Addressing the challenges of a cross-national investigation: lessons from the Pittsburgh-Pisa study of treatment-relevant phenotypes of unipolar depression

Background To date, no cross-national RCT has addressed the mechanisms underlying the relative success of pharmacological and psychotherapeutic interventions for depression. A multi-site clinical trial that includes psychotherapy as one of the treatments presents numerous challenges related to cross-site consistency and communication. Purpose This report describes how those challenges were met in the study Depression: The Search for Treatment Relevant Phenotypes, being carried out at the University of Pittsburgh and the University of Pisa, Italy. Methods Implementing the study required the investigators to address methodological and practical challenges related to the different requirements of the two Institutional Review Boards (IRBs), psychotherapy training, independent evaluator training, patient recruitment, development of common tools for data entry, quality control and generation of weekly reports of patient progress as well as establishing a similar clinical and research framework in two countries with substantially different health care systems. Results By having bilingual investigators and staff members who spent time at one another's sites, making use of frequent conference-call staff meetings and being flexible within the bounds of the sometimes contradictory requirements of the IRBs, the investigators were able to meet the human subjects protection requirements of both institutions, surmount language barriers to consistent therapist and evaluator training and develop common tools for study management. As a result, recruitment goals were met at both sites and retention rates were high. One instance of inconsistent implementation of the protocol was corrected within the first year. Limitations This study was conducted in two Western cultures by researchers with long-standing collaboration. Our findings may not be generalizable to other countries or research settings. Conclusions The implementation of a cross-national protocol and the adoption and maintenance of common procedures is possible when investigators are aware of the challenges this may present and are proactive in trying to address them.

Vortioxetine 20 mg/day in patients with major depressive disorder: updated analysis of efficacy, safety, and optimal timing of dose adjustment

Summary: This study analyzed the efficacy and tolerability of vortioxetine in patients with major depressive disorder. The results showed that vortioxetine 20 mg/day was more effective than 10 mg/day, with similar tolerability. In flexible-dose studies, nearly half of the patients received 20 mg/day after 1 week and two-thirds received 20 mg/day as their final dosage.

Emotional blunting in patients with depression. Part I: clinical characteristics

Summary: This study investigates the experience of emotional blunting in depression, its impact on overall functioning and quality of life, and the perspectives of patients and healthcare providers. The findings show that almost three-quarters of patients in the acute phase of depression and one-quarter of those in remission reported severe emotional blunting. Furthermore, approximately 56% of patients believed that their emotional blunting was caused by their depression, while 45% believed that their antidepressant medication was negatively affecting their emotions. Over one-third of patients were considering stopping or had stopped their antidepressant as a result.

Are empathy profiles and perceived social support associated with depressive and grief-related symptoms in suicide survivors?

Summary: The study found that lower empathy and higher personal distress were significantly associated with depressive symptoms, and perceived social support was also related to depressive and grieving symptoms. Empathy-focused psychotherapies and increased social support may help reduce symptoms in suicide survivors.

Summary: This study assessed the efficacy and tolerability of Vortioxetine in treating anxiety symptoms in patients with major depressive disorder. The results showed a clear dose-response relationship for Vortioxetine in improving depressive, anxiety, and functioning symptoms. Additionally, Vortioxetine was found to be superior to agomelatine in patients with an inadequate response to prior therapy.

Survival and reoperation in acute aortic syndromes-a single-centre experience of 912 patients

Summary: This study focuses on a large family with 12 members affected by bipolar disorder. Through whole-exome sequencing, certain genes, including CLN6 and ZNF92, were identified as having variations associated with the disorder. The findings suggest that heterozygous carriers of CLN6 may develop bipolar disorder later in life if combined with additional variants in ZNF92.

Specific Learning Disorders and Eating Disorders: an Italian retrospective study

Do interpersonal events buffer or worsen depressive and grief related symptoms in people bereaved through suicide?

Summary: Life events can amplify emotional suffering caused by suicide of a loved one, while supportive social relationships can facilitate coping. This study explored the effects of stressful events and perceived social support on depressive and grief symptoms among 378 help-seeking Italians bereaved through suicide. Although no significant impact of interpersonal stressors was found, perceived social support buffered the intensity of depressive symptoms. Therefore, emphasizing and utilizing individuals' perceived social networks could strengthen postvention programs.
