Recent changes in lifestyle, including physical inactivity and unhealthy diets, are likely to have played an important role in the global epidemic of obesity, type 2 diabetes, and metabolic syndrome. Implementation of a healthier lifestyle, with an increase in physical activity and a reduction of body weight, based on the regulation of calories and fat intake, are the basis for the prevention and treatment of both type 2 diabetes and metabolic syndrome. Intervention studies based on changes in lifestyle in individuals at risk found that diabetes incidence was reduced by 42% to 63%. Similarly, intensive lifestyle changes in patients with the metabolic syndrome have been shown to reduce the prevalence of the syndrome by 20% to 48%. Reduction of body weight, improvement of the quality of diet, and promotion of physical activity are the main approaches to prevent and treat patients with type 2 diabetes or metabolic syndrome.

Thyroid surgery during the COVID-19 pandemic: results from a systematic review

L. Scappaticcio, M. I. Maiorino, S. Iorio, C. Camponovo, A. Piccardo, G. Bellastella, G. Docimo, K. Esposito, P. Trimboli

Summary: Elective thyroid surgery during the COVID-19 pandemic may face delays, but the risk of SARS-CoV-2 transmission post-surgery is relatively low. Complications after surgery include hypoparathyroidism and recurrent laryngeal nerve injury.

Repeat thyroid FNAC: Inter-observer agreement among high- and low-volume centers in Naples metropolitan area and correlation with the EU-TIRADS

Lorenzo Scappaticcio, Pierpaolo Trimboli, Sergio Iorio, Maria Ida Maiorino, Miriam Longo, Laura Croce, Marcello Filograna Pignatelli, Sonia Ferrandes, Immacolata Cozzolino, Marco Montella, Andrea Ronchi, Renato Franco, Mario Rotondi, Giovanni Docimo, Katherine Esposito, Giuseppe Bellastella

Summary: This study examined the inter-observer agreement between high-volume and low-volume institutions in thyroid cytology and explored the distribution of discordant FNAC reports in the European Thyroid Imaging and Reporting Data System (EU-TIRADS) categories. The study found significant differences in FNAC reports between the high-volume and low-volume institutions, and poor inter-observer agreement.

Biochemical predictors of diabetic foot osteomyelitis: A potential diagnostic role for parathormone

Paola Caruso, Maria Ida Maiorino, Lorenzo Scappaticcio, Chiara Porcellini, Rita Matrone, Paolo Cirillo, Margherita Macera, Maurizio Gicchino, Maria Teresa Vietri, Giuseppe Bellastella, Nicola Coppola, Katherine Esposito

Summary: This study aimed to evaluate PTH levels in individuals with diabetic foot ulcers (DFU) and investigate the relationship between PTH levels and osteomyelitis (OM) in this population. The results showed that PTH levels were lower in DFU patients with OM compared to those without OM, and alkaline phosphatase levels were higher in the OM group. OM and alkaline phosphatase levels were independent predictors of PTH levels in this selected population.

Neuroimmunoendocrinology of SARS-CoV-2 Infection

Giuseppe Bellastella, Paolo Cirillo, Carla Carbone, Lorenzo Scappaticcio, Antonietta Maio, Graziella Botta, Maria Tomasuolo, Miriam Longo, Alessandro Pontillo, Antonio Bellastella, Katherine Esposito, Annamaria De Bellis

Summary: This review discusses the neuroimmune endocrinological aspects of SARS-CoV-2 infection. The virus targets the central nervous system, affecting the hypothalamic-pituitary axis and potentially causing autoimmune processes and satellite gland disorders. However, further research is needed to understand the short- and long-term effects on the endocrine system.

Iron and Ferroptosis More than a Suspect: Beyond the Most Common Mechanisms of Neurodegeneration for New Therapeutic Approaches to Cognitive Decline and Dementia

Michele Cerasuolo, Irene Di Meo, Maria Chiara Auriemma, Francesca Trojsi, Maria Ida Maiorino, Mario Cirillo, Fabrizio Esposito, Rita Polito, Anna Maria Colangelo, Giuseppe Paolisso, Michele Papa, Maria Rosaria Rizzo

Summary: Neurodegeneration is a complex process involving multiple mechanisms, with diseases like Parkinson's, multiple sclerosis, Alzheimer's, prion diseases, and ALS being examples. Iron overload has been identified as a potential cause of neurodegeneration, with dysregulated iron metabolism and increased oxidative stress being common in these diseases. Alzheimer's, in particular, shows increased iron content in vulnerable brain regions, leading to mitochondrial alterations and lack of antioxidant defenses. Iron metabolism and ferroptosis play a significant role in diabetes-induced cognitive decline, and iron chelators offer a promising therapeutic approach.
