In fact, over a quarter of the patients had negative ERCP, sludge, or small stones which could be cleared by flushing/glucagon without further instrumentation. While large stones may require advanced techniques, this represents a small percentage of patients. Surgery-first management offers an efficient alternative for the majority of patients with suspected choledocholithiasis.

Summary: The study demonstrates that endoscopic papillary large balloon dilation (EPLBD) effectively reduces the recurrence rate of common bile duct stone (CBDS) in patients with recurrence. Compared to traditional endoscopic retrograde cholangiopancreatography (ERCP), EPLBD treatment results in higher rates of complete clearance in one session and significantly lowers the CBDS recurrence rate within 2 years and during long-term follow-up. Multiple recurrences are also decreased with EPLBD treatment.

Summary: Even with advanced imaging techniques, the presence of common bile duct sludge or stones can be missed during ERCP. Pre-procedural EUS or MRCP can reduce the proportion of unnecessary ERCPs. A shorter time interval between EUS or MRCP and ERCP increases the detection rate of suspected CBD stones during ERCP.

The results showed good face validity and excellent content validity of this tool, which provides a useful, inexpensive, versatile, and easy tool for training regarding sphincterotomy, precut, and papillectomy.

Summary: The aim of this study was to evaluate the safety and efficacy of a novel self-assembling peptide hemostatic gel for bleeding after EST, and to compare it with a conventional endoscopic hemostasis technique. The results showed that the novel self-assembling peptide hemostatic gel had comparable safety and efficacy to the conventional technique, with shorter procedure time. However, further prospective randomized trials are needed for verification.
