Summary: Cardiac resynchronization therapy (CRT) is recommended for patients with symptomatic systolic heart failure and left bundle branch block, despite optimal medical therapy. The 2021 ESC guidelines emphasize the importance of CRT in heart failure patients with reduced left ventricular ejection fraction, sinus rhythm, and typical left bundle branch block. Other pacing strategies and alternative sites are available if CRT is not feasible or effective.

Summary: Although cardiac resynchronization therapy (CRT) is an effective treatment for heart failure patients with reduced ejection fraction, not all patients respond to it, suggesting a lack of understanding of the underlying mechanisms of CRT response.

Summary: This study analyzed the level of evidence and class of recommendations in 50 ESC guidelines released in the past 12 years. The results showed that only 16% of the recommendations had the highest quality of evidence. Although there was a slight increase in LOE A recommendations in recent years, further scientific research efforts are needed to improve the quality of evidence.

Summary: SGLT2 inhibitors are a class of antidiabetic treatments used in type 2 diabetes mellitus, particularly in patients with cardiovascular disease, high risk, heart failure, or renal disease. While common adverse events are easily treatable, attention must be paid to some potentially serious adverse events.

Summary: The study aims to compare standard in-hospital treatment and management of AHF with clinical guidelines and variations across regions. The results show significant differences in how AHF patients are treated and managed, with suboptimal adherence to clinical guidelines in some regions.

Summary: This study aimed to investigate the optimal atrioventricular (AV) delay in cardiac resynchronization therapy (CRT) and to explore a simple intracardiac electrogram-based optimization method. The study found that optimal AV delays vary among patients and differ from nominal settings. The paced AV delay can be easily calculated using the intracardiac electrogram method after optimizing the sensed AV delay.

Summary: The 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure provides patient-centric recommendations for clinicians to prevent, diagnose, and manage patients with heart failure. It is based on contemporary evidence and aims to improve quality of care and align with patients' interests.

Summary: In heart failure patients, there is a high prevalence of iron deficiency, yet only a third of patients undergo diagnostic testing for iron deficiency, and the recommended intravenous iron supplementation for heart failure patients with reduced ejection fraction is underutilized. Cardiologists are encouraged to adhere to ESC guidelines for optimal treatment outcomes.

Summary: The prevalence of heart failure (HF) has increased following the new diagnostic algorithm introduced in the 2021 HF guidelines. This study aimed to determine the contemporary prevalence of HF in a representative sample from the general population. The results showed an increase in HF prevalence, particularly in patients with reduced/mildly-reduced ejection fraction. This increase offers opportunities for preventive therapies in HF patients.

Summary: Heart Failure (HF) is a multi-faceted and life-threatening syndrome with significant morbidity and mortality worldwide, affecting over 64 million people. While the incidence of HF has stabilized in industrialized countries, the prevalence is increasing due to population ageing and improved treatment. Developing countries lack substantial data on HF epidemiology, which shows different features compared to the Western world. This review provides updated estimates on global burden of HF including prevalence, incidence, outcomes, and costs.

Summary: There are regional disparities in the treatment of chronic heart failure (CHF) in both urban and rural areas in Japan, with lower adherence rates in rural areas. Improving the use of guideline-directed treatment for CHF in rural areas may lead to better outcomes for CHF patients.

Summary: The study examines the prognosis of patients with significant aortic stenosis (AS) based on the presence of metabolic health and obesity. The results show that metabolically healthy obese patients and metabolically unhealthy obese patients are associated with lower all-cause mortality rates. In contrast, metabolically healthy nonobese patients exhibit the worst survival.
