Context In primary aldosteronism (PA), discriminating unilateral from bilateral disease is crucial because adrenalectomy is frequently curative in the former case but rarely helps in the latter. Various series have reported the utility of postural stimulation testing (PST), cross-sectional imaging and adrenal vein sampling (AVS) in the assessment of PA, but most of these studies were retrospective.

Pathological predictors of metastatic disease in testicular non-seminomatous germ cell tumors: which tumor-node-metastasis staging system?

Glenda Scandura, Thomas Wagner, Luis Beltran, Constantine Alifrangis, Jonathan Shamash, Daniel M. Berney

Summary: Pathological predictors such as lymphovascular invasion, tumor size, percentage of embryonal carcinoma, and stromal rete testis invasion are strong indicators of metastatic disease at presentation in patients with testicular non-seminomatous germ cell tumors. These factors should be considered in future revisions of the TNM classification for testicular cancers. Additionally, changes in the AJCC TNM eighth edition, including invasion of the epididymis and hilar soft tissue, were found to be significant predictors of metastatic disease.

An introduction to the WHO 5th edition 2022 classification of testicular tumours

Daniel M. Berney, Ian Cree, Vishal Rao, Holger Moch, John R. Srigley, Toyonori Tsuzuki, Mahul B. Amin, Eva M. Comperat, Arndt Hartmann, Santosh Menon, George J. Netto, Mark A. Rubin, Samra Turajlic, Maria R. Raspollini, Satish K. Tickoo

Summary: The 5th edition of the WHO Blue Book provides a comprehensive update on testicular tumours, while retaining the structure and nomenclature of the previous edition. The classification paradigm remains rooted in morphology, but important changes have been made in terms of terminology and the inclusion of new entities. The update highlights the importance of both molecular understanding and morphology in the study of testicular cancer.

WHO 2022 landscape of papillary and chromophobe renal cell carcinoma

Summary: The 5th edition of the WHO Classification of Tumours of the Urinary and Male Genital Systems introduces molecularly defined renal tumour subtypes, eliminates the subcategorization of papillary renal cell carcinoma (pRCC), includes emerging entities within the pRCC spectrum, introduces a category of 'other oncocytic tumours', accepts eosinophilic solid and cystic RCC as a new entity, and highlights the need for a reproducible grading system for chromophobe renal cell carcinoma (chRCC).

WHO 2022 classification of penile and scrotal cancers: updates and evolution

S. Menon, H. Moch, D. M. Berney, I. A. Cree, J. R. Srigley, T. Tsuzuki, E. Comperat, A. Hartmann, G. Netto, M. A. Rubin, A. J. Gill, S. Turajlic, P. H. Tan, M. R. Raspollini, S. K. Tickoo, M. B. Amin

Summary: Squamous cell carcinoma (SCC) is the most common malignant tumor of the penis. The 2022 WHO classification reinforces the 2016 classification and subclassifies precursor lesions and tumors into human papillomavirus (HPV)-associated and HPV-independent types. The use of p16 immunohistochemistry is practical for differentiating HPV-associated from HPV-independent penile SCC. The classification and reporting of scrotal tumors is provided for the first time in the fifth edition of the WHO Blue book, following the schema of penile cancer classification.

Measuring cancer burden in prostatic needle core biopsies: simplified assessments outperform complex measurements in assessing outcome: evidence to assist pathologist efficiency and minimize datasets

Daniel M. Berney, Kier Finnegan, Kim Chu, Samson W. Fine, Murali Varma, Jack Cuzick, Luis Beltran

Summary: This study aimed to determine the optimal method of measuring cancer extent in prostate cancer biopsies. The results showed that only the percentage of positive cores (%+cores) was a significant predictor of patient outcome in multivariate models. Thus, we propose using %+cores as a simple method of assessment, which is preferred over more complicated methods.
