Clinical practice for antiplatelet and anticoagulant therapy in neurosurgery: data from an Italian survey and summary of current recommendations - part I, antiplatelet therapy

Pietro Fiaschi, Corrado Iaccarino, Roberto Stefini, Enrico Prior, Alessandro Prior, Gianluigi Zona

Summary: There is a lack of consensus in managing antiplatelet therapy for neurosurgical patients, impacting patient treatment significantly. The survey reveals a considerable variability in neurosurgical care for patients on antiplatelet therapy, indicating a scarcity of evidence on this issue.

Summary: Basel-Vanagaite-Smirin-Yosef syndrome (BVSYS) is a rare autosomal recessive genetic disorder caused by variants in the MED25 gene, with common features including severe developmental delay and various anomalies. This report presents two sisters with BVSYS, one of whom died at a young age, and whole exome sequencing identified a variant in the MED25 gene.

Enhanced Otolith Function Despite Severe Labyrinthine Damage in a Case of Pneumolabyrinth and Pneumocephalus Due to Otogenic Meningitis Associated With Superior Canal Dehiscence

Andrea Castellucci, Cecilia Botti, Luigi Renna, Silvia Delmonte, Claudio Moratti, Rosario Pascarella, Giovanni Bianchin, Angelo Ghidini

Summary: This case report presents an unusual clinical scenario of otogenic meningitis causing pneumolabyrinth and pneumocephalus in a patient with superior canal dehiscence, resulting in profound sensorineural hearing loss. The case provides insight into the etiology of pneumolabyrinth and conveys the possibility of pneumocephalus being intracranially carried by superior canal dehiscence. Additionally, it demonstrates a unique pattern of functional dissociation among inner-ear receptors, with enhanced otolith function despite severe labyrinthine damage.

Summary: The study found that the 30-day mortality rate after MT in COVID-19 patients was 29%, and abnormalities in lymphocyte count, LDH, and aspartate levels may indicate poorer outcomes. The rate of symptomatic intracranial hemorrhage was 5.4%.

The course of intracranial pressure after antipyretic therapy in acute brain injury

Edoardo Picetti, Patrizia Ceccarelli, Laura Malchiodi, Francesca Mensi, Solange Risolo, Franco Servadei, Corrado Iaccarino, Ilaria Rossi, Sandra Rossi

Summary: This study aimed to evaluate the variations in intracranial pressure (ICP) after antipyretic therapy in acute brain injury patients and their relationship to baseline ICP. The results showed that the variation in ICP after antipyretic therapy was influenced by baseline ICP. Specifically, patients with baseline ICP greater than 15 mmHg showed a significant increase in ICP after antipyretic therapy, while a significant reduction in ICP was observed in patients with baseline ICP less than or equal to 15 mmHg.

Summary: This study analyzed data from the European TRISP collaboration to assess the frequency of symptomatic and fatal intracranial hemorrhage (ICH) in patients with intracranial tumors who underwent intravenous thrombolysis (IVT) for suspected ischemic stroke. The results showed a potential high risk of symptomatic ICH in patients with tumors of the pituitary region and suggested that these findings could guide individual treatment decisions in this population.
