Summary: This study investigated the association of 2D and 3D left atrial strain with low-voltage area in relation to the recurrence of atrial fibrillation after pulmonary vein isolation. The results showed that 3D left atrial reservoir strain and pump strain were lower in patients with recurrent AF. Furthermore, the association of 3D left atrial strain with recurrence of AF was independent of other clinical and echocardiographic parameters, making it a potential predictor for outcomes in patients undergoing PVI.

Summary: Atrial fibrillation (AF) often requires invasive treatment in the form of ablation, but incomplete ablation of the pulmonary veins (PV) can still cure AF in some patients. This suggests that an antiarrhythmic effect other than electrical isolation plays a role in preventing AF. We propose that modifying the arrhythmogenic substrate in the PV can be a new therapeutic approach for a subgroup of patients with PV reentry.

Summary: This article describes the catheter ablation procedure for an atrial tachycardia in a 77-year-old male patient after pulmonary vein isolation for atrial fibrillation. High-resolution mapping revealed a figure-of-eight reentrant atrial tachycardia with clockwise rotation in the right pulmonary vein carina and counterclockwise rotation via two conduction gaps along the previous isolation line.

Summary: This study aimed to investigate whether more extensive radiofrequency catheter ablation of the bi-atrial arrhythmogenic substrate instead of clinical arrhythmia ablation alone results in superior clinical outcomes in patients with pulmonary arterial hypertension (PH) and supraventricular arrhythmias. The results showed similar arrhythmia recurrence rates between the two treatment groups, and no excess of procedural complications or clinical events in the extensive ablation group.

Summary: This study aimed to investigate the characteristics of fractionated potentials (FPs) in non-macroreentrant atrial tachycardia (nAT) and introduce a new method of cross-mapping for clarifying their roles. The ratio of FP duration to AT cycle length was different among nAT groups, with a simple cross-mapping method employed to determine FPs responsible for particular patterns. Results showed that FPs could be found on target in most nAT cases following previous AF ablation, with potential implications for differentiation and ablation strategies.

The results showed that elderly patients (>=75 years) had an approximately threefold higher risk of symptomatic atrial fibrillation (AF) recurrence after cryoPVI compared to younger patients.
