Summary: This study aimed to investigate the impact of colonoscopy and endoscopy on the risk of periprosthetic joint infection (PJI) after total joint arthroplasty (TJA). The results showed that undergoing colonoscopy within 2 months after total knee arthroplasty (TKA) and endoscopy within 1 month after total hip arthroplasty (THA) significantly increased the odds of PJI. It is recommended to delay invasive endoscopic procedures for at least 2 months after TKA and 1 month after THA to minimize the risk of PJI.

Summary: Lower gastrointestinal endoscopy is the gold standard for diagnosing and removing colonic polyps. However, some patients refuse conventional colonoscopy due to fear of risks and discomfort. Robotic colonoscopy offers a promising alternative, allowing for a painless examination of the colon. Barium enema, computed tomographic colonography, and colon capsule endoscopy are also possible alternatives.

Summary: Gastrointestinal (GI) diseases are a serious threat to human health, and the detection and treatment of gastrointestinal diseases burden medical institutions. This study proposes a novel method using convolutional neural networks to detect gastrointestinal diseases from wireless capsule endoscopy images, achieving better results than existing methods and demonstrating the potential of using deep learning models for effective GI disease screening.

Summary: Acute lower gastrointestinal bleeding (LGIB) is a common and serious condition that requires hospitalization. These revised guidelines provide recommendations for risk stratification, diagnostic testing, therapeutic options, and management of antithrombotic medications for LGIB patients. Important changes include the use of risk stratification tools, the role of reversal agents for anticoagulant patients, the increasing use of computed tomography angiography (CTA), and updated recommendations for resuming medication after LGIB.

Summary: Recent advancements in endoscopic technology have revolutionized the treatment of gastrointestinal diseases, improving patient outcomes and quality of life. While robotics have enhanced maneuverability and treatment capability for endoscopists, further developments are needed to address technical limitations and create more sophisticated instruments. Innovative robotic systems are being developed to eventually facilitate minimally invasive robotic surgery across various clinical fields for lower patient risk.

Summary: More than 17.7 million gastrointestinal endoscopic procedures are performed annually in the United States, with a low risk of adverse events. However, infections can occur, especially due to the patient's gut flora and emerging multi-drug-resistant microorganisms. This systematic review provides an overview of the demographic, clinical, and management data of endoscope-associated infections (EAIs). The review also highlights different prevention methods and the potential of disposable endoscopes to minimize EAIs.

Summary: This study aimed to develop a prediction model for bacteremia in hepatobiliary infection to support the identification and management of bacteremia in the emergency department. The results showed that age, vital signs, and laboratory tests were significant factors for predicting bacteremia. The model achieved good discrimination and showed satisfactory sensitivity and specificity in both internal and external validation.

Summary: The study found that the use of CSI-EMR does not increase the complete or en bloc resection rate of colorectal lesions with SSL endoscopic features, but does increase the R0 resection rate. Although the procedure time is longer for CSI-EMR, there is no significant difference in adverse events or recurrence rate compared to EMR. Further evaluation is needed to assess the association of CSI-EMR with R0 resection and non-recurrence.

Summary: This study retrospectively analyzed endoscopic resection data from January 1999 to December 2019 to investigate the risk factors for pathological upgrades of gastric low-grade dysplasia (LGD) to early gastric cancer (EGC) or high-grade dysplasia (HGD). The results showed that lesion size exceeding 1.5 cm, mucosal nodularity, heterogeneous color, presence of erosion, and open-type gastric atrophy were significantly associated with upgraded pathology to EGC. Among EGC cases, 89.2% had pre-existing dysplasia. Therefore, endoscopic evaluations should be performed in LGDs, especially those exhibiting surface nodularity, erosion, heterogeneous color, and large size, to prevent possible pathological upgrades and co-existence of carcinomas.

Summary: The study investigated the prognostic significance of soluble PD-L1 (sPD-L1) and exosomal PD-L1 (exoPD-L1) in patients with gastric cancer receiving chemotherapy. It was found that low levels of sPD-L1 and exoPD-L1 were associated with better overall survival and progression-free survival. Pretreatment sPD-L1 could be used as an independent prognostic factor for overall survival, while exoPD-L1 may reflect the immunosuppressive state of patients.
