Fatty Liver Increases the Association of Metabolic Syndrome With Diabetes and Atherosclerosis

OBJECTIVE-To analyze the participation of fatty liver (FL) in the association of metabolic syndrome (MS) with type 2 diabetes and coronary artery calcification (CAC). RESEARCH DESIGN AND METHODS-A total of 765 subjects (52% women) aged 30 to 75 years without clinical atherosclerosis were included in this study. MS was defined in accordance with the Adult Treatment Panel III (ATPIII) guidelines, while FL and CAC were identified by computed tomography. RESULTS-There were increasing frequencies of type 2 diabetes and CAC in all three groups: control, MS without FL, and MS plus FL. Multivariable-adjusted logistic regression analyses showed that FL increased the association of MS with type 2 diabetes in both women [odds ratio 10.6(95% CI 3.4-33.7)] and men [12.1 (4.1-36.1)]. In women, FL also increased the association of MS with CAC [2.34 (1.07-5.12)]. CONCLUSIONS-FL increases the association of MS with type 2 diabetes and subclinical atherosclerosis.

Summary: This study characterized CD4(+)CD28(null) cells in patients with chronic hyperuricemia and investigated the effect of allopurinol on CD28 expression and the balance of T helper phenotypes. The results showed that hyperuricemic patients had an expanded CD4(+)CD28(null) cell subset with an increase in T-bet(+) cells and a decrease in ROR?t(+) cells. After allopurinol treatment, CD4(+)CD28(null) cells decreased, T-bet(+) cells decreased, GATA-3(+) cells increased, and ROR?t(+) cells increased. These findings suggest that allopurinol partially restores CD28 expression and rebalances T helper phenotypes in chronic hyperuricemia patients.

Summary: This study investigated the expression and role of microRNAs (miRs) in monocytes in coronary arterial disease (CAD). The results showed that miR-21-5p and miR-221-5p were significantly upregulated in CAD patients, while miR-155-5p was significantly downregulated. Furthermore, the upregulation of miR-21-5p and miR-221-5p was associated with an increased risk of CAD.

Summary: This study establishes a dysfunctional adiposity index (DAI) as an early predictor of cardiometabolic abnormalities, independently associated with diseases such as diabetes, non-alcoholic fatty liver disease, subclinical atherosclerosis, and hypertension. By evaluating adipocyte morphology and functionality, and correlating with parameters such as adipocyte mean area, number, and adiponectin/leptin ratio, DAI provides a method for early identification of adipose tissue abnormalities.
