Prognostic Relevance of Pulmonary Arterial Compliance in Patients With Chronic Heart Failure

Background: Reduced pulmonary arterial compliance (Ca) is a marker of poor prognosis in idiopathic pulmonary arterial hypertension. We tested the hypothesis that pulmonary arterial Ca could be a predictor of outcome in patients with chronic heart failure (CHF). Methods: We enrolled 306 patients with CHF due to systolic left ventricular dysfunction (sLVD) who underwent a clinically driven right-sided heart catheterization. Pulmonary arterial Ca was measured by the ratio between stroke volume and pulse pressure (SV/PP). The primary end point was cardiovascular death; secondary end point was the composite of cardiovascular death, urgent heart transplantation, and appropriately detected and treated episode of ventricular fibrillation. Results: An inverse relationship was observed between SV/PP and pulmonary vascular resistance, the mean resistance-compliance product (RC-time) being 0.30 +/- 0.2 s. In patients with pulmonary capillary wedge pressure (PCWP), 15 mm Hg, the mean RC- time was 0.34 +/- 0.14 s, and in patients with PCWP >= 15 mm Hg it was 0.28 +/- 0.22 s. Eighty-seven patients died in a follow-up period of 50 +/- 32 months. At receiver operating characteristic curve analysis, the optimal prognostic cutoff point of SV/PP was 2.15 mL/mm Hg. An elevated (. 2.15) SV/PP was more strongly associated with survival than any other hemodynamic variable; it was associated with poor prognosis both in patients with high (P <.003) and in patients with normal pulmonary vascular resistance (P <.005). Conclusions: Pulmonary arterial Ca is a strong prognostic indicator in patients with CHF with sLVD. Most importantly, its prognostic role is retained in patients with normal pulmonary vascular resistance.

Prognostic impact of diabetes in chronic and acute heart failure

Summary: There is a strong bidirectional relationship between diabetes mellitus (DM) and heart failure (HF), with DM activating multiple molecular and pathophysiological mechanisms that can damage the heart in the long term. However, the prognostic role of DM in chronic and acute HF is still unclear, with gaps in evidence due to the heterogeneity of patient phenotypes and the necessity to better characterize individual patients and their comorbidities. This review aims to summarize the negative impact of DM on HF and analyze available data on the prognostic implications in both chronic and acute HF.

Impact of serum uric acid levels on cardiovascular events and quality of life in patients with chronic coronary syndromes: Insights from a contemporary, prospective, nationwide registry

Summary: Using data from a nationwide registry on patients with chronic coronary syndromes, this study assessed the impact of serum uric acid levels on quality of life and major adverse cardiovascular events. The results showed that patients with hyperuricemia had a greater burden of cardiovascular disease and worse quality of life. However, serum uric acid levels did not significantly influence the higher rates of cardiovascular mortality, hospitalization for heart failure, and major adverse cardiovascular events observed during the 1-year follow-up.

Summary: For a long time, cardiologists have underestimated the role of the right heart in heart failure caused by left heart disease. However, the importance of evaluating right ventricular structure and function in left heart failure is now well recognized and emphasized in the latest heart failure guidelines. Yet, there are still unanswered questions about which imaging techniques and parameters should be used to assess the severity of RV dysfunction, and whether less commonly used diagnostic tools have a role in this field.

Summary: A meta-analysis of 35 studies found that right atrial (RA) imaging assessment is a relevant prognostic marker in patients with pulmonary hypertension (PH). RA area, RA indexed area, RA peak longitudinal strain, and RA total emptying fraction were significantly associated with combined end-points including death, clinical worsening, and/or lung transplantation. RA volume and volume index showed marginal significant associations.

Summary: This study developed a machine learning algorithm that integrated peak atrial longitudinal strain (PALS) with guideline-recommended diastolic variables to improve the classification and prognostic stratification of heart failure patients. The algorithm showed better predictive ability for adverse events compared to guideline-based classification, regardless of ejection fraction status.

Summary: The aim of this study is to assess the clinical relevance of a proposed definition for RV functional recovery (RVFnRec). A decrease in RV end-diastolic volume of -15 mL and an increase in compliance had the best predictive value for RVFnRec. RVFnRec is associated with improvements in exercise capacity and clinical worsening.

Summary: This study analyzed the influence of aortic stiffness on disproportionate secondary mitral regurgitation (sMR) and found that aortic stiffness may play a role in the discrepancy between mitral valve lesion severity and sMR hemodynamic burden.
