Immunocytochemistry in Europe: results of the European Federation of Cytology Societies (EFCS) inquiry

Immunocytochemistry in Europe: results of the European Federation of Cytology Societies (EFCS) inquiry Objective: This report describes the results of the first chosen topic of the European Federation of Cytology Societies (EFCS) scientific committee, which concerns the application of immunocytochemistry (ICC) to cytological material. While ICC has become an important ancillary method, not only for diagnosis but also to assess prognostic and predictive factors on cytological material, there are many different methodologies used and the lack of standardization has been criticized. This inquiry aimed first to obtain an overview of the techniques used and second to suggest mechanisms for standardization and quality control. Methods: We report the results of 28 replies from 13 countries to a web-based inquiry into types of specimens, preparations and technical methods. Results: Conventional smears were the preparations most commonly used, followed by cytospins, cell blocks and liquid-based preparations, in that order. Avidin-biotin complex, labelled streptavidin-biotin or peroxidase anti-peroxidase were used in 61% and enhanced-polymer technology in 39%. Automated staining techniques were used by 64%. More than half used the same antibody dilutions as histopathology although only 20% used cell blocks. Conclusion: Reduction of variability by using automation, appropriate controls and customized dilution of antibodies according to different samples could improve the quality of ICC and standardize the techniques, along with quality control and quality assurance measures.

Summary: Small cell tumors in the breast, including neuroendocrine neoplasms and lymphomas, present a diverse range of cytological features and should be carefully evaluated with consideration of clinical features. Due to the wide variety of tumors with small cell characteristics in the breast, they should be categorized into neuroendocrine and non-neuroendocrine tumors for proper diagnosis.

Summary: This study evaluated the performance of TBSRTC in the paediatric population and questioned the management recommendation of ATA Guidelines Task Force on Paediatric Thyroid Cancer. The results showed high risk of malignancy in the paediatric population and a high rate of malignancy in operated patients, indicating a need for better preoperative indicators for stratification.

Immunohistochemistry Applied to Breast Cytological Material

Summary: Fine-needle aspiration biopsies (FNABs) of the breast are minimally invasive procedures that can diagnose suspicious breast lesions. When handled properly, FNAB samples can provide sufficient material for ancillary testing. Immunocytochemistry (ICC) can differentiate between different types of cancer, and cellblocks are more versatile than cytology slides. Laboratories without experience should validate these techniques before adopting them.

Searching for SARS-CoV-2 in Cancer Tissues: Results of an Extensive Methodologic Approach based on ACE2 and Furin Expression

Summary: This study evaluated the expression of ACE2 and furin in cancer cells and found that colon and gastric carcinoma cells have high expression levels of ACE2 and furin, while thyroid carcinoma cells do not express ACE2. Although some cases tested positive for SARS-CoV-2 by tissue section PCR, immunohistochemistry and in situ hybridization did not show viral presence in the cancer cells. This study raises the possibility of ACE2-mediated viral tropism in cancer tissues to be clarified in future studies.

Molecular/biomarker testing in lung cytology: A practical approach

The World Health Organization Reporting System for Lung Cytopathology

A New Cytology Staining Method: A Fast Approach for Rapid On-Site Evaluation on Thyroid Fine-Needle Aspiration Cytology

Summary: Thyroid fine-needle aspiration (FNA) is a well-established technique for primary diagnosis. This study compared the staining effects of two methods, namely Diff-Quik (DQ) and BlueStain (R), and found that BlueStain (R) showed better preservation of colloid and cell details, making it a more cost-effective and simpler alternative to DQ stain.

Breast Cancer Molecular Subtypes Differentially Express Gluconeogenic Rate-Limiting Enzymes-Obesity as a Crucial Player

Summary: This study investigated the expression of enzymes involved in glycolysis and gluconeogenesis in breast cancer, and found significant differences based on molecular subtype and body mass index. These differences were closely related to hormonal receptor and HER2 status, as well as pathological stage and histological grade. Obesity appeared to attenuate these differences, but adipocyte deposition and inflammatory infiltration did not affect the differences. Additionally, obesity-related conditions such as diabetes, hypertension, and dyslipidemia influenced the expression of certain enzymes.

2023 European Thyroid Association Clinical Practice Guidelines for thyroid nodule management

Summary: With the widespread use of imaging techniques, an increasing number of thyroid nodules are discovered. Most are benign and do not require treatment, while small intrathyroidal neoplasms can be safely managed conservatively. Guidelines are needed to provide a cost-effective and patient-centered approach to managing this prevalent condition. The present guidelines aim to provide a clinical practice guide for the initial workup and management of thyroid nodules in adults, and are not intended for the management of thyroid malignancy.
