Background-We tested the hypothesis that chronic treatment with sildenafil attenuates left ventricular (LV) remodeling and prevents exercise intolerance in chronic mitral regurgitation (MR). Methods and Results-MR was created in Sprague-Dawley rats by making a hole on the mitral leaflet. Two weeks after MR creation, MR and LV dilatation were confirmed by echocardiography, and rats were randomly assigned to sildenafil treatment (MR+sildenafil group; 50 mg/kg PO twice a day; n=16) or normal saline only (MR group; n=16) and continued for 4 months. Sixteen sham rats were compared with MR rats. After 4 months, LV size was smaller in the MR+sildenafil compared with the MR group (LV end-systolic dimension, 4.7 +/- 0.3 for sham versus 5.9 +/- 0.3 for MR+sildenafil versus 7.4 +/- 0.5 mm for MR; P<0.05; LV end-diastolic dimension, 8.3 +/- 0.4 versus 10.5 +/- 0.2 versus 11.7 +/- 0.61 mm, respectively; P<0.05). LV ejection fraction was greater in the MR+sildenafil group than in the MR group (70.2 +/- 2.2% for sham versus 67.0 +/- 4.2% for MR +sildenafil versus 58.9 +/- 2.5% for MR; P=0.01). Serial treadmill test revealed that exercise capacity was reduced in the MR but not in the MR +sildenafil group. Transcriptional profiling of cardiac apical tissues revealed that gene sets related to inflammatory response, DNA damage response, cell cycle checkpoint, and cellular signaling pathways were significantly enriched by genes with reciprocal changes. Pathological analysis showed that perivascular fibrosis was more prominent in the MR than in the MR +sildenafil group and that the percentage of terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling-positive cells was 2-fold greater in the MR compared with the MR+sildenafil group. Conclusions-Sildenafil significantly attenuates LV remodeling and prevents exercise intolerance in a rat model of chronic MR. This benefit may be associated with the antiapoptotic, anti-inflammatory effects of sildenafil. (Circulation. 2012; 125:1390-1401.)

Prediction of Deterioration of Left Ventricular Function Using 3-Dimensional Speckle-Tracking Echocardiography in Patients With Left Bundle-Branch Block

Summary: This study found that 2D global longitudinal strain (GLS) is associated with cardiovascular outcomes in patients with left bundle-branch block, but the predictive value of 3D speckle-tracking echocardiography has not been investigated in these patients. The study retrospectively analyzed the association between 2D and 3D strains and left ventricular ejection fraction (LVEF) in patients with preserved or reduced LVEF. The results showed that 2D-GLS is predictive of maintaining LVEF in patients with preserved LVEF, while a lower 3D-global circumferential strain is associated with persistently reduced LVEF in patients with reduced LVEF.

Association between renin-angiotensin-aldosterone system blockade and clinical outcomes in patients with hypertension: real-world observation from a nationwide hypertension cohort

Summary: This study investigated the association between the use of ACEi, ARB, or non-RASi and incident cardiovascular events in a nationwide hypertension cohort. The results showed that patients on ACEi and ARB had lower risks of myocardial infarction, ischemic stroke, atrial fibrillation, heart failure, and all-cause deaths compared to those on non-RASi. Sensitivity analysis and propensity score matching results supported these findings.

Summary: The aim of this study was to investigate the prognostic utility of left ventricular global longitudinal strain (LV-GLS) in patients with hypertrophic cardiomyopathy (HCM) and a left ventricular ejection fraction (LVEF) of 50-60%. The study included 349 HCM patients with an LVEF of 50-60%, and the results showed that LV-GLS was an independent predictor of a composite outcome including cardiovascular death in these patients. Therefore, LV-GLS can aid in risk stratification in HCM patients with an LVEF of 50-60%.

Historical Review and Future of Cardiac Xenotransplantation

Summary: With the development of immunosuppressive drugs, xenotransplantation has made major advances by understanding the immune rejection mechanisms. Hyperacute rejection is provoked by three main carbohydrate antigens on porcine endothelial cells: α1,3-galactose, SDa blood group antigen, and N-glycolylneuraminic acid. Genetic engineering has enabled knock-outs of these antigens, reducing rejection rates. Co-stimulation blockade and expression of multiple human transgenes in xenografts have also improved survival. However, there are still obstacles including zoonotic infections, physiological discordances, and healthcare costs.

Major Clinical Issues in Hypertrophic Cardiomyopathy

Summary: Hypertrophic cardiomyopathy (HCM) is a common inheritable cardiomyopathy with a high disease burden in Korea. Sudden cardiac death, atrial fibrillation and thromboembolic risk, dynamic left ventricular outflow tract obstruction, and heart failure progression are important issues in HCM. Implantable cardioverter-defibrillators and anticoagulation are effective strategies for preventing sudden cardiac death and reducing thrombotic risk. Symptomatic dynamic LVOT obstruction can be improved with negative inotropes and septal reduction therapy. Myocardial fibrosis plays a role in the progression of heart failure in HCM patients.
