Potential Clinical Significance of a Plasma-Based KRAS Mutation Analysis in Patients with Advanced Non-Small Cell Lung Cancer

Purpose: Non-small cell lung cancer (NSCLC) with KRAS mutation may be resistant to epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKI). This study aims to evaluate a plasma-based KRAS mutation analysis and the clinical significance of plasma KRAS mutation as a predictive marker for tumor resistance to EGFR-TKIs in patients with NSCLC. Experimental Design: DNA extracted from plasma and matched tumor tissues were obtained from 273 patients with advanced stage NSCLC. Patients were followed up prospectively for treatment outcomes. KRAS mutations in codon 12 and 13 were detected using PCR-restriction fragment length polymorphism. Mutations in plasma and matched tumors were compared. Associations between KRAS mutation status and patients' clinical outcomes were analyzed. Results: KRAS mutation was found in 35 (12.8%) plasma samples and 30 (11.0%) matched tumor tissues. The consistency of KRAS mutations between plasma and tumors is 76.7% (23 of 30; kappa = 0.668; P < 0.001). Among 120 patients who received EGFR-TKI treatment, the response rate was only 5.3% (1 of 19) for patients with plasma KRAS mutation compared with 29.7% for patients with no KRAS mutation in plasma DNA (P = 0.024). The median progression-free survival time of patients with plasma KRAS mutation was 2.5 months compared with 8.8 months for patients with wild-type KRAS (P < 0.001). Conclusions: KRAS mutation in plasma DNA correlates with the mutation status in the matched tumor tissues of patients with NSCLC. Plasma KRAS mutation status is associated with a poor tumor response to EGFR-TKIs in NSCLC patients and may be used as a predictive marker in selecting patients for such treatment. Spatial distribution, ecological risk, and human health assessment of heavy metals in lake surface sections - a case study of Qinghai Lake, China

Summary: This study conducted an ecological and human health risk assessment of metals and heavy metals in Qinghai Lake, the largest inland saltwater lake in China. The concentration, distribution, and source of these metals were investigated. It was found that the metals in the sediments of Qinghai Lake mainly came from five rivers entering the lake, with copper and zinc showing the highest enrichment capacity. The results of the risk assessment indicated low ecological risks and no threat to human health from the heavy metals in the lake's sediments.

HLA and tumour immunology: immune escape, immunotherapy and immune-related adverse events

Summary: This review provides a current understanding of the role of HLAs in cancer immunology. It summarizes how HLA alterations contribute to immune escape and resistance to immunotherapy, and potential methods to overcome HLA defects. The review also discusses the potential association between HLA and immune-related adverse events (irAEs). Downregulation, loss of heterogeneity, and entire loss of HLAs are responsible for immune escape. Overcoming HLA defects can be effective in cancer therapy. Non-classical HLA-I molecules, such as HLA-E and HLA-G, are reliable predictors of prognosis, while classical HLA-I expression is a potential predictor of favorable immunotherapy response. Certain HLA types may be associated with enhanced affinity to self-antigen-mimicked tumor antigens, which may positively correlate with irAEs triggered by checkpoint inhibitors.

Angiotensin-(1-7) ameliorates sepsis-induced cardiomyopathy by alleviating inflammatory response and mitochondrial damage through the NF-κB and MAPK pathways

Summary: There is an abnormal activation of the renin angiotensin aldosterone system (RAAS) in patients with Sepsis-Induced Cardiomyopathy (SIC). This study measured the protein concentrations of angiotensin II (Ang II) and angiotensin-(1-7) (Ang-(1-7)) in peripheral plasma collected from SIC patients and healthy controls, and explored the relationship between SIC, Ang II, and Ang-(1-7) levels. It was found that SIC patients had elevated Ang II and Ang II/Ang-(1-7) levels, while the healthy controls had higher Ang-(1-7) levels. Additionally, exogenous Ang-(1-7) showed protective effects against SIC by inhibiting inflammatory and oxidative stress pathways.

Summary: This study aimed to quantify the effects of climate change and land-use/land-cover dynamics on green water in the Sanjiang Plain. The results showed that after 2000, green water flow and storage in the region significantly increased. Climate change, particularly precipitation variations, accounted for over 56% of the changes in green water storage, while land-use changes, especially cropland transformation, contributed over 20% to the changes in green water flow.

Adsorption and desorption of heavy metals at water sediment interface based on bayesian model

Summary: The study analyzed the migration and distribution of heavy metals (HMs) in rivers and lakes and established a Bayesian network model to understand the impact of environmental factors on the adsorption-desorption behavior of HMs. Factors such as cation exchange capacity, organic matter, soluble fluoride, and pH were found to play important roles in HM adsorption and desorption. The study also revealed synergistic effects between environmental factors in affecting the release of bioavailable HMs. Bayesian network signals provided insights into the transfer direction of metals in the water-sediment interface.

Diffusive gradients in thin films for transfer of phosphorus, nitrate and ammonium in lake sediments

Summary: In this study, a passive sampling technique called diffusion gradients in thin films (DGT) was used to investigate the biogeochemical behaviors and dynamic transfer of PO4-P, NH4-N, and NO3-N in sediments of Lake Hulun in China. Based on the analysis of DGT concentration profiles and redox status, it was found that the release of phosphorus (P) is mainly controlled by the reductive dissolution of Mn(IV) or Fe(III) (oxyhydr)oxides, while the release of ammonium (NH4-N) is influenced by denitrification and dissimilatory nitrate reduction. The results of this study provide valuable information for lake pollution control and sediment remediation strategies.

Removal efficacy of fly ash composite filler on tailwater nitrogen and phosphorus and its application in constructed wetlands

Summary: This study investigated rural domestic sewage treatment facilities in two urban areas in Jiaxing and found high concentrations of total phosphorus and ammonia nitrogen in the plain river network. A new synthetic filler (FA-SFe) was selected to enhance nitrogen and phosphorus reduction in constructed wetlands, highlighting the importance of fillers. Experiments showed the high adsorption capacity of FA-SFe, with removal rates of ammonia nitrogen and total phosphorus reaching 71.3% and 62.7% respectively in actual wastewater treatment.

Association of Clinical and Radiological Features with Disease Severity of Symptomatic Immune Checkpoint Inhibitor-Related Pneumonitis

Summary: This study aims to investigate the predictive ability of clinical and chest CT features in predicting the severity of symptomatic immune checkpoint inhibitor-related pneumonitis (CIP). The results showed that the diagnostic performance of chest CT scores was better than clinical symptom scores, and the combination of the three scores demonstrated the best diagnostic value. Therefore, the clinical and chest CT features have important application value in assessing the disease severity of symptomatic CIP, and the routine use of chest CT is recommended in comprehensive clinical evaluation.
