Summary: Hepatocellular carcinoma (HCC) is a common cancer in men and a leading cause of cancer death. Liver transplantation (LT) is a treatment option for HCC patients. Locoregional therapy, such as transarterial chemoembolization and radiofrequency ablation, has been used as a pretreatment before LT. In Western countries, locoregional therapy is used to bridge until transplantation or for downstaging. Recent reports have clarified the effects of bridging and downstaging locoregional therapy before LT.

The results show that repeated pretransplant LRTs are associated with an increased risk of tumor recurrence and inferior patient survival after living donor liver transplantation (LDLT).

Summary: Liver transplantation provides the best survival benefit for hepatocellular carcinoma (HCC) patients who cannot undergo resection. The Milan criteria have been developed to select suitable candidates for transplantation, aiming to improve survival and reduce the risk of HCC recurrence. However, up to 20% of cases experience HCC recurrence after transplantation, posing a significant challenge due to poor prognosis. Various extended criteria have been proposed to accommodate the increasing demand for organs and the associated higher risk of recurrence. Radiologists need to be aware of the possibility of HCC recurrence involving multiple organs after transplantation. Understanding the location and radiologic appearance of recurrent HCC is crucial for choosing the most appropriate therapy.

Favorable Survival with Non-curative Treatments for Patients with Early-Stage Hepatocellular Carcinoma After Liver Transplant Denial

Summary: In this study, patients with early-stage HCC who were denied liver transplant listing due to non-tumor reasons and instead underwent locoregional therapy (LRT) had favorable overall survival, with those with Child-Pugh A cirrhosis showing significantly longer survival compared to those with Child-Pugh B/C cirrhosis. Patients with Child-Pugh A cirrhosis had survival rates approaching the national average for liver transplant recipients, while those with Child-Pugh B/C cirrhosis had significantly worse outcomes.

Efficacy and Safety of Lenvatinib in Hepatocellular Carcinoma Patients with Liver Transplantation: A Case-Control Study

Summary: The study aimed to develop a predictive score for tumor thrombosis in hepatocellular carcinoma patients. Analysis of data from 2243 patients revealed that PVTT was associated with poorer prognosis and correlated with tumor size, growth pattern, and alpha-fetoprotein level. A risk prediction score based on eight variables was developed, which can help clinicians identify patients with more aggressive cancers and higher mortality rates for optimized management of hepatocellular carcinoma.
