Conflicting results have often been observed for the prognosis of hepatocellular carcinoma (HCC) patients, but few studies have attempted to explore the reasons for the conflicting results. We aimed to distinguish the prognosis of patients with HCC with cirrhosis (HCC-C) and that of patients with HCC without cirrhosis (HCC-NC). Patients with hepatitis B virus (HBV)-associated HCC treated by curative liver resection at a single institution between 1995 and 2013 were retrospectively evaluated. Kaplan-Meier and multivariate analyses were performed to identify risk factors, including tumor-related factors, hypoxia-inducible factor 1 alpha expression, HBV X protein (HBx) expression, and HBx double mutations for overall survival and recurrence-free survival in these patients. The long-term prognosis of HCC-NC patients is better than that of HCC-C patients. Male sex, poor differentiation, preoperative serum alanine aminotransferase level greater than 80 IU/L, and alpha-fetoprotein level greater than 400 ng/mL were risk factors for overall survival among HCC-NC patients but not among HCC-C patients, and age greater than 50 years was associated with poor overall survival only in cirrhotic patients. HCC-C patients benefit more from antiviral therapy following curative hepatectomy than do HCC-NC patients. The clinical value of the biomarkers hypoxia-inducible factor 1 alpha, HBx, and HBx double mutations for predicting HCC prognosis was significantly different between these two groups. There were differences in tumor-related prognostic factors, effectiveness of the antiviral therapy after hepatectomy, and biomarkers between HCC-C and HCC-NC patients, indicating that subgroup analysis of the prognostic factors may result in better management of HCC and that HCC patients, especially those with liver cirrhosis, should be given antiviral therapy.

Single-cell landscape of the ecosystem in early-relapse hepatocellular carcinoma

Yunfan Sun, Liang Wu, Yu Zhong, Kaiqian Zhou, Yong Hou, Zifei Wang, Zefan Zhang, Jiarui Xie, Chunqing Wang, Dandan Chen, Yaling Huang, Xiaochan Wei, Yinghong Shi, Zhikun Zhao, Yuehua Li, Ziwei Guo, Qichao Yu, Liqin Xu, Giacomo Volpe, Shuangjian Qiu, Jian Zhou, Carl Ward, Huichuan Sun, Ye Yin, Xun Xu, Xiangdong Wang, Miguel A. Esteban, Huanming Yang, Jian Wang, Michael Dean, Yaguang Zhang, Shiping Liu, Xinrong Yang, Jia Fan

Summary: This study showed that early-relapse hepatocellular carcinoma (HCC) tumors exhibit innate-like CD8(+) T cells with overexpression of KLRB1 (CD161) and a low cytotoxic state, unlike the exhausted state seen in primary HCC. These cells were associated with a worse prognosis. Additionally, potential immune evasion mechanisms were identified in recurrent tumor cells, which dampen dendritic cell antigen presentation and recruit innate-like CD8(+) T cells.

Summary: This study investigated the value of circulating tumor cells (CTCs) as predictors of tumor recurrence following liver transplantation (LTx) in hepatocellular carcinoma (HCC) patients. Results showed that post-operative CTC count >= 1 per 5 mL blood could be a potential biomarker for predicting tumor recurrence, especially in patients with no detectable CTCs before LTx and negative tumor serological markers. Moreover, post-operative serial CTC detection may be useful in post-surgical surveillance for HCC recurrence.

Circulating tumor cell detection and single-cell analysis using an integrated workflow based on ChimeraX(R)-i120 Platform: A prospective study

Peng-Xiang Wang, Yun-Fan Sun, Wei-Xiang Jin, Jian-Wen Cheng, Hai-Xiang Peng, Yang Xu, Kai-Qian Zhou, Li-Meng Chen, Kai Huang, Sui-Yi Wu, Bo Hu, Ze-Fan Zhang, Wei Guo, Ya Cao, Jian Zhou, Jia Fan, Xin-Rong Yang

Summary: The ChimeraX(R)-i120 platform successfully combines CTC detection with single-cell molecular analysis, demonstrating high sensitivity, accuracy, and reproducibility. High CTC-positive rates were found in five types of cancer, while CTCs were rarely detected in healthy donors. In hepatocellular carcinoma patients, CTC status was significantly associated with tumor characteristics, prognosis, and treatment response.

Summary: This study used single-cell RNA sequencing to analyze circulating tumor cells from liver cancer patients, revealing that the transcriptional dynamics of CTCs are associated with stress response, cell cycle, and immune evasion signaling. Additionally, it was identified that CCL5 plays a crucial role in immune evasion of CTCs.

Summary: This study extends the risk-adjusted monitoring approach to include both average surgical risk changes and the existence of variance, leading to improved monitoring efficiency. Simulation results show that the proposed method is more efficient in detecting changes in surgical performance variance and small shifts in risk.

Aberrant cholesterol metabolic signaling impairs antitumor immunosurveillance through natural killer T cell dysfunction in obese liver

Wenshu Tang, Jingying Zhou, Weiqin Yang, Yu Feng, Haoran Wu, Myth T. S. Summary: Obesity and hypercholesterolemia are major risk factors for hepatocellular carcinoma. This study found that obesity leads to hepatic cholesterol accumulation, which suppresses natural killer T (NKT) cell-mediated antitumor immunosurveillance. By lowering cholesterol and inhibiting cholesterol biosynthesis, the function of NKT cells can be restored to prevent HCC development promoted by obesity.
