Risk of malnutrition (over and under-nutrition): Validation of the JaNuS screening tool

Background & aims: Malnutrition (over and under-nutrition) is highly prevalent in patients admitted to hospital and it is a well-known risk factor for increased morbidity and mortality. Nutritional problems are often misdiagnosed, and especially the coexistence of over and undernutrition is not usually recognized. We aimed to develop and validate a screening tool for the easy detection and reporting of both undernutrition and overnutrition, specifically identifying the clinical conditions where the two types of malnutrition coexist. Methods: The study consisted of three phases: 1) selection of an appropriate study population (estimation sample) and of the hospital admission parameters to identify overnutrition and undernutrition; 2) combination of selected variables to create a screening tool to assess the nutritional risk in case of undernutrition, overnutrition, or the copresence of both the conditions, to be used by non-specialist health care professionals; 3) validation of the screening tool in a different patient sample (validation sample). Results: Two groups of variables (12 for undernutrition, 7 for overnutrition) were identified in separate logistic models for their correlation with the outcome variables. Both models showed high efficacy, sensitivity and specificity (overnutrition, 97.7%, 99.6%, 66.6%, respectively; undernutrition, 84.4%, 83.6%, 84.8%). The logistic models were used to construct a two-faced test (named JaNuS - Just A Nutritional Screening) fitting into a two-dimension Cartesian coordinate graphic system. In the validation sample the JaNuS test confirmed its predictive value. Internal consistency and test-retest analysis provide evidence for the reliability of the test. Conclusion: The study provides a screening tool for the assessment of the nutritional risk, based on parameters easy-to-use by health care personnel lacking nutritional competence and characterized by excellent predictive validity. The test might be confidently applied in the clinical setting to determine the importance of malnutrition (including the copresence of over and undernutrition) as a risk factor for morbidity and mortality. Front-of-pack labels: Directive versus informative approaches

Summary: The 2022 Front-of-pack labels (FOPLs) aim to communicate the health value of food items to consumers and support public health policies. However, directive FOPLs like Nutri-Score have drawbacks such as overemphasizing selected nutritional factors, ignoring the food matrix and holistic aspects of nourishment, and allowing ultra-processed products. In contrast, informative FOPLs focus on individual portions and avoid classifying foods as healthy or unhealthy. The adoption of FOPLs has not been effectively linked to improvements in health outcomes.

Orthorexia Nervosa Practices in Rheumatoid Arthritis: The DORA Study

Summary: This study aimed to evaluate the tendencies of orthorexia nervosa (ON) in patients with rheumatoid arthritis (RA). The results showed that most patients had ON tendencies, with a higher tendency in female patients and a lower tendency in older patients with higher body mass index. These findings highlight the importance of awareness and screening for ON in patients with RA.

Lifestyle Intervention in NAFLD: Long-Term Diabetes Incidence in Subjects Treated by Web- and Group-Based Programs

Summary: A study compared a web-based intervention (WBI) with a standard group-based intervention (GBI) for the prevention and treatment of non-alcoholic fatty liver disease (NAFLD), and found that both interventions had similar effects on weight loss and the risk of developing diabetes in NAFLD patients. The study followed 546 NAFLD individuals for 60 months and observed similar changes in weight and incidence of diabetes between the two intervention groups.

Digital Anthropometry: A Systematic Review on Precision, Reliability and Accuracy of Most Popular Existing Technologies

Summary: Digital anthropometry (DA) has been developed for body composition evaluation and postural analysis. This review examines the current state of DA technology and evaluates the reliability and accuracy of available technologies on the market. DA shows high reliability and accuracy, especially in assessing obese patients and correlates well with conventional anthropometry and body composition estimates. DA is cost-effective and time-efficient, making it a useful screening tool before more expensive imaging techniques or as an alternative to other less affordable techniques. However, caution is needed in interpreting results due to the heterogeneity of available studies. Furthermore, there is a need for integrated technologies to analyze body composition according to multi-compartmental models.

Dynapenia, Muscle Quality, and Hepatic Steatosis in Patients with Obesity and Sarcopenic Obesity

Summary: Accumulating evidence suggests a connection between sarcopenic obesity and NAFLD, but the impact of NAFLD on muscle contractility remains uncertain. This study aimed to investigate the effect of NAFLD on dynapenia in patients with sarcopenic obesity. The results showed that despite no significant differences in body weight, glycometabolic parameters, and hepatic steatosis prevalence, patients with sarcopenic obesity had worse serum lipid profiles, higher body fat, and lower skeletal muscle mass. There was a positive correlation between disposition index and muscle quality index after controlling for menopause and body fat percentage.

Rare forms of genetic paediatric adrenal insufficiency: Excluding congenital adrenal hyperplasia

Summary: Adrenal insufficiency is a severe endocrine disorder characterized by insufficient secretion of glucocorticoid and/or mineralocorticoid due to impaired adrenal function or inadequate adrenal stimulation. This review focuses on rare genetic causes of primary adrenal insufficiency with isolated glucocorticoid or combined glucocorticoid and mineralocorticoid deficiencies, as well as rare syndromes of isolated mineralocorticoid deficiency. The diagnosis of these rare genetic forms of pediatric adrenal insufficiency is challenging due to their clinical heterogeneity and rarity, and this review aims to provide an overview of their genetic and clinical features, as well as diagnostic and therapeutic approaches.

Androgen insensitivity syndrome: a review

Summary: Androgen insensitivity syndrome (AIS) is a disorder characterized by peripheral androgen resistance due to androgen receptor mutations in individuals with a 46 XY karyotype. The severity of hormone resistance determines the wide variety of phenotypes. A multidisciplinary approach involving physicians, surgeons, and psychologists is recommended for the management of AIS, to provide support in gender identity choices and appropriate therapeutic decisions.

Severity of Inflammation Is Associated with Food Intake in Hospitalized Geriatric Patients-A Merged Data Analysis

Summary: The impact of inflammation on food intake is unclear, but it is a key risk factor for malnutrition. A study of geriatric hospitalized patients across Europe found that inflammation was the most prominent risk factor for low food intake and malnutrition, surpassing other factors such as age, gender, infection, and comorbidity. A CRP level of ≥3.0 mg/dL is associated with a high risk of malnutrition in hospitalized geriatric patients.

Impact of migration on diabetes burden: audit in the metropolitan area of Bologna, Italy

Insulin resistance and metabolic flexibility as drivers of liver and cardiac disease in T2DM

Summary: This review investigates the mechanisms through which metabolic flexibility and insulin resistance contribute to the progression of NAFLD and heart disease. Both play a critical role in glucose and lipid metabolism, and disruption of metabolic flexibility leads to abnormal lipid metabolism and fat accumulation in the liver, while insulin resistance affects cardiac glucose metabolism and lipid metabolism, leading to impaired cardiac function. Regular physical activity promotes metabolic flexibility, while weight loss achieved through calorie restriction improves insulin sensitivity.

Risk of binge eating disorder in patients with metabolic dysfunction-associated steatotic liver disease

Summary: This study aimed to evaluate the association between metabolic dysfunction-associated steatotic liver disease (MASLD) and eating disorders. The results showed that binge eating disorder (BED) was present in approximately 17.8% of MASLD patients screened in a specialized metabolic disorders unit. This study highlights the importance of addressing eating disorders in the treatment of MASLD.
