The updated consensus includes a modern definition of actionable GERD, describes the likelihood of symptoms being related to reflux episodes, and provides diagnostic strategies and testing options based on proven or unproven GERD.

Summary: Epithelial barrier dysfunction is a major characteristic of GERD, which is associated with symptom origination, inflammatory remodeling, and carcinogenesis. Alginate-based antireflux medications have been found to protect against peptic barrier disruption, and this study reveals the molecular mechanisms behind the injury and protection. The findings highlight the potential of targeting regulated intramembrane proteolysis and molecular mechanisms induced by weakly acidic reflux to treat GERD and prevent carcinogenesis.

Summary: With the development of endoscopic technology, the study of esophageal flora has gained attention in understanding the pathogenesis, early diagnosis, and treatment of esophageal diseases. The composition of esophageal microflora varies among different esophageal disease states, with gram-positive bacteria dominating the normal esophagus and gram-negative bacteria dominating esophagitis. The diversity of esophageal flora is significantly reduced in patients with esophageal cancer, and Fusobacterium nucleatum has been identified as a potential microbial biomarker for esophageal squamous cell carcinoma.

The stent is capable of adapting to the dynamic environment of the esophagus and wirelessly harvesting energy through deep tissue. In vivo experiments using pig models demonstrated that continuous electrical stimulations delivered by the stent significantly increased the pressure of the lower esophageal sphincter.

Summary: Despite the potential for hospice to improve care for those at the end of life, it is underutilized for patients with ESLD. Increasing the appropriate utilization of hospice for ESLD requires a better understanding of patient eligibility, which can be based on predictors of high short-term mortality and liver transplant ineligibility. Such hospice criteria should be data-driven and should accommodate the uncertainty faced by patients and physicians.

Summary: Understanding the prognostic significance of AKI stage 1B (sCr =1.5 mg/dL) compared with stage 1A (sCr < 1.5 mg/dL) is important for initial management decisions in hospitalized cirrhosis patients. A nationwide US cohort study found that stage 1B patients had a higher risk of 90-day mortality, AKI progression, and decreased probability of AKI recovery compared to stage 1A patients. These findings can guide AKI management decisions for cirrhosis patients in the hospital.

The results showed that rAH developed in approximately 8.8% of AH survivors and was associated with lower long-term survival. Marital status and medications for alcohol use disorder were found to be associated with a lower risk for rAH.
