Flat and depressed (nonpolypoid) colorectal lesions have been described for over two decades by Japanese investigators. These neoplastic lesions are typically smaller than polypoid ones and can be more difficult to identify during screening colonoscopy. In particular, depressed type colorectal lesions are usually small in size, with a number of studies showing them to be at greater risk for developing high-grade dysplasia or submucosal invasive cancer. It has also been suggested that they may follow a different carcinogenic pathway to flat elevated or protruding adenomas. This paper summarizes recent data of nonpolypoid colorectal neoplasms from Western and Asian countries.

Summary: The guidelines for colonoscopy screening and surveillance were developed by the Japan Gastroenterological Endoscopy Society based on scientific methods and expert consensus. Limited high-level evidence in this field required a focus on expert opinions. The guidelines consist of 20 clinical questions and eight background knowledge topics as the current guiding principles.

Post-polypectomy surveillance: the present and the future

Summary: An effective and safe post-polypectomy surveillance program should consider risk stratification based on baseline colonoscopy results. Major international guidelines have recommended and updated risk-stratified surveillance programs, with more individuals being classified as low-risk for less frequent or no surveillance. The new colonoscopy screening and surveillance guidelines in Japan also incorporate risk-stratified post-polypectomy surveillance.
