BACKGROUND & AIMS: Serum levels of ferritin are commonly measured to assess iron stores but are affected by factors such as obesity and chronic disease. Published reference ranges have not changed in decades, and the number of patients whose levels exceed the upper limits has been increasing. As a result, more patients are evaluated for iron overload. METHODS: We compared serum levels of ferritin in 1188 Australian adults who participated in the 2005 Busselton Population Survey with levels from the 1995 survey. Parametric regression was used to assess the effects of body weight and biochemical parameters on serum level of ferritin to derive contemporary population-appropriate reference ranges. RESULTS: In 2005, age-adjusted levels of ferritin were 21% higher in men (P < .0001) and 10% higher in women (P = .01) than in 1995; 31% of men exceeded levels of 300 mu g/L, compared with 23% in 1995. Body mass index (BMI) >= 25 kg/m(2) was associated with higher levels of ferritin in men >= 35 years old and in postmenopausal women (P <= .002). Serum level of g-glutamyltransferase (GGT) correlated with serum level of ferritin (P < .0001). In men, the estimated 95th percentiles ranged from 353 to 495 mu g/L (<35 years), from 350 to 511 mu g/L (>= 35 years, BMI <25 kg/m(2)), and from 413 to 696 mu/L (>= 35 years, BMI >= 25 kg/m(2)) when GGT levels were 10-75 IU/L. In women, the 95th percentiles ranged from 106 to 235 mu g/L (premenopausal), from 222 to 323 mu g/L (postmenopausal, BMI <25 kg/m(2)), and from 249 to 422 mu g/L (postmenopausal, BMI >= 25 kg/m(2)) when GGT levels were 8-45 IU/L. CONCLUSION: Serum levels of ferritin increased significantly between 1995 and 2005. Reference ranges that accommodate demographic and biomedical variations will assist clinicians in identifying individuals who require further evaluation for iron overload.

Summary: The APOE genotype is strongly associated with late-onset Alzheimer's disease, with specific lipid species mediating up to 30% and 10% of the treatment effect of APOE epsilon 2 and epsilon 4, respectively. Plasma lipid species represent a potential therapeutic target for Alzheimer's disease.

mTOR Inhibitors Induce Erythropoietin Resistance in Renal Transplant Recipients

Summary: This study investigated the role of mTOR inhibitors in hemopoiesis regulation in stable renal transplant recipients. The results showed that EPO levels were significantly higher in the group receiving mTOR therapy, indicating that EPO resistance rather than impaired iron metabolism may contribute to the impaired hemopoiesis in RTR on mTOR therapy.

Summary: Through a large-scale multi-ancestry genome-wide association meta-analysis, 1020 independent association signals implicating 559 genes related to COPD were identified. These genes were enriched in 29 pathways. Variants showed heterogeneity across ancestries, age and smoking groups, and the genetic risk score was strongly associated with COPD across ancestry groups. New potential causal variants, genes, proteins and pathways were highlighted, including those targeted by existing drugs. These findings contribute to a better understanding of lung function and COPD mechanisms and have implications for functional genomics experiments and future COPD therapies.

TWEAK/Fn14 Signalling Regulates the Tissue Microenvironment in Chronic Pancreatitis

N. Dianah B. Abu Bakar, Rodrigo Carlessi, Jully Gogoi-Tiwari, Julia Kohn-Gaone, Vincent Williams, Marco Falasca, John K. Olynyk, Grant A. Ramm, Janina E. E. Tirnitz-Parker

Summary: The TWEAK/Fn14 signalling pathway plays a crucial role in tissue injury and regeneration, and its involvement in pancreatic injury and cancer is not well understood. This study demonstrates that TWEAK/Fn14 signalling is important in the establishment and progression of chronic pancreatitis, increasing the risk of pancreatic cancer. The findings suggest the TWEAK/Fn14 pathway as a potential therapeutic target.

?' fibrinogen levels as a biomarker of COVID-19 respiratory disease severity

Lucy Z. Kornblith, Bindhya Sadhanandhan, Sreepriya Arun, Rebecca Long, Alicia J. Johnson, Jamie Noll, C. N. Ramchand, John K. Olynyk, David H. Farrell

Summary: COVID-19 is associated with a strong inflammatory response leading to organ failure, thrombosis, and death. A study investigated a novel inflammatory biomarker, GPF, in 103 hospitalized patients with COVID-19 and 19 healthy controls. The findings showed that GPF levels were significantly associated with the severity of COVID-19, and it may be a useful marker for evaluating the respiratory disease severity.

Serum 21-Deoxycortisol for Diagnosis of Nonclassic Congenital Adrenal Hyperplasia in Women With Androgen Excess

Jennifer L. Ng, Ee Mun Lim, Rui Zhang, John P. Beilby, Gerald F. Watts, Suzanne J. Brown, Bronwyn G. A. Stuckey

Summary: Liquid chromatography and tandem mass spectrometry (LC-MS/MS) can accurately differentiate nonclassic congenital adrenal hyperplasia (NCCAH) and polycystic ovary syndrome (PCOS) using measured 17-hydroxyprogesterone (17OHP) and 21-deoxycortisol (21DF). The LC-MS/MS-specific 17OHP thresholds are lower than those based on immunoassay and are suitable for NCCAH diagnosis. 21DF shows high sensitivity and specificity for the diagnosis of NCCAH and heterozygosity (HTZ).

Summary: By using single-nucleus RNA sequencing, researchers identified a disease-associated hepatocyte transcriptional state that is increasingly prevalent in chronic liver disease and associated with structural variants. Integrated analysis of human data confirmed this finding and revealed a correlation between high levels of this hepatocyte transcriptional state and an increased risk of hepatocellular carcinoma development.
