Evaluation of the modulation of p-glycoprotein (P-gp) on the intraocular disposition of its substrate in rabbits

Purpose: To evaluate the functional role of P-gp and ocular tissue distribution of intravitreally injected Rhodamine-123 (Rho-123) in the presence of P-gp specific blocker (GF 120918) in normal as well as rifampicin-fed rabbits using microdialysis and direct sampling technique. Methods: Intravitreal pharmacokinetics of Rho-123 were conducted in male New Zealand albino rabbits. Direct sampling and microdialysis were employed to study the disposition of Rho-123 in normal as well as rifampicin-fed conditions. Control animals received Rho-123 at the concentration of 350 ng in PBS (0.05 ml) intravitreally, and the blocker-treated group received GF 120918 intravenously at the dose of 3.5 mg/kg 30 min prior to intravitreal injection of Rho-123. In case of direct sampling, four eyes were enucleated at different time points, and ocular tissues and humors were stored at -86 degrees C until analysis by HPLC with fluorescence detection. Results: In direct sampling, the blocker group showed significant increase (2.6 fold) in the mean vitreous concentration of Rho-123. Other tissues like ret-choroid, iris, and cornea also showed significant increase in their mean concentration. Microdialysis did not significantly predict the changes observed with direct sampling. Rifampicin-fed rabbits showed a vitreous pharmacokinetic profile comparable with non-fed (control) animals, and the pharmacokinetic parameters were unaffected by the blocker pretreatment. Conclusion: Intravenously injected blocker significantly altered the ocular disposition of intravitreally injected P-gp substrate. Rifampicin pretreatment did not upregulate P-gp transporters of the retina to the extent to affect the intravitreal kinetics of Rho-123 significantly.

Correlation of Levator-Muller's Complex Thickness on UBM to Botulinum Toxin A Injection in Thyroid-Related Upper Eyelid Retraction

Summary: This study used ultrasound biomicroscopy (UBM) to measure the thickness of levator palpebra superioris-Muller's muscle complex (LMC) in patients with thyroid-related upper eyelid retraction (UER) and correlated it with the clinical response to botulinum toxin A (BTA) injection. The results showed that UER patients had significantly thicker LMC compared to normal controls. Further studies are needed to explore the correlation between UBM features and response to BTA injection.

Summary: This study aims to investigate the pharmacokinetics of first-line anti-TB drugs in children and adolescents globally. The results show that younger age and HIV-positive status are associated with lower AUC(0-24) for all first-line anti-TB drugs, while severe malnutrition is associated with lower AUC(0-24) for isoniazid and pyrazinamide. The determinants of Cmax are generally similar to those of AUC(0-24).

Evaluation of liposomal amphotericin B for the treatment of fungal keratitis in a tertiary eye care hospital

Summary: This study evaluated the efficacy of liposomal amphotericin B (L-AMB) for the treatment of fungal keratitis. Patients with confirmed fungal keratitis were administered topical L-AMB and randomized into three groups treated with different formulations. L-AMB in gel form was found to be an effective antifungal agent that promoted the healing of fungal ulcers with less vascularization and better tolerance.

Ocular manifestations of central insulin resistance

Summary: This study found that central insulin resistance leads to increased intraocular pressure, dysfunction in trabecular meshwork and ciliary body, decreased cell density and insulin receptor expression in the retina, peripapillary atrophy and vascular dysregulation. These changes are accompanied by dysregulation of genes involved in inflammation, glial activation and apoptosis, and affect multiple pathways relevant to neurodegeneration.

Impact of Topical Cyclosporine-A or Topical Chloroquine on Post-LASIK Ocular Surface Stability - A Randomized Controlled Trial

Summary: The purpose of this study was to compare the effect of topical cyclosporine-A 0.05% and chloroquine phosphate 0.03% as adjuncts to standard therapy in maintaining ocular surface stability after LASIK. The results showed that both CsA and CHQ groups had significantly better OSDI score, MMP-9 levels, tear osmolarity, TBUT, and Schirmer score at 6 months compared to the control group (p < 0.001). CsA group had comparable MMP-9 levels to baseline and lower ocular irritative effects compared to the CHQ group. Both CsA and CHQ are effective adjuncts in maintaining ocular surface stability after refractive surgery.

Summary: The study aims to evaluate the systemic absorption of mitomycin C (MMC) during trabeculectomy and its potential systemic toxicity. The results showed that MMC was not detected in the plasma samples of the participants, indicating negligible systemic absorption or plasma concentration below 1.56 ng/ml (1000 times lower than the concentration where systemic toxicity was not observed).

Cationic Nano-Lipidic Carrier Mediated Ocular Delivery of Triamcinolone Acetonide: A Preclinical Investigation in the Management of Uveitis

Summary: This study evaluated a novel nano-lipoidal eye drop formulation of triamcinolone acetonide (TA) for the topical treatment of uveitis. The developed triamcinolone acetonide-loaded nanostructured lipid carriers (cTA-NLC) showed sustained release and enhanced efficacy in vitro. In vivo efficacy was tested on Wistar rats and a pharmacokinetic study was conducted in rabbits. The results demonstrated that the cTA-NLC formulation effectively managed uveitis with minimal signs of inflammation.

Summary: Fungal endophthalmitis is a potentially blinding condition, more commonly reported in Asia and specifically India. It has a lower incidence compared to bacterial endophthalmitis but is more challenging to treat. The current mainstays of treatment include vitrectomy, intravitreal antifungal agents, and systemic therapy. However, difficulties in clinical diagnosis, delayed microbiological culture, and limited options of antifungal drugs make the treatment more difficult and less rewarding.

Real-life comparison of three intravitreal antibiotic drug regimens in endophthalmitis
