Fighting against sudden cardiac death: need for a paradigm shift-Adding near-term prevention and pre-emptive action to long-term prevention

Summary: More than 40 years after the first ICD implantation, sudden cardiac death (SCD) remains a major cause of death worldwide. The current strategy of long-term prevention based on left ventricular ejection fraction has limited impact, as many SCD cases occur in the general population without known risk factors. It has been found that SCD is often preceded by warning symptoms, and prompt medical attention can greatly improve survival. New technologies and digital monitoring allow better identification of individuals at short-term SCD risk, enabling a new approach of near-term prevention based on the hours and minutes preceding SCD.

The results demonstrate the safety and efficacy of the procedure, with higher risk of procedural complications in the valve-in-ring group. The presence of paravalvular leak is associated with significantly worse midterm prognosis.

Sports-related sudden cardiac arrest in young adults

Summary: Data on sports-related sudden cardiac arrest (SrSCA) among young adults are limited. This study aimed to determine the overall incidence, characteristics, and outcomes of SrSCA in young adults. Among the cases identified, most occurred in recreational male sports participants and the use of automated external defibrillators (AEDs) by bystanders was disappointingly low. However, survival rates were significantly higher among those who received bystander cardiopulmonary resuscitation (CPR) and early AED use. Coronary artery disease was found to be the most prevalent cause of SrSCA in young adults.

Summary: The incidence and mortality of ventricular fibrillation (VF) among patients hospitalized for acute myocardial infarction (AMI) have significantly decreased over the past 20 years. However, VF remains associated with a 10-fold increased relative risk of in-hospital mortality, without affecting post-discharge mortality. These findings highlight the need for interventions during hospitalization to further reduce mortality in VF patients.

Summary: Body surface area (BSA) has been reported to be a stronger predictor for prognosis than body mass index in heart failure (HF) patients. However, the sex-specific association of BSA with mortality remains unclear. This study evaluated the association of BSA with two-year all-cause mortality in HF patients and found a U-shaped curve association in females and a linear regression association in males.

Tailored planning and management strategies based on sex could potentially improve clinical outcomes in TAVI.

Recent surveys showed that radiation exposure is a major concern for women wanting to pursue careers in interventional cardiology or cardiac electrophysiology (EP). This joint statement aims to provide clear information on radiation risk and the experiences of radiation-exposed cardiologists who continue to work during pregnancy, as well as propose measures to improve workplace safety and promote equity.
