Our purpose is to review noninvasive methods for measuring central arterial pressure. Indices of central arterial pressure measured from central aortic and peripheral arterial waveforms have shown value in predicting cardiovascular events and death, as well as in guiding therapeutic management. This article reviews noninvasive techniques of measuring central arterial pressure that have been validated against intra-arterial pressure. This paper explains methods to derive central (aortic and carotid) pressure from radial and brachial sites. It focuses on specific issues of brachial calibration applied to carotid pressure waveforms, which were regarded as a surrogate of aortic pressures used in three major studies (Framingham, Asklepios, and Australian National Blood Pressure 2 studies). We explain why radial-based methods are superior to carotid-based methods for estimating central pressure. Physiological principles of pressure measurement need be satisfied to ensure accurate recording.

Summary: Durable continuous-flow left ventricular assist devices (cfLVADs) outperform medical therapy alone in terms of survival, cardiac functional status, and overall quality of life in advanced heart failure. Previous studies have overlooked the influence of arterial compliance on pump performance and arterial pressure. This study examines the impact of alterations in arterial compliance, preload, and afterload on continuous-flow pump function and measured hemodynamics through an in-vitro pulsatile mock circulatory loop.

Impact of Obesity Phenotype on Central Aortic Hemodynamics and Arterial Stiffness in a Chinese Health Assessment Population

Summary: This study found that central obesity (measured by waist circumference and waist-hip ratio) may have greater predictive value for vascular stiffness than BMI. Further research focusing on arterial wave travel and its relationship with body fat distribution is needed.

Pulsatile energy consumption as a surrogate marker for vascular afterload improves with time post transcatheter aortic valve replacement in patients with aortic stenosis

Summary: This study found that arterial stiffening has a significant impact on pulsatile left ventricular afterload in patients with aortic stenosis, beyond the effect of systemic hypertension. Circulatory afterload pulsatile efficiency (CAPE) was used as an indicator of vascular function, measuring the ratio of steady state energy consumption (SEC) to pulsatile energy consumption (PEC) required to maintain systemic circulation. After transcatheter aortic valve replacement (TAVR), eight out of 20 patients showed a reduction in PEC, which was strongly correlated with the number of days post TAVR. Patients assessed within 100 days of TAVR had an increase in PEC compared to baseline. Overall, no significant differences were found in various parameters of arterial function between baseline and post TAVR. Improved flow profiles after TAVR likely contribute to vascular remodeling and improvement of pulsatile afterload, as reflected by the proposed index of CAPE.

Interpretation of the central aortic pressure waveform in elderly patients with aortic stenosis

Summary: The central aortic pressure waveform is simple in form but complex in physiological interpretation. There is increasing interest in the contour of the pressure wave in elderly patients with aortic valve stenosis. Methods to measure the central aortic pressure waveform and transcatheter aortic valve implantation technologies have enhanced our understanding of waveform alterations in elderly patients.

Prognostic role of pulmonary impedance estimation to predict right ventricular dysfunction in pulmonary hypertension

Summary: We evaluated the relationship between pulmonary impedance and pulmonary artery pressure and pulmonary vascular resistance using a cardiac magnetic resonance/right heart catheterization method. The results showed that pulmonary impedance is more closely related to pulmonary artery hypertension and better predicts maladaptive right ventricular remodeling. This straightforward method could be used to better assess pulsatile load in patients with pulmonary artery hypertension.

Summary: Understanding and predicting changes in the systemic and pulmonary circulations in pulmonary hypertension (PH) is increasingly important. Describing the circulation in complete quantitative terms, including characteristic impedance (Zc), is necessary to do so. Previous studies have not described the relationships between systemic and pulmonary Zc according to PH classification.

Association between Brachial-Ankle Pulse Wave Velocity as a Marker of Arterial Stiffness and Body Mass Index in a Chinese Population

Summary: Arterial stiffness measured by BAPWV is positively associated with blood pressure, but the relationship between BAPWV and BMI may only be significant in certain age and gender groups. This suggests that BMI may not be the most accurate indicator of obesity-related CV risk assessment. Further research is needed to understand the implications of these findings for individuals with higher BMI.
