Summary: Mild to moderate psoriasis is commonly treated with topical therapy. This article reviews literature on topical treatments that have been evaluated in clinical trials and real-life studies lasting at least 12 months. The use of calcipotriol/bethamethasone dipropionate foam as maintenance treatment can improve the efficacy and safety of topical therapy in psoriatic patients. Introducing new nonsteroidal topical drugs and adopting proactive therapy appear to be crucial for achieving satisfactory long-term outcomes in psoriasis.

Summary: This article discusses the relationship between psoriasis and the nervous system and analyzes the mechanisms that lead to neurological complications during anticytokine therapies. Biological drugs used in psoriasis have positive therapeutic results but also come with side effects. Serious neurological side effects are most often seen with the use of anti-TNF-alpha drugs, requiring monitoring and early intervention.

Summary: The estimated global prevalence of HBV infection is about 316 million. Despite universal vaccination programs and effective antiviral therapy, HBV infection remains a major cause of cirrhosis and hepatocellular carcinoma. Nucleos(t)ide analogues (NA) have been the preferred treatment for chronic hepatitis B for many years, but the low probability of HBV surface antigen seroclearance requires indefinite treatment. Recent advances in understanding the viral cycle and the role of the immune system have led to the development of new therapeutic approaches and ongoing clinical trials with promising results.

Summary: Erythrodermic psoriasis is a severe and potentially life-threatening form of psoriasis, which is less responsive to conventional therapies. The advent of newer biologics, particularly IL-17 and IL-23 inhibitors, has shown promise in the management of EP. However, there is a lack of high-quality evidence regarding their long-term efficacy and safety, and the management of patients with prior biologic failure remains challenging.

Summary: This study evaluated the effect of biologic agent treatment on serum uric acid levels in patients with psoriasis. The results showed a significant decrease in serum uric acid levels in patients after 3 months of biologic agent treatment. Treatment with specific biologic agents such as adalimumab, infliximab, ixekizumab, secukinumab, and ustekinumab resulted in a significant decrease in uric acid levels.

Summary: Long-term proactive management is superior to reactive management in prolonging time to first relapse, reducing number of relapses, and increasing days in remission in adults with psoriasis vulgaris. Cal/BD foam is well tolerated and should be considered for long-term proactive management of psoriasis patients.

Summary: This study aimed to explore the predictive value of infliximab trough level (ITL) in combination with inflammatory biomarkers on long-term endoscopic outcomes in Crohn's disease (CD) patients during infliximab (IFX) maintenance therapy. The results showed that low ITL and high inflammatory biomarker levels increased the risk of endoscopic activity at different stages of treatment. The combination of ITL and inflammatory biomarkers can moderately predict endoscopic activity. Furthermore, ITL was also associated with endoscopic relapse-free survival.

Summary: This article introduces dermatologists to the noninfectious inflammatory skin diseases associated with pulmonary involvement and emphasizes the importance of screening and monitoring for lung disease in high-risk patients. It also highlights the need for dermatologists to recognize cutaneous manifestations of lung disease and refer patients to pulmonary specialists for appropriate treatment. The article emphasizes the common pulmonary complications and high mortality rates associated with autoimmune connective tissue diseases, sarcoidosis, vasculitis, and autoimmune blistering diseases.

Summary: The skin is equipped with immune cells like mast cells and CD8 T cells which play a role in recognizing and responding to environmental challenges. These cells are essential for maintaining tissue balance and regulating the immune response in skin diseases. Studies have shown that mast cells can influence the migration and function of CD8 T cells, while CD8 T cells can also activate mast cells. The close proximity between mast cells and CD8 T cells has been observed in dermatoses such as alopecia areata. This review summarizes the bidirectional interactions between human mast cells and CD8 T cells, focusing on their involvement in psoriasis, atopic dermatitis, and vitiligo, and discusses unresolved questions.

Summary: This article reviews the similarities and differences between acrodermatitis continua of Hallopeau (ACH) and generalized pustular psoriasis (GPP). IL-36RN mutations are more commonly found in GPP patients, while ACH patients have a lower frequency of these mutations. Although ACH can occur before, simultaneously, or after the development of GPP, the response to treatment appears to differ between the two conditions. Based on the findings, ACH should be classified separately from GPP rather than being considered a subtype of GPP.

Summary: Psoriasis has negative impacts on physical, mental, and social health, leading to reduced quality of life. Current management of psoriasis may not adequately address all aspects of health. In order to restore or maintain health, healthcare should focus on the patient's overall well-being, including life satisfaction, quality of life, and mental well-being. This expert review emphasizes the importance of incorporating well-being into the holistic therapeutic approach for psoriasis patients, and provides recommendations for clinical practice to implement well-being assessment and treatment.
