Sustained Impact of Intermittently Scanned Continuous Glucose Monitoring on Treatment Satisfaction and Severe Hypoglycemia in Adults with Type 1 Diabetes (FUTURE): An Analysis in People with Normal and Impaired Awareness of Hypoglycemia

Summary: This study investigates the impact of nationwide reimbursement of intermittently scanned continuous glucose monitoring (isCGM) on adults with type 1 diabetes with impaired or normal awareness of hypoglycemia over 24 months. The results show sustained improvement in severe hypoglycemia, work absenteeism, and hypoglycemia fear after isCGM reimbursement, with increased treatment satisfaction. isCGM is a valuable tool under long-term real-world conditions regardless of hypoglycemia awareness level.

Summary: Randomised controlled trials and other prospective clinical studies have traditionally used HbA1c as a measure of average blood glucose levels, but with the increasing use of continuous glucose monitoring (CGM), CGM-derived metrics are being considered for use in these studies. This consensus statement recommends the use of CGM data in clinical studies to provide additional clinical information beyond HbA1c. Standardizing the collection and reporting of CGM data in clinical trials can enhance the interpretability of the data and inform therapeutic decisions related to hypoglycaemia, postprandial hyperglycaemia, and glucose variability.

Summary: The Belgian Diabetes in Pregnancy follow-up study (BEDIP-FUS) aims to investigate the impact of BMI, adiposity, and glucose intolerance on the metabolic profile and future risk for T2D in women and offspring after a five-year period. The study involves recruiting 375 women-offspring pairs based on the results of glucose challenge and oral glucose tolerance tests during pregnancy. The primary outcomes measured are glucose intolerance in mothers and BMI z-score in offspring. The study is important for individualizing follow-up care for women with different degrees of hyperglycemia during pregnancy and their offspring.

Summary: The ONWARDS 4 trial aimed to compare the efficacy and safety of once-weekly insulin icodec with once-daily insulin glargine U100 in individuals with long-standing type 2 diabetes. The results showed that icodec demonstrated similar improvements in glycaemic control compared to glargine U100, with fewer basal insulin injections and no increase in hypoglycaemic rates. The trial had limitations including its relatively short duration and open-label design.

High-Throughput Analysis of Neutrophil Extracellular Trap Levels in Subtypes of People with Type 1 Diabetes

Summary: Neutrophils, an overlooked innate immune cell type, may play a crucial role in autoimmune diseases like type 1 diabetes. However, studying neutrophil function has been challenging due to disease heterogeneity and a lack of standardized methods. In this study, live-cell imaging was used to analyze neutrophil function and showed that neutrophils behave similarly in type 1 diabetes patients as healthy individuals. This suggests that neutrophils might contribute to autoimmune diseases through a mechanism called NET formation.

Footprint of pancreas infiltrating and circulating immune cells throughout type 1 diabetes development

Summary: This study investigates the dynamics and phenotypes of immune cells infiltrating the pancreas and their changes in peripheral blood during the development of type 1 diabetes (T1D) in non-obese diabetic (NOD) mice. The early stages of T1D are characterized by an influx of dendritic cells and neutrophils in the pancreas. CD4+ and CD8+ T cells migrate from the pancreatic draining lymph nodes to the pancreas, coinciding with increased beta cell autoimmunity and insulitis severity. These findings provide important insights into the disease process and may contribute to the development of new therapies.
