Summary: This study investigated the costs of Mohs surgery over time and found that the total cost per claim decreased while the patient's out-of-pocket expense increased from 2007 to 2019.

Summary: This study compared the effectiveness of a novel antibiotic-free wound dressing with a triple-antibiotic petrolatum-based ointment in postoperative wound care. The results showed that the antibiotic-free wound dressing group had significantly lower rates of contact dermatitis, better healing time, and higher healing quality compared to the antibiotic group.

Summary: Older age, previous cryotherapy, and multiple prior surgeries were significantly associated with an increased risk of MMS requiring >= 3 stages. Recurrent BCC was associated with the risk of complicated MMS in partially adjusted multivariate analyses, but not in the fully adjusted analyses. Knowledge of these risk factors may help improve the planning of Mohs surgeries.

Summary: This study aimed to evaluate institutional adherence to appropriate use criteria for Mohs micrographic surgery (MMS) and found high adherence but also some instances of inappropriate treatment, primarily due to treating adjacent lesions in the same session.

Summary: This article describes a multidisciplinary approach for treating large EMPD tumors involving critical anatomy in the anogenital region, utilizing MMS for peripheral margin clearance and intraoperative CK7 immunostaining. Clinical pearls for the management of anogenital EMPD are based on 53 multidisciplinary cases treated at the author's institution between 2014 and 2020.

Summary: This study found that dermatologists were fairly accurate at predicting postoperative pain, with their predictions correlated with patient-reported pain. When physicians overestimated pain, they were not more likely to prescribe opioids.

Summary: Most patients undergoing Mohs surgery were willing to attempt home suture removal and were more willing, confident, and less anxious after viewing educational resources. Patients with previous suture removal experience were more likely to attempt removal after their procedure, and 97% of patients who were willing were successful. No major complications were reported.

Summary: This study investigated the 30-day postoperative rate of thrombotic complications in patients with AF or a history of DVT who underwent perioperative DOAC interruption during dermatologic surgery. Results showed that perioperative DOAC interruption appears to be safe and efficacious in dermatologic surgery, with a low incidence of thrombotic complications.

Summary: This study reviews the methods that dermatologists can use to reduce patient anxiety in dermatology offices. Preoperative education through telephone calls, videos, and visual models can help reduce anxiety. Intraoperatively, methods such as music, guided imagery, coloring books, medications, hypnosis, and distraction techniques can be used.

Summary: This retrospective cohort study assessed the incidence of histopathologic upgrading of cSCC during MMS from 2017 to 2019. Male sex, prior field treatment, location on the ear/lip, rapid growth of cSCC, and tumor diameter >= 2 cm were significant predictors of tumor upgrading. Upgraded tumors were more likely to require >= 3 MMS stages, complicated closure, or referral repairs.

Summary: Traditional in-person discussions are commonly used for preoperative education in Mohs micrographic surgery (MMS), but modern techniques like teledermatology consultations, videos, 3D models, pamphlets/online materials, and shared medical appointments are also effective. Analogies and preoperative phone calls are more controversial. Researchers emphasize the need for further studies to provide clear guidelines for Mohs surgeons on educational techniques.

Summary: This study systematically reviewed the reconstructive methods for nasal defects after tumor removal and found a lack of comprehensive and systematic research in the literature. Most of the included studies were case series or small cohort studies, representing low level evidence.

Summary: As the US population ages, safe surgical procedures are necessary for treatment of cutaneous neoplasms in very elderly patients. This study aimed to determine the incidence of complications associated with Mohs micrographic surgery (MMS) in patients aged 85 and older, and the risk factors that predispose to complications. Results showed that independent risk factors associated with a statistically higher incidence of complications were anticoagulant use, extremity location, greater than 2 MMS stages, and flap repair.
