Summary: Polymyositis (PM) and dermatomyositis (DM) are subtypes of idiopathic inflammatory myositis characterized by progressive muscle weakness. They affect multiple organs and systems and an understanding of their biomarkers can aid in diagnosis, treatment, and prognosis prediction. This review summarizes the classic biomarkers such as antiaminoacyl tRNA synthetases (ARS) antibody, anti-Mi-2 antibody, and anti-nuclear matrix protein 2 (NXP2) antibody, as well as potential novel biomarkers. Classic biomarkers have become mainstream for diagnosis, while novel biomarkers have research potential and can contribute to classification standards and application value.

Summary: Dermatomyositis and polymyositis are autoimmune diseases characterized by muscle weakness. Proteomics analysis revealed differential expression of plasma exosomal proteins related to the complement and coagulation cascade pathway in these diseases. These proteins correlated with clinical parameters and could serve as predictors for disease. Additional proteins were also identified and validated, suggesting their potential significance in the pathogenesis of the diseases.

Summary: The purpose of this study was to determine the prevalence of RP-ILD in DM/PM patients and compare prognoses between treatments. The study found that the prevalence of RP-ILD in DM/PM was approximately 8.9% with a poor long-term prognosis. Biological agents appeared to provide the best therapeutic outcomes, but strong immunosuppressive treatments may have life-threatening side effects.

Summary: Aggarwal et al. recently published the results of a phase III trial investigating the use of intravenous immunoglobulin (IVIG) in dermatomyositis (DM). The study demonstrated that patients who received IVIG showed greater improvement in disease activity compared to those who received placebo, leading to FDA approval for the use of IVIG in DM. However, the study also raised concerns regarding the inclusion of patients with different subtypes of DM, such as skin-predominant/amyopathic and post-myopathic DM.

Summary: This study aimed to define the transcriptional profile of muscle biopsies from anti-Mi2-positive dermatomyositis (DM) patients. A set of 135 genes, including SCRT1 and MADCAM1, was specifically overexpressed in anti-Mi2-positive DM muscle. The expression levels of these genes correlated with anti-Mi2 autoantibody titres, markers of disease activity and with the other members of the gene set.

Summary: This study evaluated the use of IVIG for the treatment of dermatomyositis in a randomized, placebo-controlled trial. The results showed that IVIG treatment resulted in a significant increase in the percentage of patients with at least minimal improvement in disease activity compared to placebo. However, IVIG treatment was associated with adverse events, including thromboembolism.

Summary: Trials on physical exercise in dermatomyositis and polymyositis patients are diverse. This systematic review aimed to assess the available evidence to support the safety and effectiveness of exercise in improving strength and aerobic capacity. The meta-analysis of 19 studies revealed that physical exercise was safe and led to improvements in strength and aerobic capacity. No exacerbation was reported, and promising novel approaches were identified. Overall, exercise can be considered as a moderately effective intervention for patients with dermatomyositis or polymyositis.

Summary: This study used the 2018 Nationwide Readmissions Database to describe the rates, reasons, and characteristics of readmissions for adults hospitalized for polymyositis/dermatomyositis (PM/DM) in the USA. The findings showed that about one-third of patients were readmitted within 180 days, which imposes a significant economic burden on the healthcare system. PM and sepsis were the main reasons for readmissions.

Summary: This study aimed to investigate characteristics of myocardial involvement and compare differences between patients with polymyositis (PM) and dermatomyositis (DM) by cardiac MR (CMR) imaging. The results showed that CMR tissue characterization imaging could detect early myocardial involvement in PM and DM patients without overt left ventricular dysfunction. Furthermore, the characteristics of myocardial involvement were different between PM and DM patients, with more serious involvement seen in PM patients.

Summary: This study investigated the familial aggregation of type 2 diabetes mellitus (T2DM) among different kinship relatives in the Taiwanese population. The results showed that individuals with first-degree relatives with T2DM had a higher risk of developing T2DM, followed by those with second-degree relatives. The risk was higher when the affected family member was the mother. There was a dose-response relationship between the number of affected family members and the risk of T2DM.

Summary: This study compared the findings of nail and enthesis ultrasonography in patients with psoriasis and psoriatic arthritis. The results showed significant clinical differences between patients with psoriasis and those with psoriatic arthritis. Nail ultrasonography revealed that onychopathic changes were more common in the psoriasis group. Enthesis ultrasonography showed that the lower-limb enthesopathy was more severe in cases of psoriatic arthritis.

Summary: This study retrospectively evaluated the influence of febuxostat on adverse events and mortality in gout. The results showed that febuxostat use gradually increased and reduced the incidence rate of Stevens-Johnson syndrome or toxic epidermal necrolysis in all gout patients. However, for gout patients with comorbidities, febuxostat use may increase the incidence rate of acute myocardial infarction, percutaneous coronary intervention, and all-cause mortality.

Summary: This study compared the cardiovascular, renal, and limb outcomes of type 2 diabetes patients after percutaneous coronary intervention (PCI) who were treated with sodium-glucose cotransporter-2 inhibitors (SGLT2i) versus dipeptidyl peptidase-4 inhibitors (DPP4i). The findings showed that SGLT2i was associated with lower risks of heart failure hospitalization, coronary revascularization, composite renal outcomes, and all-cause mortality compared to DPP4i.
