Prospective Assessment of Risk of Bacteremia Following Colorectal Stent Placement

Colorectal stent insertion is an invasive endoscopic procedure. However, there are no reports regarding the incidence of bacteremia with colorectal stent. This study was to evaluate the risk of bacteremia and infectious complications after stent insertion for colorectal obstruction. Patients who underwent colorectal stent insertion were enrolled consecutively. Blood cultures were obtained before colorectal stent insertion and at 30 min after the procedure. Patients were monitored for 48 h after colorectal stent insertion to detect the development of infectious complications. Procedural data collected included location of obstruction, degree of bowel preparation, obstructive symptoms, and the time required for the procedure. Of 64 patients undergoing colorectal stent, four (6.3%) had a positive post-stent blood culture. All patients, including those with positive cultures, remained asymptomatic during the 48 h after the procedure. Site of obstruction, degree of bowel preparation, age, and underlying disease were not different between the two groups. Development of bacteremia was associated with long procedure time (p < 0.05). Colorectal stent insertion does not induce significant bacteremia in patients with colorectal obstruction. These findings suggest that the routine use of prophylactic antibiotics may not be necessary in colorectal stent insertion.

Summary: The study found that the use of CSI-EMR does not increase the complete or en bloc resection rate of colorectal lesions with SSL endoscopic features, but does increase the R0 resection rate. Although the procedure time is longer for CSI-EMR, there is no significant difference in adverse events or recurrence rate compared to EMR. Further evaluation is needed to assess the association of CSI-EMR with R0 resection and non-recurrence.

Efficacy of bougie dilation for normal diet in benign esophageal stricture

Summary: This study aimed to identify the clinical and endoscopic factors affecting the achievement of a normal diet with only bougie dilation in patients with benign esophageal stricture (BES). Results showed that approximately one-third of BES patients can achieve a normal diet through fewer than four dilation sessions, short length of stricture, pre-procedural dysphagia status, and larger dilator diameter.

Tumor heterogeneity and carcinoma in resected specimens of gastric low-grade dysplasia: A retrospective single center study

Summary: This study retrospectively analyzed endoscopic resection data from January 1999 to December 2019 to investigate the risk factors for pathological upgrades of gastric low-grade dysplasia (LGD) to early gastric cancer (EGC) or high-grade dysplasia (HGD). The results showed that lesion size exceeding 1.5 cm, mucosal nodularity, heterogeneous color, presence of erosion, and open-type gastric atrophy were significantly associated with upgraded pathology to EGC. Among EGC cases, 89.2% had pre-existing dysplasia. Therefore, endoscopic evaluations should be performed in LGDs, especially those exhibiting surface nodularity, erosion, heterogeneous color, and large size, to prevent possible pathological upgrades and co-existence of carcinomas.

Collagenous Gastoduodenitis in the Form of a Gastric Ulcer

Prognostic value of soluble PD-L1 and exosomal PD-L1 in advanced gastric cancer patients receiving systemic chemotherapy

Summary: The study investigated the prognostic significance of soluble PD-L1 (sPD-L1) and exosomal PD-L1 (exoPD-L1) in patients with gastric cancer receiving chemotherapy. It was found that low levels of sPD-L1 and exoPD-L1 were associated with better overall survival and progression-free survival. Pretreatment sPD-L1 could be used as an independent prognostic factor for overall survival, while exoPD-L1 may reflect the immunosuppressive state of patients.

Accuracy of administrative claim data for gastric adenoma after endoscopic resection

Summary: This study evaluated the diagnostic accuracy of an administrative database for resected gastric adenomas. The study used International Classification of Diseases (ICD) 10-codes to identify gastric adenoma cases. The results showed that the overall diagnostic accuracy of ICD-10 codes for gastric adenoma was 98.7%, with a specificity of 88.5%, positive predictive value of 95.2%, and negative predictive value of 96.8%.
