Summary: The study aimed to characterize patient safety events reported by medical imaging personnel in Finland from 2007 to 2017, including the number and quality of reported injuries, risk assessment, and planned improvement of operations. The findings showed that the majority of reported incidents involved patient injuries, with minor harm related to contrast agents being the most common consequence. Supervisors generally assessed the risks as low. The most common suggestion for learning from incidents was to discuss them with staff.

Summary: Studies have shown that computerized order entry (CPOE) does not consistently prevent medication ordering errors as intended. This study analyzed safety events reported to the Agency for Healthcare Research and Quality (AHRQ) Network of Patient Safety Databases (NPSD) from 6/2010 to 12/2020. The most commonly reported and harmful medication ordering errors were incorrect dose and incorrect drug orders. Future research should focus on developing interventions to prevent these errors, with a priority on wrong-dose and wrong-drug errors.

Summary: Epilepsy is a group of nervous system-related problems characterized by a predisposition to epileptic seizures. In intensive care units (ICU), the frequency of seizures can range from 3.3% to 34%, and antiepileptic treatment is common practice. However, co-administration of drugs through the same infusion line, especially in ICU, poses risks to patient safety. In this study, the compatibility of sodium valproate (VPA) with parenteral nutrition admixtures (PNAs) was investigated, and the lipid emulsion composition was found to significantly influence drug-PN compatibility. Most tested PNAs showed particles above 1000 nm, disqualifying their simultaneous administration with VPA.
