OBJECTIVE: To test whether weight and the weight gain rate during different age periods are associated with being overweight/obese at 10 years of age. METHODS: A nested case-control study was performed in a clinical historic cohort that was selected based on medical records from the Albert Einstein Hospital Social Program in Sao Paulo, Brazil. A sample of 378 eutrophic and overweight/obese children was analyzed. RESULTS: After adjusting for birth weight and gestational age, the likelihood of being overweight/obese at 10 years of age was 4.04-fold greater when progressing from one quartile of weight gain to the immediately superior quartile in the first semester of life and 3.24-fold greater when this occurred from 2-5 years of age. A one-quartile change in weight gain in the first semester was associated with a 0.5 z-score increase in BMI at age 10. A robust independent effect of weight at age 5 confirmed that earlier weight gain was an important predictor. CONCLUSIONS: The amount of weight gain during the first 6 months of life and between 2 and 5 years of age and weight at age 5 were important predictors of overweight/obesity at 10 years of age.

Birth weight associated with dual energy X-ray absorptiometry-determined muscle-bone unit in young healthy women from the Nutritionists' Health Study

Angelica Marques Martins Valente, Bianca de Almeida-Pititto, Alexandre Archanjo Ferraro, Luciana G. Dias Folchetti, Isis Tande Silva, Sandra Roberta G. Ferreira

Summary: This study examined the association between birth weight and the muscle-bone unit using DXA parameters in Brazilian young healthy women from the NutriHS cohort study. The results suggested that birth weight is related to the muscle-bone unit, indicating a potential role of the intrauterine environment in musculoskeletal health.

Summary: This study retrospectively describes the implementation of a low-cost telemedicine service in a pediatric hospital in Brazil. The results showed that 86% of the patients were satisfied with the teleconsultations, and 92% would use telemedicine again. These findings highlight the importance of telemedicine implementation in public healthcare services in Brazil and other developing countries.

Summary: This study compared initial clinical and laboratory parameters, as well as outcomes of mortality and rebleeding, in pediatric patients with acute upper gastrointestinal bleeding who underwent early UGIE (<12 hours) versus late UGIE (12-24 hours). The results showed no significant differences in outcomes between the early and late UGIE groups, suggesting that UGIE can be safely performed after 12-24 hours without harming the patient and optimizing clinical stabilization and treatment.

Teaching medical students to choose wisely through simulation

Thayza Marcelly Rodrigues Morato, Pedro Henrique Magalhaes Mendes, Danielle Saad Nemer Bou Ghosn, Thomaz Bittencourt Couto, Paulo Victor Ferreira Mai, Sylvia Costa Lima Farhat, Claudio Schvartsman, Nara Vasconcelos Cavalcanti

Summary: The study described the implementation of the CW campaign among medical students in the pediatrics clerkship using different teaching strategies and evaluated students' perception and performance. Results showed that students exposed to simulation training had significantly higher scores in the Objective Structured Clinical Examination (OSCE) and behavioral items compared to students who only watched the online video. Simulation training had a greater impact on undergraduate students' behavioral aspects and was well accepted by participants.

Pediatric emergency triage systems

Hany Simon Junior, Claudio Schvartsman, Graziela de Almeida Sukys, Sylvia Costa Lima Farhat

Summary: The aim of this study was to review pediatric triage systems in emergency departments, evaluating their validity and reliability. A total of 105 articles on pediatric triage systems in 12 countries were evaluated, with color-stratified triage systems and alert systems being the two main types. The review found that MTS, PaedCTAS, ESI version 4, PEWS, POPS, and PAT are valid and reliable for pediatric emergency services. However, cultural adaptation and validation studies are needed when implementing these systems in Brazil.

Summary: The study aimed to assess whether the lung ultrasound (US) score obtained at admission in children with respiratory distress predicts the need for escalated care. Results showed that children with higher lung ultrasound scores required more escalated care, indicating that lung ultrasound score can predict the severity of respiratory distress in children.

Summary: This study examined factors associated with survival and neurological outcomes after paediatric CPR in a middle-income country. Results showed that patients with liver disease, vasoactive drug infusion before cardiac arrest, shock as the cause of arrest, prolonged resuscitation, and bicarbonate administration during CPR had lower survival rates. However, most survivors had favorable neurological outcomes.
