Summary: In this study, the clinical data of 134 cases of CE and 109 cases of DBE were retrospectively analyzed. The results showed that DBE had a higher disease diagnostic rate and suspected intestinal bleeding rate than CE, but required a longer examination time, had a higher incidence of adverse reactions, and a lower examination tolerance than CE.

Summary: This study assessed the impact of videocapsule endoscopy (VCE) and double balloon enteroscopy (DBE) in patients with suspected small bowel bleeding (SSBB). DBE had a significantly higher diagnostic yield compared to VCE, but the concordance between the two procedures was generally suboptimal. Post-procedural hemoglobin values were significantly higher, and there were no differences in diagnostic yield, complications, or hemoglobin values between patients treated with direct oral anticoagulants (DOACs) and non-DOAC patients.

Summary: This study evaluated the indications, diagnostic yield, therapeutic yield, and complications of double-balloon enteroscopy (DBE) in a cohort of consecutive patients according to patients' age. The results showed that DBE had a good diagnostic yield, with vascular lesions and polyps or neoplastic masses being the main findings. Older patients had higher rates of clinically relevant findings and therapeutic intervention. DBE is an efficient and safe diagnostic and therapeutic tool, especially in the elderly population.

Summary: The Lyon Consensus 2.0 provides conclusive criteria for diagnosing gastro-oesophageal reflux disease (GERD) and offers additional metrics to support or refute the diagnosis when primary criteria are inconclusive. A global team evaluated research since the original Lyon Consensus and collaborated on developing statements to update criteria. The updated consensus includes a modern definition of actionable GERD, describes the likelihood of symptoms being related to reflux episodes, and provides diagnostic strategies and testing options based on proven or unproven GERD.

Summary: Epithelial barrier dysfunction is a major characteristic of GERD, which is associated with symptom origination, inflammatory remodeling, and carcinogenesis. Alginate-based antireflux medications have been found to protect against peptic barrier disruption, and this study reveals the molecular mechanisms behind the injury and protection. The findings highlight the potential of targeting regulated intramembrane proteolysis and molecular mechanisms induced by weakly acidic reflux to treat GERD and prevent carcinogenesis.

Summary: Patients with chronic granulomatous disease (CGD) may develop inflammatory bowel disease (IBD), but there is limited information about small bowel disease in this population. This study aimed to determine the prevalence and characteristics of small bowel disease, as well as evaluate the usefulness of video capsule endoscopy (VCE) for diagnosis. The results showed a high prevalence of small bowel IBD in patients with CGD, and radiology and duodenal/ileal disease did not correlate with VCE findings. Therefore, VCE-driven investigation should be considered for small bowel disease in CGD-associated IBD patients, especially those with colonic disease.
