Yttrium-90 (Y-90) has internal pair production that can be imaged by PET/CT. Y-90 PET/CT has better spatial resolution than bremsstrahlung SPECT/CT, and it is possible to use Y-90 PET/CT for accurate postradioembolization imaging of hepatic microsphere biodistribution. Y-90 time-of-flight PET/CT represents the current state-of-the-art in postradioembolization imaging. We present a case of Y-90 time-of-flight PET/CT scan performed after Y-90 selective internal radiation therapy in a 62-year-old man, depicting hepatic microsphere biodistribution in high resolution.
