The gut microbiome increasingly is recognized for its role in human health and disease. Initial evidence has indicated that gut microbial dysbiosis is associated with several pancreatic diseases. Although it is not known if these associations are causative, gut dysbiosis is hypothesized to mediate chronic proinflammatory changes in the pancreas. Further mechanistic and epidemiologic studies of the microbiome are needed. Ultimately, targeted modulation of the microbiota could have therapeutic value.

Linear-array EUS improves the accuracy of predicting deep submucosal invasion in non-pedunculated rectal polyps compared with radial EUS: a prospective observational study

Summary: Linear EUS was found to be more accurate in determining SMd invasion compared to radial EUS, with higher diagnostic accuracy and specificity. It can help in selecting appropriate treatment modalities for patients with non-pedunculated rectal polyp.

Artificial intelligence in pancreaticobiliary endoscopy

Venkata S. Akshintala, Mouen A. Khashab

Summary: Artificial intelligence applications in healthcare have significantly increased, specifically in the field of pancreatobiliary disorders, improving diagnostic accuracy and precision in clinical treatment decisions. However, caution is advised in interpreting AI-based literature due to limited external validation, and a multidisciplinary approach combining various data sources is recommended for better utilization of AI technologies to enhance patient care.

Downregulation of 5-hydroxymethylcytosine is an early event in pancreatic tumorigenesis

Kohei Fujikura, Zainab Alruwaii, Michael C. Haffner, Maria A. Trujillo, Nicholas J. Roberts, Seung-Mo Hong, Anne Macgregor-Das, Michael G. Goggins, Sujayita Roy, Alan K. Meeker, Ding Ding, Michael Wright, Jin He, Ralph H. Hruban, Laura D. Wood

Summary: This study revealed that 5hmC and TET1 were significantly downregulated in all types of pancreatic neoplasms and invasive pancreatic ductal adenocarcinomas, with most low-grade precancerous lesions showing downregulation. This suggests that downregulation of 5hmC and TET1 is an early event in pancreatic tumorigenesis.

Summary: The study aimed to investigate the effects of CFTR modulators on pancreatic function in patients with cystic fibrosis (CF). The systematic search revealed that CFTR modulators significantly reduced the occurrence of acute pancreatitis and improved indirect measures of pancreatic function. Future research is needed to further explore the mechanisms and impact of CFTR modulators on acute pancreatitis and pancreatic insufficiency caused by CFTR dysfunction.

Summary: Individuals with a family history of pancreatic cancer, especially with multiple first-degree relatives affected, have a significantly increased risk of developing pancreatic cancer. The risk is also higher among individuals with a family history of young-onset pancreatic cancer.
