Graft quality assessment in kidney transplantation: not an exact science yet!

Purpose of review The enduring donor shortage necessitates the development of tools capable of objectively assessing kidney graft quality and thereby allowing the safer and wider use of expanded criteria donors and kidneys donated after cardiac death. We summarize current assessment tools available prior to procurement and during preservation. Recent findings Several donor risk scores, combining donor and recipient risk factors of inferior graft outcome, exist but all lack predictive power. Histological scoring of glomerulosclerosis, tubular atrophy, interstitial fibrosis, and vascular damage in pretransplantation kidney biopsies can supply reliable, reproducible data on the actual kidney state but prospective data on their use in graft assessment are lacking. Renal resistance and certain perfusate biomarker concentrations during machine perfusion are independent risk factors of delayed graft function, but neither method has sufficient predictive power to allow kidney discard. Summary Available tools for graft quality assessment have their intrinsic value but none offer the necessary power to predict graft outcome for a specific donor-recipient pair. This is probably due to the multitude of donor, preservation, and recipient factors at stake. Only combining these factors might improve prediction of graft outcome and allow safer use of expanded criteria donors and kidneys donated after cardiac death.

Summary: Liver graft utilization rates and discard rates vary across countries, with machine perfusion strategies contributing to better DCD utilization rates. Risk factors and use of various machine perfusion techniques vary between countries, but this does not significantly impact 5-year graft survival rates. Overall, countries with routine use of machine perfusion strategies have higher DCD utilization rates without compromised outcomes.

Therapeutic lymphangiography with ethiodized oil for the management of lymphoceles and chylous ascites

Current Insights into the Metabolome during Hypothermic Kidney Perfusion-A Scoping Review

Summary: This scoping review summarizes the current knowledge on kidney metabolism during hypothermic perfusion preservation. A total of 52 papers were included in this review, which covered studies on dogs, rabbits, pigs, and humans published between 1970 and 2023. The studies reported the use of different perfusates, oxygenation levels, kidney injury levels, and devices, and examined the perfusate and tissue metabolites. The findings suggest that kidneys remain metabolically active during hypothermic perfusion, regardless of the perfusion settings. However, the understanding of kidney metabolism during hypothermic perfusion is still incomplete and influenced by various factors.

The Distinct Innate Immune Response of Warm Ischemic Injured Livers during Continuous Normothermic Machine Perfusion

Summary: During normothermic machine perfusion (NMP), livers procured after warm ischemia (WI) exhibit a distinct innate immune response compared to directly procured (DP) livers. The cytokine profile in WI livers shifts towards an anti-inflammatory phenotype during NMP and whole blood (WB) reperfusion, while pro-apoptotic signaling is stronger during NMP. WB reperfusion results in a blunted cytokine release in livers, suggesting a reconditioning effect of NMP, regardless of ischemic damage.
