Clinical use and research applications of Heidelberg retinal angiography and spectral-domain optical coherence tomography - a review

Fluorescein angiography (FA) was discovered by Nowotny and Alvis in the 1960s of the 20th century and has evolved to become the 'Gold standard' for macular diagnostics. Scanning laser imaging technology achieved enhancement of contrast and resolution. The combined Heidelberg retina angiograph (HRA2) adds novel innovative features to established fundus cameras. The principle of confocal scanning laser imaging provides a high resolution of retinal and choroidal vasculature with low light exposure providing comfort and safety for the patient. Enhanced contrast, details and image sharpness image are generated using confocality. For the visualization of the choroid an indocyanine green angiography (ICGA) is the most suitable application. The main indications for ICGA are age-related macular degeneration, choroidal polypoidal vasculopathy and choroidal haemangiomas. Simultaneous digital FA and ICGA images with three-dimensional resolution offer improved diagnosis of retinal and choroidal pathologies. High-speed ICGA dynamic imaging can identify feeder vessels and retinal choroidal anastomoses, ensuring safer treatment of choroidal neovascularization. Autofluorescence imaging and fundus reflectance imaging with blue and infrared light offer new follow-up parameters for retinal diseases. Finally, the real-time optical coherence tomography provides a new level of accuracy for assessment of the angiographic and morphological correlation. The combination of various macular diagnostic tools, such as infrared, blue reflectance, fundus autofluorescence, FA, ICGA and also spectral domain optical coherence tomography, lead to a better understanding and improved knowledge of macular diseases.

Closure of Persisting Full Thickness Macular Holes by Subretinal Fluid Application: Technical Approach and Surgical Considerations

Summary: Firm adhesions between the retina and adjacent retinal pigment epithelium may prevent the closure of macular holes after chromovitrectomy. Controlled application of subretinal fluid can release these adhesions and lead to closure of the retracted retina in large or refractory macular holes. With a standardized procedure, the operation can be carried out safely and with minimal material effort, resulting in a high secondary closure rate.

Summary: The use of SD-OCT allows for the detailed observation of ELM and EZ, and in DME, the correlation between increased VEGF levels and disease severity, retinal thickness, and disruption of ELM and EZ has been established. Following anti-VEGF therapy, the restoration of ELM precedes that of EZ, highlighting ELM as a novel retinal structural barrier.

Epiretinal Membrane in Patients with Diabetic Macular Edema

Summary: This study compared the outcomes of pars plana vitrectomy (PPV) with epiretinal membrane (ERM) peeling in diabetic retinopathy patients with and without diabetic macular edema (DME). The results showed that DME patients experienced significant improvement in visual acuity, macular thickness, and the need for intravitreal injections after surgery. DME patients were younger, had longer duration of diabetes, and higher HbA1C levels compared to diabetic ERM patients without DME.

Summary: This study evaluated the improvement in grades of topographic retinal pigment epithelium alterations (RPE-A) using spectral domain optical coherence tomography (SD-OCT) after intravitreal anti-VEGF therapy in patients with diabetic macular edema (DME). The results showed a significant improvement in RPE-A grades and reduction in central subfield thickness (CST) and cube average thickness (CAT) in the macular area post-intervention.

COVID-19 Pandemic Lockdowns Impact on Visual Acuity of Diabetic Macular Edema: A Large cohort

Summary: This study evaluated the impact of unplanned treatment gap during the COVID-19 pandemic lockdown on visual acuity in previously treated DME patients. The results showed that the lockdown led to a decrease in the number of anti-VEGF injections, but for most patients, returning to routine treatment was sufficient to maintain visual acuity, with no significant impact.
