Background: Misoprostol has been studied intensively to produce cervical dilatation. To date, no studies have evaluated the effectiveness, safety or acceptability of this treatment for repeat intrauterine device insertions. Study Design: This study was a randomized, double-blind, placebo-controlled trial on the consecutive use of the levonorgest-relreleasing intrauterine system (LNG-IUS). Women who had used their first LNG-IUS for 4 years and 3 to 9 months and opted for an immediate replacement with a second LNG-IUS received a single sublingual dose of 400 mcg misoprostol or placebo 3 h prior to the LNG-IUS insertion. Results: The proportion of easy insertions as judged by the investigator was 93% (40/43) in subjects randomized to misoprostol and 91% (42/46) in the placebo group (p=1.0 for the difference of proportions). No or mild pain at insertion was reported by 37% and 35% of subjects in the misoprostol and placebo groups, respectively. However, adverse events related to the study drug were more common in the misoprostol group. Conclusion: Sublingual misoprostol did not have a significant effect on the ease of insertion in subjects having a repeat insertion of the LNG-IUS. (C) 2010 Elsevier Inc. All rights reserved.

Use of universally offered family planning services - a cohort study in the city of Vantaa, Helsinki metropolitan area, Finland

Tuire Saloranta, Frida Gyllenberg, Anna But, Mika Gissler, Merja K. Laine, Oskari Heikinheimo

Summary: Young women are more likely to use free-of-charge family planning services, while young women speaking a foreign native language are underrepresented among service users. Women with a history of delivery, induced abortion or sexually transmitted infections, or with a lower socioeconomic or educational status are more likely to use these services.

Reproductive changes among women in their 40s: A cross-sectional study

Ingela Lindh, Finn Egil Skjeldestad, Oskari Heikinheimo, Helena Hognert, Ojvind Lidegaard, Christina Bergh, Kristina Gemzell-Danielsson

Summary: The reproductive pattern of married women in Scandinavia has changed, with an increase in the use of hormonal contraceptives among women aged 40-49. Sweden had the highest induced abortion rate, while birth rates increased continuously in Denmark and Sweden. The use of assisted reproductive technology doubled in all three countries.

Associations between use of psychotropic medications and use of hormonal contraception among girls and women aged 15-49 years in Finland: a nationwide, register-based, matched case-control study

Elena Toffol, Timo Partonen, Oskari Heikinheimo, Anna But, Antti Latvala, Jari Haukka

Summary: This study examines the relationship between the use of contraception and psychiatric medications. The findings suggest that women who use psychiatric medications are more likely to use hormonal contraception, with a specific pattern in the type of contraceptives used. Further research is needed to determine if this indicates a reduction in unwanted pregnancies among women with psychiatric disorders.

Systemic hormonal contraception and risk of venous thromboembolism

Summary: This study aimed to assess the associations between the patterns of use of different hormonal contraceptives and the risk of venous thromboembolism. The results showed that ethinylestradiol-containing combined preparations were associated with increased risk, while estradiol-containing combined preparations only had a slightly increased risk, and progestin-only contraception was not associated with venous thromboembolism risk.

Estradiol Valerate Vs. Ethinylestradiol In Combined Oral Contraceptives: Effects On The Pituitary-Ovarian Axis

Annina Haverinen, Kaisu K. Luiro, Marika H. Kangasniemi, Terhi T. Piltonen, Steinar Hustad, Oskari Heikinheimo, Juha S. Tapanainen

Summary: Limited studies have compared the effects of combined oral contraceptives (COCs) containing natural estrogens and synthetic ethinylestradiol (EE) on reproductive hormones. This study compared the effects of estradiol valerate (EV)+dienogest (DNG), EE+DNG, and DNG alone (active control) on levels of follicle stimulating hormone (FSH), luteinizing hormone, Anti-Mullerian hormone (AMH), ovarian steroids, sex hormone binding globulin (SHBG), and the Free Androgen Index (FAI). The results suggest that treatment with EV+DNG may have more beneficial health effects compared to EE+DNG.

Combined oral contraceptives containing estradiol valerate vs ethinylestradiol on coagulation: A randomized clinical trial

Summary: Oral contraceptives containing ethinylestradiol (EE) are associated with a risk of venous thromboembolism. This study compared the effects of different estrogen components in combined oral contraceptives on coagulation biomarkers and found that ethinylestradiol valerate (EV) + dienogest (DNG) had a smaller impact on thrombin generation compared to EE + DNG, suggesting that EV may be a more favorable component.

Surgically confirmed endometriosis in adolescents in Finland-A register-based cross-sectional cohort study

Elina Rasp, Liisu Saavalainen, Anna But, Mika Gissler, Paivi Harkki, Oskari Heikinheimo, Kristiina Rono

Summary: This study investigated the subtypes of endometriosis in adolescents as well as trends in incidence rates and related procedures from 1987 to 2012. The results showed that peritoneal endometriosis was the most common type, and the incidence rate of surgical diagnosis of endometriosis peaked in 2001-2005 and decreased thereafter. There was an increase in procedures for ovarian and deep endometriosis, as well as the use of laparoscopy and day surgery.

Summary: Despite the high prevalence of miscarriages, there is a lack of systematic registration and research. By validating the Finnish health registry data, we found that it is reliable and feasible for identifying miscarriages, different types of miscarriages, and non-surgical treatment.

Ethinylestradiol in combined hormonal contraceptive has a broader effect on serum proteome compared with estradiol valerate: a randomized controlled trial

Summary: This study compares the effects of an estradiol-based combined oral contraceptive (COC) with an ethinylestradiol-based COC and a dienogest-only preparation on the serum proteome. The results demonstrate that the synthetic COC has a wider range of effects on the serum proteome compared to the natural estrogen COC and the progestin-only preparation, involving multiple biological functions.

Hormonal contraception use and insomnia: A nested case-control study

Timo Partonen, Elena Toffol, Antti Latvala, Oskari Heikinheimo, Jari Haukka

Summary: Limited evidence suggests that the use of hormonal contraception may be associated with insomnia. A nationwide study of women aged 15-49 found significant associations between hormonal contraception use and insomnia. Specific products, such as drospirenone and ethinylestradiol, were associated with decreased odds of insomnia, while levonorgestrel-releasing intrauterine devices and vaginal rings with etonogestrel and ethinylestradiol were associated with increased odds of insomnia.

Janina Bosas, Elena Toffol, Elina Pohjoranta, Maarit J. Mentula, Ritva Hurskainen, Satu Suhonen, Oskari Heikinheimo

Summary: Sexual well-being is closely associated with general well-being, with lower anxiety levels and higher quality of life contributing to stable and higher levels of sexual well-being in fertile-aged women. The method of contraception or relationship status did not have a significant impact on sexual well-being during long-term follow-up.
