared cytologic slides for PCR-based molecular tests. The results showed that mutation testing worked well for most samples, except for CytoRich Red Preservative-fixed slides. This indicates that stained and unstained cytologic slides are a suitable source for PCR-based molecular tests, as long as they are fixed in ethanol or stored in an air-dried condition for a short time.

Summary: This study aims to compare the effect of providing financial incentives to post-MI patients referred for outpatient cardiac rehabilitation (CR) and to confirm the effect of increasing CR participation and completion rates. It will be a single-blind, pragmatic, randomized controlled trial conducted at 2 tertiary hospitals. The primary outcomes will be used to assess the CR participation rate and completion, as well as compare changes in cardiorespiratory function, activity scale index, EuroQol 5 dimensions, and patient health questionnaire.

Clinical and endoscopic characteristics of sessile serrated lesions with dysplasia/carcinoma

Summary: This study retrospectively collected clinical, endoscopic, and pathological data of 254 SSLs and found that SSLs patients with older age, metabolic diseases, and high-risk adenomas were at higher risk of dysplasia and carcinoma. Additionally, SSLs located in the distal colon, with larger size, polypoid morphology, surface-changes, no mucus cap, and categorized as NICE type 2/3 were also associated with an increased risk of dysplasia and carcinoma in SSLs. Therefore, the clinical and endoscopic characteristics can aid in the diagnosis and management of SSLs with dysplasia and carcinoma.

Postmortem injury evaluation using the Forensic Injury Severity Score Template (FISST): a preliminary study

Summary: The Abbreviated Injury Scale (AIS) and Injury Severity Score (ISS) are important evaluation tools used in clinical practice to determine the degree of injury in trauma patients. However, their application in forensic medicine is limited. This study introduced a system called the Forensic Injury Severity Score Template (FISST) that can objectively and quantitatively evaluate the severity of postmortem injuries. The FISST scores showed statistically significant differences between different types of deaths and trauma-related death subtypes.

Orthostatic tremor after knee contusion without head trauma: A rare case report

NOL4 showed higher specificity and positive predictive value for lung NENs, making it a potential adjunctive diagnostic marker. Summary: Through a nationwide survey, it was found that most regional medical and public health management institutions in Korea are unprepared for the implementation of community-based cardiac rehabilitation (CR) programs. While many institutions have the necessary facilities for exercise training, there is a lack of patient-monitoring systems during exercise, and institutions are burdened with the cost of establishing personnel and facilities for CR.
