Summary: This report discusses a case of donor-derived Mycoplasma hominis infection in a lung transplant recipient with bilateral bronchial anastomoses dehiscence and stenosis. The infection was managed using a multidisciplinary approach, including surgical revision, targeted antibiotic therapy, and continuous ventilatory support. Diagnosis and timely treatment of this organism is challenging due to its rare nature, difficult detection in conventional cultures, and innate resistance to beta-lactams, highlighting the importance of screening and monitoring with PCR in transplant centers.

Pulmonary angioplasty: A step further in the continuously changing landscape of chronic thromboembolic pulmonary hypertension management

Summary: CTEPH, classified within group 4 by the WHO, is a potentially fatal and frequently undiagnosed form of pulmonary hypertension that develops as a sequel of acute pulmonary embolism. The standard treatment for CTEPH is surgical pulmonary endarterectomy, while balloon pulmonary angioplasty has shown promising results for technically inoperable patients. Issues such as underdiagnosis, complex operability assessment, and diversity in treatment techniques still need to be addressed.

Summary: This study examined the viable bacterial community in bronchoalveolar lavage samples from 64 lung transplant recipients and identified four distinct compositional states, called "pneumotypes," which were associated with immune activity, lung function, and risks of infection and rejection. The findings highlight the link between the lung microbial ecosystem, human lung function, and clinical stability post-transplant.

Comparison of Risk Stratification Scores in Pulmonary Arterial Hypertension: A Monocentric Retrospective Study at Lausanne University Hospital

Summary: Risk assessment is crucial in the management of pulmonary arterial hypertension (PAH). This study compared the predictive value of 3 risk assessment tools for transplant-free survival (TFs) at 3 and 5 years after diagnosis, and found that the scores were able to predict survival up to 5 years after diagnosis. The study also indicated that the predictive value of the scores increased after 1 year of treatment, supporting their clinical use and treatment efficacy.

Carbon Monoxide Diffusion Capacity as a Severity Marker in Pulmonary Hypertension

Risk Stratification in Pulmonary Arterial Hypertension, Update and Perspectives

Summary: Risk stratification is crucial in assessing patient prognosis in pulmonary arterial hypertension (PAH) and can be determined using validated risk assessment scores. The 2022 ESC/ERS guidelines highlight the importance of risk stratification not only at baseline but also during follow-up. Achieving a low-risk status has become the therapeutic goal, emphasizing personalized therapy. The application of these guidelines is also important in determining the timing for lung transplantation referral.
