To estimate the mortality rate and identify factors predicting survival in patients with polymyositis (PM) and dermatomyositis (DM). The medical records of 192 PM/DM patients who were treated at Chang Gung Memorial Hospital from 1999 through 2008 were retrospectively reviewed. The Taiwan National Death Registry (1999-2008) was used to obtain their survival status. Thirty-one (16.1%) of the 192 patients with PM/DM had an associated malignancy; 41 (21.4%) had interstitial lung disease (ILD). During the follow-up period, 55 (28.6%) patients died and the overall cumulative survival rate was 79.3% at 1 year, 75.7% at 2 years, 69.9% at 5 years, and 66.2% at 10 years. In univariate analysis, older age at PM/DM onset, anemia, thrombocytopenia, leukopenia, diabetes mellitus, ILD, cancer, and non-use of azathioprine were associated with higher mortality (p = 0.0172, 0.0484, < 0.0001, 0.0008, 0.0001, 0.0036, 0.0010, and 0.0019, respectively). In multivariate Cox regression analysis, thrombocytopenia (hazard ratio [HR] 4.94, 95% confidence interval [CI] 2.60-9.37, p < 0.0001), diabetes mellitus (HR 2.57, 95% CI 1.38-4.80, p < 0.0001), cancer (HR 2.30, 95% CI 1.26-4.22, p = 0.0030), and ILD (HR 1.98, 95% CI 1.11-3.51, p = 0.0182) were positively associated with mortality. Use of azathioprine (HR 0.35, 95% CI 0.16-0.74, p = 0.0064) was negatively associated with mortality. This study confirmed the high mortality rate (28.6%) in PM/DM patients. Survival time was significantly reduced in patients with thrombocytopenia, diabetes mellitus, ILD, and cancer patients than in those without these conditions.

Kinship analysis of type 2 diabetes mellitus familial aggregation in Taiwan

Pei-Ju Liao, Ming-Kuo Ting, Chang-Fu Kuo, Yu-Hao Ding, Ciao-Ming Lin, Kuang-Hung Hsu

Summary: This study investigated the familial aggregation of type 2 diabetes mellitus (T2DM) among different kinship relatives in the Taiwanese population. The results showed that individuals with first-degree relatives with T2DM had a higher risk of developing T2DM, followed by those with second-degree relatives. The risk was higher when the affected family member was the mother. There was a dose-response relationship between the number of affected family members and the risk of T2DM.

Trends of adverse events and mortality after DMARDs in patients with rheumatoid arthritis: Interrupted time-series analysis

Yao-Fan Fang, Jia-Rou Liu, Shu-Hao Chang, Chang-Fu Kuo, Lai-Chu See

Summary: This study investigated the trends of adverse events and mortality associated with disease-modifying antirheumatic drugs (DMARDs) in patients with rheumatoid arthritis (RA). The results showed that the introduction of new treatment for RA was associated with stable adverse events and mortality rates.

Epidemiology and Mortality of Ovarian Cancer in Taiwan: A Population-Based Study

Yun-Hui Teng, Fu-Chao Liu, Shang-Yu Huang, Chang-Fu Kuo, Huang-Ping Yu

Summary: This study estimated the incidence and mortality rates of different histological subtypes of ovarian cancer in Taiwan by analyzing the national databases. The findings showed an increasing incidence rate but decreasing overall mortality rate of ovarian cancer during the study period. Ovarian serous carcinoma was the most common histological subtype in Taiwan.

Ultrasound Can Be Usefully Integrated with the Clinical Assessment of Nail and Enthesis Involvement in Psoriasis and Psoriatic Arthritis

Yu-Shin Huang, Yu-Huei Huang, Chiung-Hung Lin, Chang-Fu Kuo, Yun-Ju Huang

Summary: This study compared the findings of nail and enthesis ultrasonography in patients with psoriasis and psoriatic arthritis. The results showed significant clinical differences between patients with psoriasis and those with psoriatic arthritis. Nail ultrasonography revealed that onychopathic changes were more common in the psoriasis group. Enthesis ultrasonography showed that the lower-limb enthesopathy was more severe in cases of psoriatic arthritis.

Effect of febuxostat on adverse events and mortality in gout in Taiwan: An interrupted time series analysis

Ping-Han Tsai, Chang-Fu Kuo, Jia-Rou Liu, Pei-Ru Li, Lai-Chu See

Summary: This study retrospectively evaluated the influence of febuxostat on adverse events and mortality in gout. The results showed that febuxostat use gradually increased and reduced the incidence rate of Stevens-Johnson syndrome or toxic epidermal necrolysis in all gout patients. However, for gout patients with comorbidities, febuxostat use may increase the incidence rate of acute myocardial infarction, percutaneous coronary intervention, and all-cause mortality.

Fetal-neonatal and maternal outcomes in women with Sjogren syndrome: a population-based registry linkage study

Tien-Ming Chan, Chiao-En Wu, Han-Hua Yu, Chao-yang Hsiao, Tse-Hsuan Su, Chun-Bing Chen, Meng-Jiung Chiou, Kuang-Hui Yu, Chang-Fu Kuo

Summary: The study found that pregnant women with Sjogren syndrome (SS) have higher risks of adverse fetal-neonatal and maternal outcomes, including stillbirth, low birth weight, small for gestational age, fetal distress, pulmonary edema, shock, and respiratory distress. Therefore, preconceptional and prenatal counseling is necessary for these women.

Cardiovascular, renal, and lower limb outcomes in patients with type 2 diabetes after percutaneous coronary intervention and treated with sodium-glucose cotransporter 2 inhibitors vs. dipeptidyl peptidase-4 inhibitors

Hsin-Fu Lee, Yi-Hsin Chan, Chi Chuang, Pei-Ru Li, Yung-Hsin Yeh, Fu-Chih Hsiao, Jian-Rong Peng, Lai-Chu See

Summary: This study compared the cardiovascular, renal, and limb outcomes of type 2 diabetes patients after percutaneous coronary intervention (PCI) who were treated with sodium-glucose cotransporter-2 inhibitors (SGLT2i) versus dipeptidyl peptidase-4 inhibitors (DPP4i). The findings showed that SGLT2i was associated with lower risks of heart failure hospitalization, coronary revascularization, composite renal outcomes, and all-cause mortality compared to DPP4i.
