Recommendations for the long-term treatment of psoriasis with infliximab: A dermatology expert group consensus

Background/Aims: Infliximab has been approved for the treatment of chronic plaque psoriasis for only a few years. As physicians gain confidence in initiating and maintaining this therapy, guidance on the management of patients beyond several months or years is needed. To date, there is little or no information about the long-term management in clinical trials or guidelines. Methods: Here we report on the key aspects related to the use of infliximab for the treatment of psoriasis. The data presented here were derived using a modified Delphi survey to obtain a consensus opinion of 11 dermatologists from Europe and Canada experienced in long-term therapy with infliximab. Results/Conclusion: The Delphi participants reviewed several important topics related to biological therapy and infliximab. This paper is not intended to provide a recommendation on all practical aspects related to biological therapy; it has rather been written to provide useful and practical information on the 'best practice' use of infliximab. It also highlights the need for dermatologists to recognize cutaneous manifestations of lung disease and refer patients to pulmonary specialists for appropriate treatment. The article emphasizes the common pulmonary complications and high mortality rates associated with autoimmune connective tissue diseases, sarcoidosis, vasculitis, and autoimmune blistering diseases.

Exploring Mast Cell-CD8 T Cell Interactions in Inflammatory Skin Diseases

Summary: The skin is equipped with immune cells like mast cells and CD8 T cells which play a role in recognizing and responding to environmental challenges. These cells are essential for maintaining tissue balance and regulating the immune response in skin diseases. Studies have shown that mast cells can influence the migration and function of CD8 T cells, while CD8 T cells can also activate mast cells. The close proximity between mast cells and CD8 T cells has been observed in dermatoses such as alopecia areata. This review summarizes the bidirectional interactions between human mast cells and CD8 T cells, focusing on their involvement in psoriasis, atopic dermatitis, and vitiligo, and discusses unresolved questions.

Prevalence, comorbidities and mortality of generalized pustular psoriasis: A literature review

Summary: Generalized pustular psoriasis (GPP) is a rare auto-inflammatory skin disease with acute episodes of pustule formation. Understanding the epidemiology and clinical data of GPP is challenging due to its rarity. This review article provides an overview of GPP prevalence estimates in key demographics, genetic mutations associated with the disease, incidence of comorbidities, mortality rate, and causes of death.

Acrodermatitis continua of Hallopeau and generalised pustular psoriasis: Should they be the same or different entities

Summary: This article reviews the similarities and differences between acrodermatitis continua of Hallopeau (ACH) and generalized pustular psoriasis (GPP). IL-36RN mutations are more commonly found in GPP patients, while ACH patients have a lower frequency of these mutations. Although ACH can occur before, simultaneously, or after the development of GPP, the response to treatment appears to differ between the two conditions. Based on the findings, ACH should be classified separately from GPP rather than being considered a subtype of GPP.

In order to restore or maintain health, healthcare should focus on the patient's overall well-being, including life satisfaction, quality of life, and mental well-being. This expert review emphasizes the importance of incorporating well-being into the holistic therapeutic approach for psoriasis patients, and provides recommendations for clinical practice to implement well-being assessment and treatment.

Summary: This study compared the differences in sun exposure, sun protection, and level of worry for cutaneous melanoma between high-incidence and low-incidence provinces in Atlantic Canada, as well as between different demographic groups. The findings identified significant differences across multiple variables, which will help guide future public health efforts in preventing and reducing the incidence of cutaneous melanoma in the communities.
