Efficacy and Durability of Radiofrequency Ablation for Barrett's Esophagus: Systematic Review and Meta-analysis

BACKGROUND & AIMS: In patients with Barrett's esophagus (BE), radiofrequency ablation (RFA) safely and effectively eradicates dysplasia and intestinal metaplasia. We aimed to determine the efficacy and durability of RFA for patients with dysplastic and nondysplastic BE. METHODS: We performed a systematic review and meta-analysis of studies identified in PubMed and EMBASE that reported the proportion of patients treated with RFA who had complete eradication of dysplasia (CE-D) and intestinal metaplasia (CE-IM), and the proportion of patients with recurrent IM after successful treatment. Pooled estimates of CE-D, CE-IM, IM recurrence, and adverse events were calculated. RESULTS: We identified 18 studies of 3802 patients reporting efficacy and 6 studies of 540 patients reporting durability. Ten were prospective cohort studies, 9 were retrospective cohort studies, and 1 was a randomized trial. CE-IM was achieved in 78% of patients (95% confidence interval [CI], 70%-86%) and CE-D was achieved in 91% (95% CI, 87%-95%). After eradication, IM recurred in 13% (95% CI, 9%-18%). Progression to cancer occurred in 0.2% of patients during treatment and in 0.7% of those after CE-IM. Esophageal stricture was the most common adverse event and was reported in 5% of patients (95% CI, 3%-7%). Confidence in most summary estimates was limited by a high degree of heterogeneity, which did not appear to be caused by single outlier studies. CONCLUSIONS: Treatment of BE with RFA results in CE-D and CE-IM in a high proportion of patients, with few recurrences of IM after treatment and a low rate of adverse events. Despite the large amount of study heterogeneity, these data provide additional information for patients and providers to make informed treatment decisions.

Fruit and vegetable consumptions in relation to frequent mental distress in breast cancer survivors

Summary: The study found that moderate fruit and vegetable consumption was significantly inversely associated with frequent mental distress (FMD) among breast cancer survivors. Summary: The study found that FMT was safe and effective in treating CDI in patients with cirrhosis, with a success rate of 85.7%. Risk factors for FMT failure in these patients included the use of non-CDI antibiotics, severe Child-Pugh scores, and probiotic use. The rate of FMT-related adverse events was 33.3%, but severe adverse events were rare and not associated with infection or death.

Hospice care for end stage liver disease in the United States

Summary: Despite the potential for hospice to improve care for those at the end of life, it is underutilized for patients with ESLD. Increasing the appropriate utilization of hospice for ESLD requires a better understanding of patient eligibility, which can be based on predictors of high short-term mortality and liver transplant ineligibility. Such hospice criteria should be data-driven and should accommodate the uncertainty faced by patients and physicians.

Attendance at a Transitional Liver Clinic May Be Associated with Reduced Readmissions for Patients with Liver Disease

Summary: This study examined the impact of a Physician Assistant-led post-discharge transitional liver clinic (TLC) on hospital readmissions for patients with liver disease. The results showed that patients who attended the TLC had a significant reduction in the 30-day readmission rate. This finding provides valuable clinical insights for patients with liver disease and highlights the need for further randomized trials to establish the efficacy of PA-led post-discharge transitional care.

Prognostic significance of acute kidney injury stage 1B in hospitalized patients with cirrhosis: A US nationwide study

Summary: Understanding the prognostic significance of AKI stage 1B (sCr =1.5 mg/dL) compared with stage 1A (sCr < 1.5 mg/dL) is important for initial management decisions in hospitalized cirrhosis patients. A nationwide US cohort study found that stage 1B patients had a higher risk of 90-day mortality, AKI progression, and decreased probability of AKI recovery compared to stage 1A patients. These findings can guide AKI management decisions for cirrhosis patients in the hospital.

Summary: This study investigated the frequency, risk factors, and outcomes of recurrent alcohol-associated hepatitis (rAH) in survivors of alcohol-associated hepatitis (AH). The results showed that rAH developed in approximately 8.8% of AH survivors and was associated with lower long-term survival. Marital status and medications for alcohol use disorder were found to be associated with a lower risk for rAH.
