The results showed that EN was mostly used for the prevention and correction of undernutrition, preoperative optimization, and induction of remission in patients with active and long-standing Crohn's disease (CD). The study also identified barriers to using EN, such as low patient acceptance and poor adherence.

Summary: The cost of caring for patients with inflammatory bowel disease (IBD) continues to increase worldwide due to the steady increase in prevalence, the chronic nature of the diseases, expensive treatments, intensive monitoring, and their impact on productivity. This Commission urges evaluation of cost increases against improved disease management and the need for data interoperability and big data approaches to assess effectiveness and cost-effectiveness. International collaborations and novel care models should be sought to enhance education and training for clinicians, patients, and policy makers.

Summary: New data shows that the incidence and prevalence of inflammatory bowel diseases (IBD) are on the rise worldwide, with approximately 0.2% of the European population currently suffering from IBD. Medical therapy and disease management have significantly advanced, emphasizing close monitoring of disease progression and a treat-to-target approach to prevent early bowel damage and disability. The surgical rate in Europe has decreased over time, with high health economic burden associated with IBD in the region.

Summary: Inflammatory bowel disease (IBD) has been on the rise worldwide, with impaired nutritional status in patients presenting in various forms such as malnutrition, sarcopenia, and micronutrient deficiency, as well as overweight, obesity, and sarcopenic obesity. Malnutrition can disrupt the gut microbiome, leading to an imbalanced state that triggers inflammation. Despite the clear link between IBD and malnutrition, little is known about the underlying mechanisms that promote inflammation and malnutrition. This Review explores the potential mechanisms behind the vicious cycle of malnutrition and inflammation, and their implications for clinical treatment.

Summary: Inflammatory Bowel Disease (IBD) is a chronic autoimmune disease characterized by intestinal inflammation. The incidence of IBD is rapidly increasing in newly developed countries, making it a global concern. Two major types of IBD, ulcerative colitis (UC) and Crohn's disease (CD), have different manifestations and locations of inflammation in the digestive tract. Current FDA-approved treatments for IBD are only effective for 50% of patients and come with serious side effects, highlighting the need for safer and more effective therapies. Plant-based diets, such as the Mediterranean diet rich in anti-inflammatory phytochemicals, have shown benefits in preventing and improving symptoms of IBD. Terpenes, a class of phytochemicals abundant in Mediterranean herbs and citrus fruits, have been studied for their potential to modulate inflammatory signaling in IBD. However, there is still conflicting and inconclusive evidence regarding their effectiveness. This review evaluates preclinical studies on terpenes and their impact on basic inflammatory signaling in IBD.

The study found that the true prevalence of malnutrition was greatly underestimated when only using clinical assessment based on BMI and weight loss. Summary: This qualitative study investigated the decision-making process of surgeons regarding protective ileostomy creation after rectal cancer surgery. The results showed that 76% of surgeons supported the creation of ileostomy. When making the decision, 88% of surgeons considered risk factors. Emotions influenced the decision-making process in 22% of cases, and personal experiences influenced it in 49% of cases. The most frequently considered risk factors were the distance between the anus and the tumor (96%), neoadjuvant chemoradiotherapy (88%), positive intraoperative leak test (65%), blood loss (37%), and immunosuppression therapy (35%).

Summary: This retrospective multicentre cohort study aimed to identify risk factors for a permanent stoma (PS) in patients with left-sided obstructive colon cancer (LSOCC) and develop pre- and postoperative prediction models. The results showed that 37% of LSOCC patients had a PS, and 53% of those with a stoma directly after resection had a PS due to non-reversal. Both baseline characteristics and treatment strategies determine the risk of a PS in LSOCC patients. The developed predictive models will help physicians understand the role of individual variables in the risk of a PS and inform patients about the probability of a PS.

Summary: This study aimed to compare different treatment strategies for anastomotic leakage (AL) after low anterior resection (LAR) and found that endoscopic vacuum-assisted surgical closure (EVASC) resulted in improved rates of healed and functional anastomosis compared to conventional treatment. Early initiation of EVASC within the first week after surgery achieved a 100% functional anastomosis rate.

Summary: In this study, a 3D-printed anatomically correct phantom of a female peritoneum was used to validate the thermal module of a treatment planning software. The software, based on OpenFoam, helps understand and map thermal heterogeneities during Hyperthermic IntraPeritoneal Chemotherapy (HIPEC) treatments. The experimental data compared well with simulated temperature distributions, indicating the accuracy of the software.

Summary: In patients with limited peritoneal metastasis from colorectal cancer, cytoreductive surgery followed by hyperthermic intraperitoneal chemotherapy is a potentially curative treatment option. This study investigated the impact of treatment temperature and duration on the effectiveness of oxaliplatin and mitomycin C in preclinical models, and found that higher temperatures and longer treatment durations resulted in improved efficacy and reduced toxicity.

Summary: This study aimed to evaluate the variation in measurements and anatomical classifications of lateral lymph nodes (LLNs) before and after training. The results showed that after training, there was an improvement in the correct identification of anatomical location and a reduction in the deviation of short-axis measurements. The conclusion of this study is that adequate training is crucial for the accurate evaluation of LLNs.
