Radiomic Profiling of Head and Neck Cancer: 18F-FDG PET Texture Analysis as Predictor of Patient Survival

Background and Purpose. The accurate prediction of prognosis and pattern of failure is crucial for optimizing treatment strategies for patients with cancer, and early evidence suggests that image texture analysis has great potential in predicting outcome both in terms of local control and treatment toxicity. The aim of this study was to assess the value of pretreatment F-18-FDG PET texture analysis for the prediction of treatment failure in primary head and neck squamous cell carcinoma (HNSCC) treated with concurrent chemoradiation therapy. Methods. We performed a retrospective analysis of 90 patients diagnosed with primary HNSCC treated between January 2010 and June 2017 with concurrent chemo-radiotherapy. All patients underwent F-18-FOG PET/CT before treatment. F-18-FDG PET/CT texture features of the whole primary tumor were measured using an open-source texture analysis package. Least absolute shrinkage and selection operator (LASSO) was employed to select the features that are associated the most with clinical outcome, as progression-free survival and overall survival. We performed a univariate and multivariate analysis between all the relevant texture parameters and local failure, adjusting for age, sex, smoking, primary tumor site, and primary tumor stage. Harrell c-index was employed to score the predictive power of the multivariate cox regression models. Results. Twenty patients (22.2%) developed local failure, whereas the remaining 70 (77.8%) achieved durable local control. Multivariate analysis revealed that one feature, defined as low-intensity long-run emphasis (LILRE), was a significant predictor of outcome regardless of clinical variables (hazard ratio < 0.001, P = 0.001).The multivariate model based on imaging biomarkers resulted superior in predicting local failure with a c-index of 0.76 against 0.65 of the model based on clinical variables alone. Conclusion. LILRE, evaluated on pretreatment F-18-FDG PET/CT, is associated with higher local failure in patients with HNSCC treated with chemoradiotherapy. Using texture analysis in addition to clinical variables may be useful in predicting local control.

Summary: This retrospective study evaluated the efficacy and safety of involved-pelvic-node SBRT for oligorecurrent prostate cancer and found it to be a safe and effective treatment with a 100% local control rate in the series. In addition, prognostic factors such as number of secondary lesions, time-to-nadir PSA, PSA nadir value, and the time interval between primary treatment and SBRT were identified. Future prospective randomized studies are needed to further understand oligorecurrent prostate cancer.

Summary: This systematic review investigates the effectiveness of rehabilitation interventions for individuals with post-acute COVID-19 syndrome (PACS). The study suggests that rehabilitation can improve dyspnea, anxiety, and kinesiophobia, although results on pulmonary function are inconsistent. However, improvements were detected in muscle strength, walking capacity, sit-to-stand performance, and quality of life. Further high-quality studies are needed to confirm these findings.

Summary: Intraoperative cell salvage reduces the need for blood transfusion in cancer surgery, but concerns about re-infusing cancer cells have hindered its use. This study monitored cancer cells in salvaged blood and showed that cell salvage does not concentrate proliferating cancer cells. Leucodepletion and irradiation steps preserve the quality of salvaged blood, making irradiation unnecessary. Prospective trials are needed to establish a consensus on the use of cell salvage in complex cancer surgery.

Safety injections of nuclear medicine radiotracers: towards a new modality for a real-time detection of extravasation events and 18F-FDG SUV data correction

Summary: This study aimed to propose a new technique for monitoring and managing extravasation events during FDG PET/CT imaging by providing an early evaluation and correction to the estimated SUV value through an SUV correction coefficient. A cohort of 70 patients undergoing examinations was enrolled, and two portable detectors were secured on their arms to acquire dose-rate time curves. The proposed metrics allowed for the characterization of extravasation events and early SUV correction, highlighting the potential usefulness of this technique in clinical practice.

Summary: This cross-sectional single-center study investigated the ability of anterior segment optical coherence tomography (AS-OCT) to distinguish vitreous involvement due to vitreoretinal lymphoma (VRL) from vitritis in uveitis. AS-OCT images from 28 patients were analyzed using radiomics software, and a classification model was built using xgboost python function. The model achieved an 87% accuracy in diagnosing VRL or uveitis.
