Treatment of keloids with laser-assisted topical steroid delivery: a retrospective study of 23 cases

Topical or intralesional corticosteroids are referred to as gold standard treatments for keloids. Recent studies showed that ablative fractional laser (AFL) treatment facilitates delivery of topical drug deeply into the skin by creating vertical channels. The objective of the present study was to assess the ablative erbium laser in fractionated mode, combined with topical high potent corticosteroid cream for treating resistant keloid scars. We conducted a retrospective study in the laser center of the Department of Dermatology (University Hospital of Nice, France), from January 2010 to June 2012, on patients with keloids who were resistant to a first-line of treatment. A 2940-nm ablative fractional erbium laser was used. Topical betamethasone cream was applied twice a day under occlusion with transparent film dressings. A total of 23 patients with 70 keloids were treated from January 2010 to June 2012. The median percentage of improvement was 50% (range -43 to 84). The mean follow-up was 8 months (range 3-18), and a recurrence was observed for eight lesions (22%). Although this observation warrants a prospective comparative evaluation, it supports the interest of the laser-assisted delivery of steroids for treating keloids scars.

Pigmented demodicosis presenting as lentigo maligna diagnosed by reflectance confocal microscopy

Summary: The presence of pigmented circles around hair follicles is a key feature in diagnosing lentigo maligna and benign pigmented macules. In this case report, reflectance confocal microscopy was used to diagnose pigmented demodicosis instead of lentigo maligna, avoiding an unnecessary biopsy and shedding light on the mechanisms of hyperpigmentation.

Commentary: Facial Aesthetic Dermatological Procedures and Photoprotection in Chinese Populations COMMENT

Summary: The medical literature on aesthetic dermatology has largely overlooked the needs of patients with darker skin types, who have different concerns and priorities. In Chinese individuals, pigment alteration is a major concern as it is associated with aging and a cultural beauty standard of fair skin. Non-invasive cosmetic procedures like lasers and injections are also popular in China, but can lead to hyperpigmentation as a side effect in this population. It is important to consider and tailor approaches to address both primary concerns of photoaging and the side effects of cosmetic treatments.

Impact of house dust mite in vitiligo skin: environmental contribution to increased cutaneous immunity and melanocyte detachment

Summary: Vitiligo is an autoimmune disease characterized by melanocyte loss and white patches on the skin. This study demonstrates that house dust mite (HDM) can induce inflammatory responses and melanocyte detachment in vitiligo through protease activity. HDM may act as an external source of pathogen-associated molecular pattern molecules in vitiligo, and MMP-9 inhibitors could be potential therapeutic targets. Furthermore, studying specific vitiligo endophenotypes can help understand the environmental contribution to the disease.

Summary: This study assessed the socioeconomic status (SES) of HS patients using the French DEPrivation index (FDep) and found a significant association between HS and low SES in the adult population. In children aged 7-17, low SES was associated with obesity and tobacco consumption, but no association with HS was observed after matching for these confounding factors.

Outdoor clinical testing with reference sunscreens to determine differences in skin response between populations of different ethnicity: A combined data analysis from 128 subjects

Summary: This study compared the differences in erythema and pigmentation responses of Chinese and White European individuals to three different sunscreens in outdoor conditions. The findings showed that White Europeans had a higher incidence of erythema response and a higher rate of photoprotection failure, especially with SPF 15 and 30 sunscreens. Therefore, ethnicity should be taken into account when making recommendations for sun safety.

Summary: This study aimed to assess the efficacy and tolerance of a New Trio (NT) combination compared to Kligman's trio (KT) in treating melasma. The results showed that both the NT and KT groups demonstrated significant improvement in mMASI after 12 weeks, with the NT group showing a more significant improvement. Therefore, the NT combination can be considered as an effective treatment option for melasma with better tolerance.
