Low hepatitis C virus-viremia prevalence yet continued barriers to direct-acting antiviral treatment in people living with HIV in the Netherlands

Cas J. Isfordink, Colette Smit, Anders Boyd, Marieke J. A. de Regt, Bart J. A. Rijnders, Reinout van Crevel, Robin P. Ackens, Peter Reiss, Joop E. Arends, Marc van der Valk

Summary: Low prevalence of HCV-viremic people with HIV in the Netherlands coincides with widespread DAA uptake. Barriers to DAA uptake are mostly patient-related, and the risk of HCV transmission from untreated individuals in care is low.

Use of a smartphone app to inform healthcare workers of hospital policy during a pandemic such as COVID-19: A mixed methods observational study

R. I. Helou, C. M. Waltmans-den Breejen, J. A. Severin, M. E. J. L. Hulscher, A. Verbon

Summary: This study evaluated a COVID-19 app designed for healthcare workers and its user experience. The results showed that the app was widely used in hospitals and had high user satisfaction. The study suggests that an app with local hospital policies is valuable during a pandemic.

Natural resistance against infections: focus on COVID-19

Mihai G. Netea, Jorge Dominguez-Andres, Frank L. van de Veerdonk, Reinout van Crevel, Bali Pulendran, Jos W. M. van der Meer

Summary: Individuals exposed to SARS-CoV-2 may exhibit natural resistance to infection, with variability in susceptibility observed. Factors contributing to natural resistance include host genetic variants, viral interference, and innate immune responses. Understanding natural resistance can assist in developing better preventive and therapeutic measures for future pandemics.

Survey of delivery of parenteral antimicrobials in non-inpatient settings across Europe

Caroline Emilie, Phebe de Nocker, Nadia Saidani, Mark Gilchrist, R. Andrew Seaton, Sanjay Patel, Guillaume Beraud, Diamantis Kofteridis, Jeroen Schouten, Nathalie Thilly, Marvin Berrevoets, Marlies Hulscher, Franky Buyle, Celine Pulcini

Summary: Delivery of parenteral antimicrobials in non-inpatient settings (DPANS) varies among European countries, with the availability and organization of DPANS being highly heterogeneous. Most countries provide DPANS through specialized outpatient parenteral antimicrobial therapy (OPAT) services or infusion services, but only a few have formal OPAT team structures with specifically trained staff. Some countries rely on ad hoc arrangements for DPANS. The majority of countries cover the costs of DPANS for patients managed by specialized OPAT/infusion services.

Why we prescribe antibiotics for too long in the hospital setting: a systematic scoping review

Robin M. E. Janssen, Anke J. M. Oerlemans, Johannes G. Van der Hoeven, Jaap Ten Oever, Jeroen A. Schouten, Marlies E. J. L. Hulscher

Summary: This review examines the behavioral determinants that contribute to the prolongation of antibiotic therapy in daily hospital practice. The findings reveal that these determinants are largely related to individual healthcare professionals' knowledge, skills, and cognition, as well as their interactions. Additionally, certain determinants are identified that explain the variation in duration for different pathogens, diseases, and patient, professional, or hospital department characteristics.

Samuel Afari-Asiedu, Martha Ali Abdulai, Alma Tostmann, Ellen Boamah-Kaali, Kwaku Poku Asante, Heiman F. L. Wertheim, Marlies Hulscher

Summary: This review examines the effectiveness of interventions aimed at improving antibiotic dispensing practices at the community level in low- and middle-income countries. The results indicate that educational meetings, distribution of educational materials, and educational outreach meetings are effective strategies for improving antibiotic dispensing.

Hand hygiene compliance and its drivers in long-term care facilities; observations and a survey

Anja Haenen, Sabine de Greeff, Andreas Voss, Janine Liefers, Marlies Hulscher, Anita Huis

Summary: Hand hygiene compliance is low in long-term care facilities and is influenced by multiple factors, which vary among different cadres of staff. Strategies to improve hand hygiene should consider these determinants and their variations across different staff positions.

Infection incidence, timing and dose dependency in rheumatoid arthritis patients treated with rituximab: a retrospective cohort study

Lara D. Veeken, Merel A. A. Opdam, Lise M. Verhoef, Calin Popa, Reinout van Crevel, Alfons A. den Broeder

Summary: This study aims to determine the infection incidence in a large real-life population of RA patients using RTX, with special focus on (ultra-)low dosing and time since last infusion. The results showed that the risk of infection was lower with a dose of 200 mg, and infections seemed to occur more frequently in the first two months after infusion, suggesting an association with peak concentration.

Successful addition of topical antibiotic treatment after surgery in treatment-refractory nontuberculous mycobacterial skin and soft tissue infections

Ralf Stemkens, Maarten Cobussen, Wouter Hoefsloot, Erik de Laat, Reinout van Crevel, Rob E. Aarnoutse, Jakko van Ingen

Summary: Treatment of skin and soft tissue infections caused by nontuberculous mycobacteria can be challenging. Negative pressure wound therapy with instillation and dwell time of topical antibiotics can be a successful and well-tolerated treatment option.

Protection against tuberculosis by Bacillus Calmette-Guerin (BCG) vaccination: A historical perspective

Todia P. Setiabudiawan, Remi K. Reurink, Philip C. Hill, Mihai G. Netea, Reinout van Crevel, Valerie A. C. M. Koeken

Summary: BCG, developed 100 years ago, is the only licensed tuberculosis vaccine and is widely used worldwide. However, despite universal vaccination policies, TB burden remains high. The level of protection provided by BCG varies between age groups and settings. This review presents the evidence and factors influencing BCG's ability to protect against TB, as well as the immunological mechanisms involved. Possible strategies to boost BCG's efficacy are also discussed.

Facilitators and barriers to status disclosure and partner testing of women living with HIV in Indonesia: a mixed methods study

Annisa Rahmalia, Rudi Wisaksana, Marie Laga, Reinout van Crevel, Koen Peeters Grietens

Summary: This study examined the disclosure of HIV status and partner testing among women living with HIV in Bandung, Indonesia. The findings showed that women faced barriers in disclosing their status and getting their partners tested. Women with longstanding HIV infections in particular would benefit from partnership counseling and specific guidelines for partner notification and testing.
