Summary: This study evaluated the extent of ulcer healing and clinical effectiveness of proton pump inhibitor (PPI) administration for ulcerative differentiated early gastric cancer (EGC) before endoscopic submucosal dissection (ESD). Results showed that partial or complete ulcer healing following PPI administration predicts differentiated EGC confined to the mucosa.

Summary: This study evaluated the severity of intraoperative bleeding and analyzed the risk factors of postoperative bleeding associated with endoscopic submucosal dissection (ESD) for gastric neoplastic lesions. It found that proximal location of lesions was a significant risk factor of major intraoperative bleeding, while chronic kidney disease and intraoperative bleeding were independent risk factors of postoperative bleeding.

Summary: Increasing blood pressure under hemostasis during gastric ESD to identify and coagulate potential bleeding spots can reduce the risk of postoperative bleeding. Deeper tumor invasion is associated with a higher risk of post-ESD bleeding, and taking proton pump inhibitors three times a day may be more effective than twice a day.

Summary: Proton pump inhibitors are widely used for the treatment of acid-related gastrointestinal disorders and stress ulcer prophylaxis. However, their use may lead to significant complications such as fractures, Clostridium difficile infection, acute kidney injury, chronic kidney disease, and hypomagnesemia. In the hospital setting, proton pump inhibitors are often overused and inappropriately continued after discharge.

Summary: This study investigated the preventive efficacy of carbazochrome sodium sulfonate (CSS) for post-endoscopic submucosal dissection (ESD) bleeding. The results showed that CSS administration was not effective in preventing post-ESD bleeding in both the overall patient population and higher-risk patients.

Summary: The study evaluated the effectiveness of PPI alone, PPI + rebamipide combination therapy, and H2RA + rebamipide combination therapy in promoting ulcer healing after ESD, with PPI + rebamipide being more effective in reducing ulcer residual ratio. There were no significant differences in ulcer stage or delayed bleeding among the groups.

Summary: This study reports a hybrid hydrogel for the treatment of gastric ulcers induced by endoscopic submucosal dissection (ESD). The hydrogel, loaded with drug-carrying nanoparticles, showed unique stability in gastric tissue and released the drug slowly, inducing polarization of macrophages and promoting gastric ulcer healing in an in vivo model.

Summary: This study reports a two-component in-situ hydrogel prepared from maleimide-based oxidized sodium alginate and sulfhydryl carboxymethyl-chitosan, which gelates mainly based on click chemistry and Schiff base reaction. The hydrogel showed short gelation time, outstanding tissue adhesion, favorable hemostatic properties, and good biocompatibility. Animal experiments demonstrated that the hydrogel provides appropriate mucosal uplift height and durable maintenance time, and significantly accelerates wound healing and repair.

Summary: This study aimed to evaluate the factors involved in the interruption of colorectal ESD, finding that 0-I type tumors and MRS were significant factors. Colon location, deep submucosal tumor invasion, and intermediate invasive growth pattern were also associated with interruption. Observation of these factors can help predict and potentially prevent interruptions during the ESD procedure.

Summary: This study compared the clinical outcomes of marginal ulcer bleeding (MUB) with peptic ulcer bleeding (PUB). The results showed that MUB had a higher rebleeding rate and a higher mortality rate compared to PUB. Prophylactic use of proton pump inhibitors (PPIs) may reduce the risk of rebleeding in MUB.

Summary: This retrospective analysis of 145 cases of endoscopic submucosal dissection for duodenal neoplasms (D-ESD) examined the management and healing process of ulcers. The study found a decrease in delayed adverse events with complete ulcer suture after D-ESD. Additionally, a minimum duration of 3 weeks of proton-pump inhibitors (PPIs) may be necessary for proper healing.

Summary: This study demonstrated that adding endocytoscopy (ECS) to conventional white light imaging (WLI) improved the biopsy sensitivity and eosinophil detection in patients with eosinophilic esophagitis (EE). The sensitivity of a single biopsy specimen and the eosinophil count were significantly higher in the ECS group compared to the WLI group. ECS-based biopsy also had higher sensitivity for the diagnosis of edema and linear furrows compared to WLI.

Summary: This study discovered the correlation between 3D genome structures and molecular subtypes of pancreatic cancer (PC) through Hi-C analysis. They found that HNF1B regulates the expression of related genes by influencing the 3D structures. These findings are significant for a better understanding of the molecular features of PC.

Summary: This study reviewed the data of 924 patients with IgG4-SC from a nationwide survey in Japan and found that 15% of these patients developed cancer. The risk of cancer development was significantly influenced by relapse, and most cancers occurred within 8 years after the diagnosis of IgG4-SC, with the highest risks in pancreatic and biliary cancers. Maintenance steroid treatment had a positive impact on the prognosis of these patients.

Summary: This study aimed to investigate the incidence and risk factors of interstitial lung disease (ILD) in patients with pancreatic cancer (PC) receiving gemcitabine plus nab-paclitaxel chemotherapy. The researchers found that ILD occurred in 2.7% of patients receiving this chemotherapy regimen and was associated with older age. Therefore, monitoring ILD-related symptoms is necessary when administering gemcitabine plus nab-paclitaxel to older patients with PC.

Summary: Colonic diverticular bleeding is a common cause of lower gastrointestinal bleeding. This study aimed to investigate the association between 24-h blood pressure and diverticular rebleeding. The findings suggest that lower blood pressure in the early morning and a higher pre-awakening surge are risk factors for diverticular rebleeding.
