Long-Term Outcome of Aortic Dissection With Patent False Lumen Predictive Role of Entry Tear Size and Location

Background-Patent false lumen in aortic dissection has been associated with poor prognosis. We aimed to assess the natural evolution of this condition and predictive factors. Methods and Results-One hundred eighty-four consecutive patients, 108 surgically treated type A and 76 medically treated type B, were discharged after an acute aortic dissection with patent false lumen. Transesophageal echocardiography was performed before discharge, and computed tomography was performed at 3 months and yearly thereafter. Median follow-up was 6.42 years (quartile 1 to quartile 3: 3.31-10.49). Forty-nine patients died during follow-up (22 type A, 27 type B), 31 suddenly. Surgical or endovascular treatment was indicated in 10 type A and 25 type B cases. Survival free from sudden death and surgical-endovascular treatment was 0.90, 0.81, and 0.46 (95% CI, 0.36-0.55) at 3, 5, and 10 years, respectively. Multivariate analysis identified baseline maximum descending aorta diameter (hazard ratio [HR]: 1.32 [1.10-1.59]; P = 0.003), proximal location (HR: 1.84 [1.06-3.19]; P = 0.03), and entry tear size (HR: 1.13 [1.08-1.2]; P < 0.001) as predictors of dissection-related adverse events, whereas mortality was predicted by baseline maximum descending aorta diameter (HR: 1.36 [1.08-1.70]; P = 0.008), entry tear size (HR: 1.1 [1.04-1.16]; P = 0.001), and Marfan syndrome (HR: 3.66 [1.65-8.13]; P = 0.001). Conclusions-Aortic dissection with persistent patent false lumen carries a high risk of complications. In addition to Marfan syndrome and aorta diameter, a large entry tear located in the proximal part of the dissection identifies a high-risk subgroup of patients who may benefit from earlier and more aggressive therapy. The results showed that ICD systems are cost-effective for patients with ischemic and nonischemic heart disease younger than 68 years old. However, for older nonischemic patients, the cost-effectiveness ratio is higher.

Despite the increased use and duration of these treatments between 2010 and 2017, there was no substantial reduction in major cardiovascular outcomes.

Development of a risk score for patients with ischaemic cardiomyopathy

Summary: This study aimed to evaluate predictors of major adverse cardiovascular events (MACE) in patients with ischaemic cardiomyopathy and develop a risk score. The study found that previous heart failure admission, worsening angina or dyspnea, estimated glomerular filtration rate <= 60 mL/min/1.73 m2, age > 73 years, diabetes, atrial fibrillation, end-diastolic volume index > 83 mL/m2, and scarred myocardium > 12% were independent predictors of MACE. Based on these factors, patients can be classified into intermediate, high, or very high risk. The importance of this study lies in providing a simple method to predict the risk of MACE in patients with ischaemic cardiomyopathy.

Strengths and limitations of patient registries in infective endocarditis

The HAH successfully expanded its activity and adapted to changing case-mixes and pathways throughout the pandemic, suggesting that it could advance a paradigm shift toward more person-centered, cost-effective models of clinical care for older adults.

Acute fetal cardiovascular adaptation to artificial placenta in sheep model

Summary: This study aimed to describe the acute cardiovascular adaptation of the fetus after connection to an artificial placenta (AP) using ultrasound and invasive and non-invasive hemodynamic assessment. The results showed that the connection to the AP system resulted in a reduction in the pulsatility index and increased umbilical venous peak velocity. While cardiac structure and function were preserved in the short term, the system led to non-physiologically elevated venous pressure and pulsatile flow.

Feasibility and safety of synchrotron-based X-ray phase contrast imaging as a technique complementary to histopathology analysis

Summary: X-ray phase contrast imaging (X-PCI) is a non-destructive technique for high-resolution, three-dimensional imaging of soft tissue samples. This study assessed the quality of standard histopathological techniques on human tissue samples that had been irradiated with different doses of X-rays in the context of an X-PCI experiment. The results showed that previous X-PCI did not affect the quality of routine histochemical and immunohistochemical staining, as well as DNA and RNA analyses on formalin-fixed, paraffin-embedded (FFPE) human samples. Based on these findings, performing X-PCI on FFPE human biopsies is feasible and acceptable.

Summary: Changes in cardiovascular anatomy and function induced by intrauterine growth restriction (IUGR) have long-term impacts and may increase the predisposition to heart disease in adulthood. This study investigated the relationship between morphological remodeling and angular variation of QRS and T-wave dominant vectors in IUGR using computational models. The results suggest that IUGR-induced morphological remodeling could contribute to the observed angle changes in IUGR patients.
