Summary: Acute fibrinous and organizing pneumonia (AFOP) is a rare lung disease that is difficult to diagnose and its etiology is unclear. This case report presents a young male with AFOP, initially misdiagnosed as community-acquired pneumonia. Further investigation revealed underlying myelodysplastic syndrome (MDS) and hemophagocytic lymphohistiocytosis (HLH). Despite treatment with glucocorticoids and chemotherapy, the patient did not show improvement. Early identification and diagnosis of AFOP in patients with hematological diseases are crucial, and more research is needed for the management of severe AFOP.

Summary: We present the case of a middle-aged male who had a left retrocardiac opacity on a chest radiograph. Further thorax CT examination revealed multiple subpleural lesions with central necrosis. Histopathological examination of the subpleural lesions confirmed the diagnosis of acute fibrinous organizing pneumonia. The patient responded well to corticosteroid treatment, and repeat CT showed resolution of the subpleural lesions.

The diagnosis and treatment of AFOP are currently uncertain. It is important to adjust the treatment plan based on the patient's symptoms, test results, and response to treatment for an accurate diagnosis and optimal treatment efficacy.

Summary: This study evaluated the clinico-radiological findings of AFOP and identified prognostic predictors based on whether a surgical or non-surgical biopsy was performed. Patients diagnosed via surgical biopsy had different symptomatic presentations and prognosis compared to those diagnosed via non-surgical biopsy. Patients with patchy or mass-like air-space consolidation on CT images showed a better prognosis in AFOP cases.

Summary: Broncholithiasis coupled with Aspergillus infection is a rare disease with complex pathogenesis and non-specific clinical manifestations, often leading to misdiagnosis and incorrect treatment. We present a rare case of asymptomatic broncholithiasis with Aspergillus infection and discuss its diagnosis, differential diagnosis, and follow-up. Our study, together with other relevant research, provides valuable insights for improving physicians' awareness and future management of these conditions.

Summary: Oxaliplatin is a commonly used agent in neoadjuvant chemotherapy for gastrointestinal tract tumors, but it can cause idiopathic non-cirrhotic portal hypertension (INCPH). This study presents a case report of INCPH induced by oxaliplatin chemotherapy and provides insight into its clinical manifestations and treatment strategies, highlighting the importance for physicians to consider the risk of INCPH when prescribing oxaliplatin.

Summary: This study describes a case report of C. lapagei in Peru and systematically reviews documented case reports of individuals infected with C. lapagei worldwide. The study found that C. lapagei has been reported in 20 cases between 2006 and 2022, primarily presenting with fever and pneumonia, and showing sensitivity to quinolones, tetracyclines, and carbapenems as potential treatment options.

Summary: Acute idiopathic pulmonary hemorrhage in infants (AIPHI) is a rare and poorly understood disease with high lethality. We report a case of a 2-day-old male who died suddenly from acute pulmonary hemorrhage. Autopsy and histological analysis revealed alveolar spaces filled with red blood cells and hyaline membranes, supporting the diagnosis of AIPHI. A systematic review of scientific literature showed only 61 reported cases, with symptoms including acute respiratory distress, upper airway bleeding, hemoptysis, epistaxis, and hematemesis. Autopsy findings included lower airway hemorrhages and hemosiderin-laden macrophages. Our findings and literature data are crucial for accurate diagnosis, and we propose autoptic criteria for post-mortem diagnosis of AIPHI.

Summary: An immunocompetent patient with community-acquired pneumonia caused by Anaplasma phagocytophilum is reported. The patient presented with fever, cough, and shortness of breath, and imaging showed lung infiltrates. Extensive investigations confirmed anaplasmosis and the patient recovered with doxycycline therapy. Empiric treatments without doxycycline in anaplasmosis pneumonia cases can lead to acute respiratory distress syndrome, therefore clinicians should be aware of this unusual presentation and initiate appropriate antimicrobial regimens.

Summary: This article presents a case of pulmonary nocardiosis caused by Nocardia caishijiensis in a patient with HIV infection. The patient was successfully treated with amikacin and thrimetroprim-sulfametoxazole. This is the first reported case of Nocardia caishijiensis infection in humans in North America and highlights the potential importance of this pathogen in nocardiosis, especially in solid organ transplant recipients.
