Frequency, Risk Factors, and Prognosis of Prolonged Delirium in Elderly Patients After Hip Fracture Surgery

Background Delirium in elderly patients after hip fracture surgery is believed to be a transient event, although it frequently lasts for more than 4 weeks. Questions/purposes We determined the incidence, risk factors, morbidity, and mortality of prolonged delirium in elderly patients after hip fracture surgery. Patients and Methods We evaluated 232 elderly patients (older than 65 years) (232 hips) who underwent hip fracture surgery for the development and duration of delirium and categorized them into three groups; nondelirium group, transient (a parts per thousand currency sign 4 weeks) delirium group, and prolonged (> 4 weeks) delirium group. Patients underwent a global geriatric evaluation, which included postoperative complications, mortality, and functional and mental status evaluations. The three groups were compared with respect to these variables. Results Seventy patients (30.2%) had delirium develop, and among these, 14 (20%) had prolonged delirium with a total incidence of 6%. Multivariate analysis showed preinjury dementia was a risk factor of prolonged delirium. At the final followup, five (62.5%) of the eight patients who were ambulatory outdoors in the prolonged delirium group became housebound, whereas only 18 (16.4%) of the 110 patients who were ambulatory outdoors in the nondelirium group became housebound. Survival at 40 months was 81.0% (95% confidence interval, 72.6%-89.3%) in the nondelirium group and 63.6% (95% confidence interval, 35.2%-92.1%) in the prolonged delirium group. Conclusions Prolonged delirium was found to be associated with a poor functional outcome and increased mortality. Level of Evidence Level II, prognostic study. Ten to 13-Year Results of Delta Ceramic-on-Ceramic Total Hip Arthroplasty in Patients Less Than 30 Years Old

Summary: This study evaluated the long-term clinical and radiographic outcomes of total hip arthroplasty using Delta ceramic-on-ceramic bearings. The results showed that the ceramic bearings did not fracture, but the incidence of squeaking increased. Some complications were observed after surgery, but the overall survivorship was good.

Effects of Thrombin-Based Hemostatic Agent in Total Knee Arthroplasty: Meta-Analysis

Summary: This meta-analysis evaluates the efficacy and safety of Floseal in total knee arthroplasty. The results show that using a thrombin-based hemostatic agent significantly reduces hemoglobin decline and the risk of allogenic transfusion, but has no significant impact on drainage volume or total blood loss. Funnel plots indicate no evidence of publication bias. Further well-designed randomized controlled trials with longer follow-up periods are needed to assess long-term efficacy and safety.

Effects of Abdominal Massage for Preventing Acute Postoperative Constipation in Hip Fractures: A Prospective Interventional Study

Summary: This prospective randomized controlled study aimed to determine the effects of abdominal massage on constipation management in elderly patients with hip fractures. The study found that patients in the massage group had significantly fewer constipation remedies and fewer defecation failures compared to the control group on postoperative day 5 and at discharge. Therefore, abdominal massage may be considered as an independent nursing initiative for constipation management.

The Early- to Mid-term Outcome of Novel Cementless Modular Femoral Stem in East Asian Patients

Summary: This study evaluated the outcomes of using a novel cementless modular, fluted, tapered stem in total hip arthroplasty (THA) cases with unstable metaphysis or a large degree of femoral bone loss. The results showed satisfactory clinical and radiological outcomes in the early-to mid-term, indicating that this modular femoral system can provide adequate fixation and be a practical option in complicated THA.

Effect of active airway warming with a heated-humidified breathing circuit on core body temperature in patients under general anesthesia: a systematic review and meta-analysis with trial sequential analysis

Summary: This study investigated the effectiveness of heated-humidified breathing circuit (HHBC) in maintaining the core temperature of patients receiving mechanical ventilation under general anesthesia. The analysis of 18 randomized controlled trials revealed that patients with HHBC had significantly higher core temperature at the end of surgery compared to those without any device or with heat and moisture exchanger. This suggests that HHBC can be used as an effective supplemental device to maintain intraoperative core temperature.

Effect of Immature Rubus occidentalis on Postoperative Pain in a Rat Model

Summary: This study aimed to compare the analgesic properties of immature and mature Rubus occidentalis extracts (iROE and mROE) using a rat model of postoperative pain. The results showed that iROE had better analgesic and anti-inflammatory effects compared to mROE. The analgesic activity of iROE may be associated with α2-adrenergic and opioid receptors.

A 10-to 12-year follow-up study of delta ceramic-on-ceramic total hip arthroplasty

Summary: In this extended follow-up study of 273 patients who underwent THA with Delta CoC bearing, no additional ceramic fractures occurred, and the incidence of squeaking and RLLs increased. Focal osteolysis was observed around the hydroxyapatite-coated stem in 1% of patients. The long-term survivorship was 98.3% at 12 years.
