Bacterial colonisation of lower airways in health and chronic lung disease

Background: To evaluate the colonisation rate and type in different groups of patients with chronic lung diseases, bronchial lavage (BL) fluid was investigated for bacteria. Methods: All patients underwent fibre-optic bronchoscopy as part of routine investigation for remote haemoptysis or nodule investigation. The standard procedure included BL and microbiological culture providing the total number of colony forming units (cfu)/mL and the number of potential pathogenic bacteria (ppb)/mL. Three groups of patients were included: 48 persons had a final diagnosis of no pathology, 53 patients with chronic obstructive pulmonary disease in a stable phase and 32 patients with a final diagnosis of bronchiectasis. Results: The median number of cfu cultured from patients with bronchiectasis was 10(5) cfu/mL compared to 5 103 cfu/mL in patients with COPD and 10(4) cfu/mL in persons with no pathology. The ppb colonisation rate varied from 10% in persons with no pathology to 43% in patients diagnosed with chronic obstructive pulmonary disease (COPD) and 63% in patients with bronchiectasis. The most frequent bacteria isolated was Haemophilus influenzae. Colonisation rates were associated with frequencies of respiratory infections; patients with bronchiectasis reported a median of three infections per year, patients with COPD reported one infection per year, and persons without pathology reported 0 infections per year (P < 0.05). Within each group a large patient-to-patient variation was found. Conclusions: Different groups of patients with chronic pulmonary diseases have very different colonisation rates. Patients with bronchiectasis have the highest colonisation rate. This correlates to the reported frequency of lower respiratory tract infections.

Summary: The prevalence of TB is high among socially marginalized citizens in Denmark. The long-term outcome of preventive treatment for LTBI in these patients is limited. However, while the completion of preventive treatment did not significantly reduce the incidence of TB, no TB cases were observed within the first 2 years following the completion of treatment.

Natural killer cell activity as a biomarker for the diagnosis of lung cancer in high-risk patients

Increased use of computed tomography in Denmark: stage shift toward early stage lung cancer through incidental findings

Summary: The number of CT thorax examinations and the incidence of lung cancer in Denmark have both been increasing. The study found that the majority of early-stage lung cancers were incidental findings. Voluntary early retirement and mortality in patients with and without chronic diseases: a nationwide Danish Registry study

Summary: This study aims to investigate whether targeted antibiotic treatment against P. aeruginosa can reduce exacerbations and mortality in patients with chronic obstructive pulmonary disease (COPD), non-CF bronchiectasis, and asthma. The ongoing trial involves 150 patients randomly assigned to receive either no antibiotic treatment or anti-pseudomonal antibiotic treatment for 14 days, with primary outcomes focusing on time to exacerbation requiring prednisolone and/or antibiotics, and days alive without exacerbation. The results will provide insights into optimizing therapeutic approaches for patients with chronic, non-CF pulmonary disease and P. aeruginosa infection.

Disability Retirement After First Admission with Acute Exacerbation of Chronic Obstructive Pulmonary Disease: A Danish Nationwide Registry Cohort Study Using a Retrospective Follow-Up Design

Summary: This study investigates disability retirement in COPD patients 3 years after the first-ever AECOPD. The study utilizes nationwide Danish registry data to analyze the socio-demographics, comorbidities, and inhaled medication use of COPD patients. The hazard ratio of disability retirement during the 3-year follow-up period is calculated. Higher age, living alone, comorbidities, specific treatments, and low educational level are associated with an increased risk of disability retirement.

Assessment of circulating biomarkers for detection of lung cancer in a high-risk cohort

The Danish Respiratory Society's National guideline for treatment of stable disease, which supports evidence-based decision making and practical issues, is described in this paper.

Association between thoracic and third lumbar CT-derived muscle mass and density in Caucasian patients without chronic disease: a proof-of-concept study

Summary: Severe chronic obstructive pulmonary disease (COPD) is associated with higher 1-year mortality after tran-scatheter aortic valve replacement (TAVR). Among COPD patients, those with a higher risk of exacerbation have a significantly increased mortality rate compared to those with a lower risk. The use of guideline recommended COPD medication and home oxygen therapy can help reduce mortality in COPD patients undergoing TAVR.
